HomeMy WebLinkAboutNC0031828_Fact Sheet_20181004FACT SHEET FOR EXPEDITED PERMIT RENEWALS
This form must be completed by Permit Writers for all expedited permits which do not require
full Fact Sheets. Expedited permits are generally simple 100% domestics (e.g., schools,
mobile home parks, etc) that can be administratively renewed with minor changes, but can
include facilities with more complex issues (Special Conditions, 303(d) listed water, toxicity
testing, instream monitoring, compliance concerns).

Facility Overview:
The design capacity of the treatment system is 0.30 MGD. No significant changes have
been made to this facility since the last renewal. The facility consists of the following
wastewater treatment units:
• Manual bar screen
• Grit removal
• Two (2) parallel mechanically operated aerated oxidation ditches
• Three (3) parallel secondary clarifiers
• Tertiary sand filters
• Alum feed for phosphorus removal
• Polymer feed for alum sludge conditioning
• Gas chlorination
• Chlorine contact chamber
• Dechlorination
• Cascade post aeration
• Automatic samplers
• Flow meter
• Aerobic sludge digesters
• Sludge drying beds
Compliance History (April 2013 to April 2018):
• 1 NOD and 4 NOVs for BOD limit violations
• 1 NOD and 13 NOVs for fecal coliform limit violations
• 1 NOD, 8 NOVs, and 1 enforcement case for ammonia nitrogen limit violations
• 2 NODS and 3 NOVs for DO limit violations
• 1 NOD and 2 NOVs for total phosphorus limit violations
• 1 NOV for TRC monitoring violation
• 1 NOV for pH monitoring violation
Mercury analysis completed 2/9/2018 — not submitted with the renewal application
(which was received 12/14/2017) as required by the permit, but has been considered
received for the renewal as the analysis was done within 12 months of permit expiration
(permit expired 4/30/2018).

Changes from previous permit to draft:
• Added eDMR language in A(1) and as A(6)
• Updated outfall map
• Added regulatory citations
• Updated facility address on cover sheet and Supplement to Permit Cover Sheet
per the renewal application
• Added stream characteristics on Supplement to Permit Cover Sheet
• Removed former footnote #4 stating "Total Residual Chlorine limits and
monitoring requirements apply only if chlorine is used in the treatment process.
See Section A. (5) for instructions should the facility's permitted UV disinfection
system fail and an alternate means of disinfection is required" as renewal
application indicated that gas chlorine disinfection is used.
• Added "All samples shall be from a representative discharge event" under
effluent table in A(1) to match language used in other permits
• Updated A(2) to reflect monthly monitoring
• Removed former A(5) Temporary Means of Disinfection as facility does not use
UV disinfection, but uses chlorine gas disinfection
Changes from draft to final:
• Facility class updated from WW-2 to WW-3 per Wastewater Operator

Comments received on draft permit:
• Maureen Kinney (Wastewater Operator Certification Group; via email 6/28/18):
o Will need to be reclassified to WW-3 due to the phosphorus nutrient limit.
o Maureen comments to WaRO (via email 6/28/18): I noticed this WW-2
classified facility has a Phosphorus Limit wouldn't that bump it to a WW-
3 with a nutrient limit?
o WaRO response to Maureen (Robbie Bullock via email 6/28/18): That is
correct. Currently, their backup is a grade 1 and he will need to be a grade
2.
