In accordance with the major modification application package received on March 3, 2015, we
are forwarding herewith a modified Attachment B, certified March 26, 2015, to City of Danville for the
subject residuals land application program.
This certification is being issued to modify Attachment B by adding
• two fields (CS-7-1 and CS7-2) belonging to James R. Blackwell with an area of 54.4
acres located in Caswell County; and
• five fields (CS-9-4, CS-9-5, CS-9-5, CS-9-6, CS9-7, and CS-9-8) belonging to Glenn
Hodges, Jr. with an area of 5 1. 6 acres located in Caswell County.
This permit shall be effective from the date of issuance until December 31, 2016 and shall void
Attachment B of Permit WQ0036561 issued August 27, 2014; and shall be subject to the conditions and
limitations as specified therein and well as those specified in the above -referenced permit.
Please replace the existing Attachment B of Permit Number WQ0036561 that was
previously certified on August 27, 2014, with this modified Attachment B. Note that only the vicinity
and buffer maps for any new and modified land application sites are attached to this modified Attachment
B. It is the Permittee's responsibility to ensure that a complete permit is created by inserting this
modified Attachment B and vicinity and buffer maps into Permit Number WQ0036561, issued on August
27, 2014, and retaining any vicinity and buffer maps for any existing and unmodified land application
sites.

As always, remember to take the time to review this modified Attachment B thoroughly, as
some of the conditions contained therein may have been changed since the last certification. Note that
this modified Attachment B is hereby incorporated by reference into Permit Number WQ0036561, issued
on August 27, 2014.
If any parts, requirements or limitations contained in this permit are unacceptable, the Permittee
has the right to request an adjudicatory hearing upon written request within 30 days following receipt of
this permit. This request shall be in the form of a written petition, conforming to Chapter 150B of the
North Carolina General Statutes, and filed with the Office of Administrative Hearings at 6714 Mail
Service Center, Raleigh, NC 27699-6714. Unless such demands are made, this permit shall be final and
binding.

i . A re -defined compliance boundary has been approved for this land application site. This re -defined compliance boundary is located 100 feet from the water
supply well on the property. Note that the 100-foot setback between wells and the land application area shall still be met

approved setback map), regardless of whether a surface or a subsurface application method is used.
4. A reduced setback from a residence under separate ownership has been approved for this land application site. Residuals may be land applied within 100 feet of
the residence owned by James Y. Blackwell at 7199 US Highway 158W, Yanceyville, North Carolina 27379 (i.e., indicated by the label "SWA-A" on the
approved setback map), regardless of whether a surface or a subsurface application method is used.
5. A reduced setback from a residence under separate ownership has been approved for this land application site. Residuals may be land applied within 100 feet of
the residence owned by Roger Henderson at 7307 US Highway 158W, Yanceyville, North Carolina 27379 (i.e., indicated by the label "SWA-D" on the approved
setback map), regardless of whether a surface or a subsurface application method is used.
6. A reduced setback from a residence under separate ownership has been approved for this land application site. Residuals may be land applied within 100 feet of
the residence owned by harry Woodall (i.e., indicated by the label "SWA-E" on the approved setback map), regardless of whether a surface or a subsurface
application method is used.
7. No residuals shall be applied until the Winston-Salem (NC) Regional Office (phone number: 336-776-9800) has inspected the fields and provided a written notification of
approval.
