EXISTING FACILITIES: MODIFICATION AND RENEWAL APPLICATIONS 1. Are the design, maintenance and operation of the treatment facilities adequate for the type of waste and disposal system? ® Yes or ❑ No If no, please explain: Description of existing facilities: This facility has not actively operated since October of 2008. The last application event was December of 2016. The facility has been up for sale for several years. The property was recently sold to Linco Properties in October 2018. Proposed flow: 60,000 GPD Current permitted flow: 60,000 GPD Explain anything observed during the site visit that needs to be addressed by the permit, or that may be important for the permit writer to know (i.e., equipment condition, function, maintenance, a change in facility ownership, etc.) Since the sale, the irrigation system has been isolated form the storage tank so the liquid now in the tank is not able to be transferred to the spray irrigation application fields. 3. Are the site conditions (e.g., soils, topography, depth to water table, etc.) maintained appropriately and adequately assimilating the waste? Has the site changed in any way that may affect the permit (e.g., drainage added, new wells inside the compliance boundary, new development, etc.)? ® Yes or ❑ No If yes, please explain: The new property owner has no plan to re -activate the spray irrigation system currently in place. The new property owner has a pre-treatment permit to transfer wastewater to the Town of Lincolnton WWTP. 5. Is the residuals management plan adequate? ❑ Yes or ❑ No If no, please explain: 6. It should be removed from the permit description. 10. ADDITIONAL REGIONAL STAFF REVIEW ITEMS The property has recently been sold from National Fruit to LINCO Properties. The renewal of this permit is to provide time and coverage for the new owner to be able to properly_ perform the required activities to be able to close the existing, spray irrigation system and eventually rescind the permit. The new owner has been provided with the name change form and been instructed to complete this paperwork immediately so the permit can be renewed. Linco has also been provided with a copy of the existing permit, the WWIS 11-13. Docx for irri atg i�ystem for irrigationystem closure and links to these documents to be used for system closure. The permit has been discussed with Central Office Non -Discharge Permitting Group and CO will request the additional information. FORM: WQROSSR 04-14 Page 3 of 3
