Dear Mr. Shaver and Ms. Steenhuis:
The North Carolina Department of Transportation plans to replace existing (1) 36" x 47' in length which
was over topped during Hurricane Florence and suffered resultant road washout at location Lat: 34.67574,
Long: -77.18571 on SR 1620 Henson Drive 0.1 miles East of SR 15 (Queens Creek Road) in Onslow
County with (1) @ 60" x 44' in length CMP w/ headwalls. NCDOT would like to request a waiver from the
depth specifications of the regional conditions 3.6.1 of the Nationwide Permit 3 and the corresponding
General Condition 11 of the Water Quality General Certification No. 4132. Due to the existing
geomorphologic fall of the stream channel, burying the inverts of the 60" CMP 1' would result in a 4 percent
pipe slope, causing a high flow erodible condition. NCDOT plans to place the pipe on a 3 percent slope
leaving a 4' fall on the outlet end at the existing blow hole ( LS' above natural stream bed downstream of
blow hole). Due to the 4' fall on the outlet end, 8 LF of additional rip rap is proposed to be placed in the
stream channel to protect from scour and destabilization.

By copy of this letter and enclosures, NCDOT is requesting issuance of Nationwide 3 approval from the US
Army Corps of Engineers and 401 Water Quality Certification (WQC) #4132 approval from the Division of
Water Resources (DWR). A method of debiting $240 to process the WQC request has been provided as
noted in the subject line of this application.

The North Carolina Natural Heritage Program (NCNHP) appreciates the opportunity to provide information
about natural heritage resources for the project referenced above.
Based on the project area mapped with your request, a query of the NCNHP database, indicates that there are
no records for rare species, important natural communities, natural areas, and/or conservation/managed areas
within the proposed project boundary. Please note that although there may be no documentation of natural
heritage elements within the project boundary, it does not imply or confirm their absence; the area may not have
been surveyed. The results of this query should not be substituted for field surveys where suitable habitat exists.
In the event that rare species are found within the project area, please contact the NCNHP so that we may

area, the NCNHP recommends contacting the US Fish and Wildlife Service (USFWS) for guidance. Contact
