Melton, Engineering Manager Aqua North Carolina, Inc. 202 MacKenan Court Cary, NC 27511 Subject: Application Returned Application No. WQ0039815 Westfall Phase 4A LPSS Dear Mr. Melton: This letter is in reference to your permit application received February 19, 2018 for the construction and operation of the subject sewer extension. In an email sent May 1, 2018, by Monti Hassan,your engineer was asked to provide additional information (copy attached) in accordance with 15A NCAC 02T.0107(e). As of today, even after a follow-up email sent August 1,2018,this information has not been received,therefore the Division must return your application as incomplete in accordance with 15A NCAC 02T.0107(e)(2). Please be advised that construction and/or operation of wastewater collection, treatment, and/or disposal facilities without a valid permit is a violation of North Carolina General Statute§143-215.1 and may subject the owner/operator to appropriate enforcement actions in accordance with North Carolina General Statute§143- 215.6A-6C. Civil penalties of up to$25,000 per day per violation may be assessed for failure to secure a permit required by North Carolina General Statute§143-215.1. When you have obtained the requested information that is needed to make your application package complete,you may submit a new application package with the appropriate permit processing fee. As the additional information request email was sent on May 15t, it is now several months overdue. In accordance with 15A NCAC 02T.0107,please provide the additional or revised information for the following comments. General Review: 1. Please provide a set of revised,final plans and specifications on digital media(CD or other digital format)for our records. 2. We noticed that the provided copy of Certificate of Public Convenience and Necessity(CPCN)was granted to Heater Utilities in 2007, even though it appears that Heater Utilities was acquired by Aqua America in 2004. Please provide an updated CPCN that is granted to Aqua America,if available. 3. Our records indicate that we have not yet received certifications for the downstream permitted facilities Phase 2A(WQ0036907)and Phase 1A,1B,1C(WQ0030680). However,it appears that these phases may be in service as they are downstream of this extension. Please provide certifications as soon as possible for these phases if they have been constructed and are in service,per 15A NCAC 02T .0116 and permit condition 7. FTSE: 1 Please resubmit FTSE using recent version(FTSE 04-16 available on PERCS website),as the following information need to be updated or added. a. FTSE must be signed by proper signatory authority,per 15A NCAC 02T.0106(b),as Jacob R.Mueller does not appear to be as the signatory authority in our database . b. FTSE listed WWTP actual flow as 0 which does not seem to be correct c. Please correct total flow for this specific request,as it appears to be incorrect d. Update total actual and obligated flows to the facility and percent of permitted flow used e. Provide downstream sewer information,as it appears to be missing Plans/Specifications: 1. It appears that the plans listed the proposed project area is in the flood hazard area,whereas the application stated that the facility is 251 feet above the mean sea level. Please clarify. 1 I. . . .v- 2. r2. Section 02600,Part 3,Item#C of the provided specifications states that the pressure test shall occur five days after the pouring of the thrust blocking. Per the Minimum Design Criteria,this should be seven days. Please revise. Calculations: 1. Please provide alternative sewer modeling information/calculations indicating minimum scouring velocity of 2 feet per second within the proposed sewer extension,per 15A NCAC 02T.0305(j). 2. Please provide pump curve for the selected pump(s)with operating point clearly marked. 3. In the provided hydraulic modeling, 450 GPD per pump was used which does not seem to be correct. Please clarify. a. Please provide calculations of how maximum flow per pump(gpm)was calculated in column 6 of the table. b. Please revise the force main lengths for zone 1 and zone 2 used in the modeling,as the lengths do not appear to match the application and plan 4 All engineering design,calculations/analysis must be signed,sealed,and dated by a professional engineer,per 15A NCAC 02T .0105(C)(2). A response to the above comments is considered required in ordered to proceed with the review of the application and issuance of the permit. If a complete response is not received within 30 days, the application and supporting documents will be returned to you in accordance with 15A NCAC 02T.107(e)(2).
