The attached Environmental Protection Agency inspection form notes the areas that were evaluated for this inspection. The findings and observations are outlined below: I. Permit The NPDES permit became effective August 1, 2015, and expires on February 28, 2020. II. Facility Site Review - The treatment system has not been-built yet. The site is located across from Artists Aly Rd on Highway 221.
