Following the issuance of the subject permit December 19, 2011 it was discovered that an expiration date
was erroneously incorporated in the permit text. As such, we are :forwarding herewith the corrected Page
1 and Page 2 of Permit No. WQ0035642, last issued December 19, 2011, for the subject facility.
Additionally two new conditions have been added to the permit for consistency with current permit
language requirements. The following corrections have been made to the permit:
1. Expiration date of November 30, 2016 has been removed.
The following new conditions have been added to the permit under 11. Performance Standards:
Reclaimed water distribution lines shall not be less than 100 feet from a well unless the piping and
integrity testing procedures meet water main standards in accordance with 15A NCAC 18C, but in
no case shall they be less than 25 feet from a private well or 50 feet from a public well.
2. Reclaimed water distribution lines shall meet the separation distances to sewer lines in accordance
with Rule .0305 of Subehapter 02T.

In accordance with the provisions of Article 21 of Chapter 143, General Statutes of North Carolina as
amended, and other applicable Laws, Rules and Regulations
PERMISSION IS HEREBY GRANTED TO

FOR THE
construction and operation of a reclaimed water utilization system extension consisting of: 1,965 linear
feet (LF) of 10-inch PVC and/or ductile iron reclaimed water line, aad all associated piping, valves and
appurtenances;
to serve the Ocean Ridge WWTF, with no discharge of wastes to surface waters, pursuant to the
application received October 24, 2011, and in conformity with the project plans, specifications, and other
supporting data subsequently filed and approved by the Department of Environment and Natural
Resources and considered apart of this permit.

1:. SCHEDULES
In accordance with 15A NCAC 02T .0116, upon completion of construction and prior to operation of
the new reclaimed water distribution facilities, a certification (attached) shall be submitted from a
licensed North Carolina Professional Engineer certifying that the permitted facility has been installed
in accordance with this permit, Division approved plans and specifications, and other supporting
documentation, including the location of all monitoring wells as applicable. If this project is to be
completed in phases and partially certified, the Permittee shall retain the responsibility to track further
construction approved under the same permit, and shall provide a final certificate of completion once
the entire project has been completed. Mail the Certification to the Division of Water Quality,
Aquifer Protection Section, 1636Mail Service Center, Raleigh, NC 27699-1636.
2. Operation of this reclaimed water distribution line shall be contingent upon proper construction and
operation of approved reclaimed water distribution lines immediately upstream and downstream of
the line approved herein. Future upstream and downstream reclaimed water distribution lines
specifications shall include quality assurance testing procedures, and those procedures shall be used
prior to operation of this reclaimed water distribution line.
H. PERFORMANCE STANDARDS
The subject reclaimed water facilities shall be effectively maintained and operated at all tinges so
there is no discharge to surface waters, nor any contravention of groundwater or surface water
standards. In the event the facilities fail to perform satisfactorily, including the creation of nuisance
conditions due to improper operation and maintenance, or failure of the utilization areas to adequately
assimilate the reclaimed water, the Permittee shall take immediate corrective actions including
Division required actions, such as the construction of additional or replacement reclaimed water
distribution facilities.
2. This permit shall not relieve the Permittee of their responsibility for damages to groundwater or
surface water resulting from the operation of this reclaimed water distribution line.
Construction of the reclaimed water distribution line permitted herein shall be scheduled as not to
interrupt service of existing utilities, or result in an overflow or bypass discharge of reclaimed water
to surface waters of the State.
3. Reclaimed water distribution Iines shall not be less than 100 feet from a well unless the piping and
integrity testing procedures meet water main standards in accordance with 15A NCAC ISC, but in no
case shall they be Iess than 25 feet from a private well or 50 feet from a public well.
4. Reclaimed water distribution lines shall meet the separation distances to sewer lines in accordance
with Rule .0305 of Subchapter 02T.
