Main Street Mayodan, NC 27027-2019 Subject: Compliance Evaluation Inspection Report NPDES Permit#NC0046302 Mayodan Water Treatment Plant, Rockingham County Dear Ms. Hopper: On July 9, 2018, Paul DiMatteo of the Division of Water Resources(Division),Winston-Salem Regional Office, conducted a Compliance Evaluation Inspection of the above referenced wastewater treatment system. Michael Amos, Operator in Responsible Charge (ORC), and William Amos, backup-ORC,were present for the inspection.This type of inspection consists of two basic parts: an on-site inspection of the facility and a review of facility files and self- monitoring data.Observations from each area of the inspection are summarized below. Site Review 1. Permit The current permit became effective on January 1, 2018 and expires on November 30, 2022. Permitted components include two alternatively utilized settling lagoons and tablet dechlorination with final disposal to an unnamed tributary to the Mayo River,class WS-IV, CA waters in the Roanoke River Basin. • 2. Facility Site Review The facility was as described in the permit.The operators said that the second lagoon has been offline since 2014.There was some vegetative growth and a trickle of water was able to reach the lagoon.The operators said that the vegetation would be cleared before being put back into use and that they would work on better sealing the dive'rter to reduce the amount of water getting through it.The water in the lagoon that was in use appeared very clear and small fish could be observed in it. 3. Operation and Maintenance The plant is operated as required.All operators are properly designated and hold adequate certifications. The facility appeared to be well operated and managed. Flow Measurement Effluent flow is measured instantaneously at the outfall using a bucket and stopwatch.There was no discharge at the time of inspection. 5. Effluent/Receiving Waters There was no discharge at the time of inspection.A path to the outfall was available and there was no visible impact to the receiving stream. Documentation Review 6. Records/Reports Discharge Monitoring Reports (DMRs) and lab data are maintained according to the permit. DMR5 include all permit parameters. Copies of DMRs, operator visitation logs, daily operation sheets, maintenance reports, lab reports and Chain-of-Custody forms were available for review during the inspection. 7. Self-Monitoring Program A review of DMRs from August 2015 through June 2018 showed no actioned violations, and a comparison of submitted data with bench sheets revealed no errors or concerns. 8. Laboratory The facility analyzes field parameters at its on-site laboratory under certification#5289. Non-field analysis is performed by Environment 1, certification#10, and whole effluent toxicity is analyzed by Environmental Testing Solutions,certification#600. Copies of all laboratory documentation were readily available. Procedures for analyzing field parameters including required calibrations appeared acceptable.
