JAY ZIMMERMAN Director Subject: Attachment A and B Certifications Permit No. WQ0037135 Southern Soil Builders RLAP Wilkes County In accordance with your permit modification request received March 21, 2017, and subsequent additional information received May 15, 2017, we are forwarding herewith modified Attachments A and B, certified June 6, 2017 to Southern Soil Builders, Inc. for the subject residuals program. This certification is being issued to modify Attachments A and B by: 1. Removal of 100 dry tons/year from the Town of Columbus in Polk County; and 2. Addition of 10 fields and 60.6 acres in Rutherford County, those fields being NC-RU-LB-01 to -06; NC-RU-FC-02 and -03; and NC-RU-NM-0I and -02. These modified Attachments A and B shall be effective from the date of certification until June 30, 2019 (i.e. expiration date of the current permit); shall void the previously certified Attachments A and B of Permit No. WQ0037135 with Attachment A certified on August 22, 2016, and Attachment B certified on April 13, 2016, and shall be subject to the conditions and limitations as specified therein and well as those specified in the above -referenced permit. Please replace the Attachments A and B of Permit No. WQ0037135 that was issued on July 3, 2014, with these modified Attachments A and B. Please note that only the maps for any new residuals -generating sources and/or land application sites are attached to this letter. It is the Permittee's responsibility to ensure that a complete permit is created by inserting these documents into the previously issued permit. If any parts, requirements, and/or limitations contained in this certification or modified Attachment A and B are unacceptable, you have the right to request an adjudicatory hearing upon written request within 30 days following receipt of the certification. This request shall be in the form of a written petition, conforming to Chapter 150B of the North Carolina General Statutes and filed with the Office of Administrative Hearings at 6714 Mail Service Center, Raleigh, North Carolina 27699-6714.
