Hi Bethany,
I did print out the permit, but then noticed our old company name is still of on the document. I think it was back in May
when I received the letter and website to update our information, and follow the directions to ensure the name was
changed. Not sure what happen, but is there any way to change the company name from Surface Guard Films LLC to
Pregis Polymask?

And enter your COC Number into the first box. Fill out the other required fields for the submittor, sign the form at the
bottom, and submit. You will receive an email with your COC.

we are under, but did not give you the COG number. That number is NCG050327.
