Cafferata OCT 17 2018 2523 Nealwood Ave Water Resources Graham,NC 27253 permitting Section SUBJECT: Compliance Evaluation Inspection Report Certificate of Coverage NCG550263 under NPDES General Permit NCG550000 Facility Located at 2523 Nealwood Ave,Alamance County Dear Mr. Cafferata: On September 28, 2018, Paul DiMatteo of this office conducted a Compliance Evaluation Inspection on the subject wastewater treatment facility. The Division is required to inspect these types of systems at least every 5 years. An inspection summary follows. Site Review The system consists of a septic tank, subsurface sand filter, chlorinator and outfall with final disposal to an unnamed tributary to Big Alamance Creek,class WS-V;NSW waters in the Cape Fear River Basin.No sewage malfunction was observed at or around the septic tank. There was no chlorine in the chlorinator. Please be sure to check the chlorinators regularly and ensure they are stocked with tablets labeled for wastewater treatment. The outfall could not be located. The outfall should be inspected twice per year and kept visible. Documentation & Monitoring Review A review of documentation was not conducted at the time of inspection. We would like to remind you to conduct and document the following: - Effluent monitoring must be performed as required in the permit. - All system components must be maintained at all times and in good working order. - Inspect the septic tank and secondary tanks at least yearly to determine if solids must be removed. - Pump the septic tank every 5 years, or when the solids level is found to be more than 1/3 of the liquid depth in any compartment, whichever is greater. All tanks should be emptied of their contents whenever any of the tanks meet this requirement. - Contents removed from septic tanks shall be disposed of at a location and in a manner compliant with all local and state regulations. Please be aware that violations of your NPDES Certificate of Coverage,or the NC statutes and regulations under which it is promulgated, are subject to fines of up to $25,000 per day, per violation, as set forth in NC General Statute 143-215.6A, Enforcement Procedures, Civil Penalties. I walked all around the culvert at Nealwood Ave, upstream and downstream, and didn't see it. I left a copy of the general permit highlighting chlorination, outfall observation and monitoring requirements on the door. Page# 2 - Permit.
