Describe the existing conditions on the site and the generel land use in the vicinity of the project at the time of this application:* The esting conditions on the site consist of a primary road facility (NC 172) within a 100-foot right-of way. The facility is mainly a two-lane facility with grassed shoulders and ditch sections. One existing section has three lanes. The general land use in the vicinity consists of commercially and residentially developed properties as v>II as parcels which are undeveloped. The undeveloped area consists of both cleared and forested parcels. 4b. List the total estimated acreage of all existing wetlands on the property: 2.11 4g. List the total estimated linear feet of all existing streams on the property: 323 4h. Explain the purpose of the proposed project:" The purpose of this project is to address a pattern of rear end crashes, frontal impact crashes, and lane departure crashes along this segment of NC 172. Between 2008 and 2013 there were 169 total crashes including 2 fatal crashes in this section of NC 172. A three-lane facility will reduce the amount of rear-end crashes by shifting turning vehides out of the through lane and into the center turn lane. This will reduce queuing of traffic in Ihe through lane. 4i. Describe the overall project in detail, including indirect impacts and the type of equipment to be used:* This project is on NC 172 from NC 210 to the Camp Lejeune Gate in Onslow County. Specifically, this project proposes widening of travel lanes, the addition of a center turn lane, drainage, curb and gutter, a right turn lane at SR 1557 (Wheeler Creek Road), and resurfacing from east of NC 210 to approximately 600 feet north of SR 1678 (Old Ferry Road). This project also includes resurfacing only on NC 172 from appropmately 600 feet north of SR 1678 (Old Ferry Road) to the Camp Lejeune gate (not induding the bridge deck). Typical road building and construction equipment will be used including excavators, dump trucks, motor graders, and milling and paving equipment. 4j. In several locations, 2:1 slopes were used where possible to minimize impacts to wetlands. 3. The proposed ditch (from 90+00 to 103+04 RT) was designed on a minimum slope to minimize excavation in wetlands. 1b. Specificallydescribe measures taken to avoid or minimize the proposed impacts through construction techniques: The project will be constructed utilizing approved Best Management Practices for Construction and Maintenance Activities. All permit plans and conditions will be enforced so that the amount of impacts is limited to what is enumerated in this application. 2.
