On May 22, 2005 the Wetlands/401 Unit of the Division of Water Quality received your permit modification
request dated May 18, 2006, for Forsyth Country Club project in Forsyth County, North Carolina. According to
your letter, the request modifies the culvert system footprint, but not the previously approved linear feet of stream
impact. Therefore, the DWQ will not require a new application and appropriate fee to approve your requested
modifications. Please contact Daryl Lamb in the DWQ Winston-Salem Regional Office for approval of your
requested modification.

To whom it may concern:
An application for a Pre-Construction Notification (PCN) for the referenced maintenance road culvert was
made to NC DWQ and USACE on January 27, 2006. The original application was approved by NC DWQ and
a Section 401 Water Quality Certification was issued (DWQ# 06-0299). The USACE rejected the proposed
culvert plan and through subsequent negotiations, a revised culvert plan was prepared which was approved by
the Corps. This revised plan impacts the same linear feet of stream (-130 LF), but reduces the width of the
culvert system footprint. The Winston-Salem Regional DENR Office is aware of this revised plan and will let
the previously issued approval stand. We are herewith providing 5 copies of the revised plan to update the
application previously submitted and approved.
An application has been submitted and is currently under review by the Federal Emergency Management
Agency (FEMA) to update the Flood Insurance Rate Map flood mapping and account for the impacts of this
project.
