In accordance with your permit modification request received on October 8, 2012 and subsequent
additional information received electronically on October 17, 2012, we are forwarding herewith a
modified Attachment A, Version 1.4, certified October 26, 2012, to Granville Farms, Inc. for the subject
residuals land application program.
This certification is being issued to approve the Town 'of Williamston Wastewater Treatment
Plant as an additional -residuals source. The tonnage from this new source is 300 dry tons per year, and it
.is intended to be used as a supplemental source. Please note that if Granville Farms intends to land
apply from all the sources listed in Attachment A during the same year, there is not adequate land
application area available to apply residuals below agronomic rates as required by Permit Condition
II 10.
Please replace the existing Attachment A of Permit No. WQ0035595 that was previously
certified on April 12, 2012, with this modified Attachment A. It is the Permittee's responsibility to
ensure that a complete permit is created by inserting this modified Attachment A into Permit No.
WQ0035595, issued on April 12, 2012.
This modified Attachment A shall be effective from the date of certification; shall void
Attachment A of Permit No. WQ0035595 that was previously (most recently) certified on April 12, 2012;
and shall be subject to the conditions and limitations as specified therein and well as those specified in the
above -referenced permit. Note that this modified Attachment A is hereby incorporated by reference into
Permit No. WQ0035595, issued on April 12, 2012.

If any parts, requirements, and/or limitations contained in this certification or modified
Attachment A are unacceptable, you have the right to request an adjudicatory hearing upon written
request within 30 days following receipt of the certification. This request shall be in the form of a written
petition, conforming to Chapter 150B of the North Carolina General Statutes and filed with the Office of
Administrative Hearings at 6714 Mail Service Center, Raleigh, North Carolina 27699-6714. Unless such
demands are made, this permit shall be final and binding.

1. Maximum Dry Tons per Year is the amount of residuals approved for land application from each permitted facility.
2. Analyses to demonstrate that residuals are non -hazardous (i.e., TCLP, ignitability, reactivity, and corrosivity) as stipulated under permit Condition IV.2.
3. Testing of metals and nutrients as stipulated under permit Condition IV.3.
4. Analyses of pathogen and vector attraction reductions as stipulated under permit Condition IVA. Permit application indicated that Fecal Coliform Density Demonstration will be performed to meet
pathogen reduction requirements. 30-Day Bench Scale Test and/or Specific Oxygen Uptake Rate Test and/or Injection and/or Incorporation will be performed to meet vector attraction reductions.
However, other methods listed under 15A NCAC 02T .1106(b) and .I 107(a) can also be used to demonstrate compliance with this permit requirement.
5. Monitoring frequencies are based on the actual dry tons applied per year using the table below, unless specified above.

6. Condition I.1. requires the Permittee to submit a permit modification to add more land application area to this permit prior to land applying during the same year from all sources listed in
Attachment A in order to comply with Condition 1I.10.
If no land application events occur during a required sampling period (e.g. no land application occur during an entire year when annual monitoring is required), then no
sampling data is required during the period of inactivity. The annual report shall include an explanation for missing sampling data. Those required to submit the annual
report to EPA may be required to make up the missed sampling, contact the EPA for additional information and clarification.
