Dear Mr. Thompson,
On October 3, 2018 Kelli Park, of this office, conducted a compliance evaluation inspection on the
wastewater treatment system located at the above address. The State requires the Division of Water
Resources to inspect these types of systems every five years.
The inspection revealed that no wastewater facility was constructed. Please remember that before you
install a wastewater system you must receive an Authorization to Construct from the Division, which
includes submittal of plans for the system.
