A. Application:
Item V11.4. There are inconsistencies between the application, engineering plans and the SFR
Loading Rate worksheet.
a. The application indicates there is a 0.53 acre field.
b. The SFR Loading Rate worksheet indicates a 0.33 acre field.
c. The engineering plans show a wetted area roughly 17,000 square feet or 0.39 acres.
Please revise so that the information is consistent throughout the application and recalculate
where necessary.
B. Plat Map:
1. During the site inspection, potential wetland features were identified that were not indicated on
the plans. These features must be delineated and surveyed as well as the surface water features
buffered with a setback distance of 100 feet. Please provide an updated site map with wetland
features (adjacent to the eastern border) delineated. Please revise the drip field layout
accordingly.

C. Plans:
1. The design drawings indicate a stream with a sinuosity and meander belt width lower that what
was observed during the site inspection. The existing stream channel appears to be within the
setback to surface waters of 100 feet. Please survey the top of the stream bank and revise the drip
field layout as necessary by providing an updated site map showing the tributary adjacent to the
southern border as it exists in the field.
2. Please add the design wetted area in square feet to the drip field layout sheet WW-4 as well as
line average spacing distances. Also, include this information on the drip field detail sheet WW-
8.
D. Enzineerin�Calculations:
1. Please provide biological treatment process calculations to support the effluent design quality or
submit effluent data from existing AX25 systems in North Carolina that can demonstrate the
proposed system can meet the minimum level of treatment for systems treating 600 gallons per
day or greater.
E. Operation & Maintenance Plan:
1. Please amend the drip field maintenance section in the O&M Plan to provide recommendations
for turf maintenance which includes occasional cutting with the method of cutting, regular time

application, and any other recommendations to facilitate the inspection of the drip line for
integrity and leaks and to support the fescue as a cover crop. Please note that a final O&M Plan
may be submitted with the Engineering Certification, however, a preliminary O&M Plan shall be
submitted with the application package.
1;: Threatened or Endangered Aquatic Species Documentation:
1. No Threatened or Endangered Aquatic Species documentation was submitted with this
application package. This documentation is required for all application packages per 15A
NCAC 02T .0105(c)(10). Please submit documentation from the Department's Natural
Heritage Program demonstrating the presence or absence of threatened or endangered
aquatic species within the boundary of the wastewater treatment, storage and irrigation
facilities. If the facility directly impacts such species, this documentation shall provide
information on the need for permit conditions in the new permit. The map submitted is
incomplete.
