The Stormwater Management Plan approval was based upon calculations that indicate riprap aprons of 16 feet and 22 feet below the stream culvert outlets. The 404/401 permitted impacts do not include these riprap apron lengths. This discrepancy will need to be resolved. Please contact me at your convenience to discuss this further. In addition, I recently inspected the site and there was evidence of sediment loss along the entrance road into the adjacent stream/wetland complex. The sediment/erosion control measures need immediate attention/maintenance along the entire length of the road, and the stormwater outfalls along the road need to be cleaned out and restored as approved in the Stormwater Management Plan. I have attached my inspection report. Thanks, Sue Homewood Division of Water Resources, Winston Salem Regional Office Department of Environmental Quality 336 776 9693 office 336 813 1863 mobile Sue. Impact #3 is installed at a 90 degree angle with the stream, very likely to cause future instability. Stormwater outlets are not in compliance with design/plans, riprap at outlets are not installed and/or too short, also stormwater outlets have not been maintained, are full of sediment, and sediment loss is evident. Indications of instability and sediment loss along road fill into stream and adjacent wetlands. page: 2
