This inspection was conducted to verify that the facility is operating in compliance with the conditions and limitations specified in Non-discharge Permit No. WQ0017530. The findings and comments noted during this inspection are provided in the enclosed copy of the inspection report entitled "Compliance Inspection Report". Please respond in writing to this office within 10 days of your receipt of this letter regarding your plans or measures to be taken to address the following issues: 1. Verify the 2016 Flow Reduction Request was submitted. 2. Verify request for new Compliance Boundary for water supply wells. Jerry West, Jason Hopfinger, Mark Teague and Dale Wike assisted with the inspection. The following items noted in the previous inspection and Notice of Violation have been addressed: 1. The Operation & Maintenance Plan has been updated. It is recommended that specific safety requirements for operators be added. 2. The nozzles in the tertiary filter underdrain have been replaced. 3. Safety Concern: The grates over the influent to the WWTP and the right-side train aeration basin have been replaced or painted to control rust. 4. An exterior elbow pipe (pvc) located between the tertiary filter basin and the right-side train clarifier has been replaced. 5. An exterior elbow pipe (steel) located at the influent splitter box on the left-side train of the WWTP has been painted. 6. The racks used to hold UV bulbs in place have been pulled. The contract operator is actively looking for replacement racks. In February/March 2016, Highlands Cove inquired about adding flow to the WWTP. The request was reviewed by the Asheville Regional Office and Nathaniel Thornburg. It was determined that the facility would be able to handle the additional flow. A flow reduction request was to be submitted to the PERCS Unit in Raleigh. Please verify that the flow reduction request was submitted. In addition, the most current facility site map includes re-drawn Compliance Boundaries (CB) around water supply wells. A request should be submitted to show the change in the CB. The request should be submitted by someone with signature authority to the Non-Discharge Permitting Unit. Is contact chamber free of sludge, solids, and growth? If UV, are extra UV bulbs available? If UV, is the UV intensity adequate? # Is it a dual feed system? Does the Stationary Source have more than 2500 lbs of Chlorine (CAS No. 7782-50-5)? If yes, then is there a Risk Management Plan on site? If yes, then what is the EPA twelve digit ID Number? (1000-____-____) If yes, then when was the RMP last updated? The UV panel is operational. The contract operator is actively looking for repalcement racks for the UV system.
