Mitchell County Dear Mr.Dulaney: In accordance with 15A NCAC 2H.0107(b), additional information requested for an incomplete application must be received within 60 days or the application may be returned.An additional information request was received by Blue Ridge Regional Hospital,Inc. on June 21, 2018,which is greater than a 120 days ago.We must hereby return the attached permit application received on May 11, 2018. After a preliminary review in accordance with N.C.G.S. 143-215.1 and 15A NCAC 21.0105,the Division has determined that the application package lacks the following items: 1. Please Complete an Engineering Alternatives Analysis [G.S. § 143-215.1(b)(5)(a)and 15A NCAC 02H .0105(c)(2)] by addressing each of the following: a. Please document that connection to a municipal or regional sewer collection system is infeasible.Please attach correspondence(letter or e-mail) from the Town of Spruce Pine indicating that the Town will not except the discharge to their collection system.The Town may want to site their sewer use ordinance to support their assertion. b. The cover letter received on May 11,2018 indicated that 4 acres of land would be needed to utilize spray irrigation as an alternative.Please provide a Present Value of Costs Analysis(PVCA) as outlined in the Division's "Engineering Alternatives Analysis(EAA) Guidance Document"for spray irrigation. An estimate for the cost of purchasing 4 acres of land can be included with the analysis. c. If subsurface disposal alternatives such as a nitrification field, an infiltration gallery, injection wells, etc. are technically feasible with the purchase of additional land,please re-evaluate these alternatives. Otherwise, indicate why each of these subsurface alternatives are infeasible. Please include a PVCA Summary Table.The EAA must include a Summary Cost Table,which summarizes present worth costs developed for all technologically feasible wastewater alternatives.The summary should include a breakdown of capital costs and recurring costs. In some situations,the Division may require the applicant to refine cost estimates for some alternatives, or possibly collect actual soil data to better characterize the land application alternative.Ultimately,the final determination on cost effectiveness is made by the Division with consideration of monetary costs as well as potential environmental impacts.Please do not overlook adding the NPDES discharge alternative cost to the summary table for comparison. 2. Other supporting information for the EAA: a. Impaired waters and TMDLs: Certain waterbodies listed as impaired on the 303(d)list and/or subject to impending TMDLs may have wastewater discharge restrictions.The list of 303(d) impaired waters is located on the DWR website, or you may contact the DWR Modeling and Assessment Branch(http://portal.ncdenr.org/web/wq/ps/mtu). b. Presence of Endangered Species: If endangered species are present in the proposed discharge location,there may be wastewater discharge restrictions.Endangered species information may be included in the Basinwide Water Quality Plan,or you may contact the U.S.Fish and Wildlife Service(919-856-4520),N.C.Wildlife Resources Commission(919-733-3633),or the N.C.Natural Heritage Program(919-733-7701). c. Complete and return the Attachment A.Local Government Review Form. See EAA Guidance for detailed instructions. 3. Please demonstration approval to discharge to the storm sewer.A map included with the additional information received on May 11,2018 indicated a flow path that discharges to a storm sewer.Please provide documentation that approval has been granted from the appropriate landowners and/or agencies(e.g.the Town or DOT). If you wish to reapply for an NPDES permit,correct these items before resubmitting the application.Please note that permit fees are nonrefundable [15A NCAC 02H.0105 (b)(1)].
