Attn: Dallace Mayhew 5240 Highway 96, Suite 104 Youngsville, NC 27596 Project Name: Lot 15 of Meadowland Mini Farms Franklin County On April 4, 2006, the 401 Oversight/Express Permitting Unit of the Division of Water Quality received a request from you regarding a project known as "Lot 15" for acceptance into the Express Review Program. This letter advises you that your project will be accepted into the Express Review Program once the following items are received: 1) Stream Origin Determination Services: a. This will require an application fee of $200.00 [for Stream Origin Determination services], made payable to the North Carolina Division of Water Quality; b. Four (4) complete and collated copies of the most recent versions of the NRCS Soil Survey map and the 1:24,000 USGS Topographic map for the site, both with the property boundaries located; and c. A vicinity map clearly marking the location of the site. 2) 401 Water Quality Certification and Riparian Buffer Authorization: a. This service will require an application fee of $3000 ($1000 for minor impacts for 401 Certification and $2000 for the Riparian Buffer Authorization). b. Five (5) complete and collated copies of the PCN Application; c. One (1) copy of the Addendum; d. Five (5) copies of all site plan information pertaining to this project (please refer to the Addendum http://h2o.enr.state.nc.ustncwetlands/appadd5.pdf ). Please keep in mind that the clock for this project will not start until receipt of the completed application package and required fees are received. Since the two services have different timelines (stream calls are a 9-day turnaround and the Certification/Buffer Authorization are a 30-day clock, you are advised to submit these separately). If the services required exceed those indicated on the Initial Interest Form, you may be required to remit an additional fee.
