JAY ZIMMERMAN Director Subject: Attachment A Certification Permit No. WQ0035207 Synagro Central DCAR Davie County In accordance with your permit modification request received July 18, 2017, we are forwarding herewith a modified Attachment A certified August 4, 2017, to Synagro Central, LLC for the subject residuals program. This certification is being issued to modify Attachment A by: Increasing total residuals dry tonnage per year from 20,568 to 22,968 by addition of Lake Townsend WTP and increase of 400 tons per year for John F. Kime WTP. It was pointed out in the process of assessing Lake Townsend WTP that there may be potential erosion control problems that arise in the future. This is the responsibility of the Lake Townsend WTP permit holder; however, if the Permittee notices the problem during their activities at the facility, notification shall be given to the Winston-Salem Regional Office so the matter can be resolved. This modified Attachment A shall be effective from the date of certification until February 28, 2021 (i.e., expiration date of the current permit), shall void Attachment A of Permit No. WQ0035207 issued November 13, 2015, that was previously certified on March 22, 2017, and shall be subject to the conditions and limitations as specified therein, as well as those specified in the above -referenced permit. Please replace the Attachment A of Permit No. WQ0035207 that was certified on March 22, 2017, with this modified Attachment A. If any parts, requirements, and/or Iimitations contained in this certification or modified Attachment A are unacceptable, you have the right to request an adjudicatory hearing upon written request within 30 days following receipt of the certification. This request shall be in the form of a written petition, conforming to Chapter 150B of the North Carolina General Statutes and filed with the Office ofAdministrative Hearings at 6714 Mail Service Center, Raleigh, North Carolina 27699-6714. Monitoring frequencies are based on the actual dry tons applied per year using the table below, unless specified above. Class A water treatment plant residuals are exempt from meeting the pathogen & vector attraction reduction requirements in Condition IVA, provided they are not mixed with residuals that have been generated during the treatment of domestic wastewater, the treatment of animal processing wastewater, or the biological treatment of industrial wastewater. The Class A water treatment plant residuals must be treated and processed in a manner as to not meet the definition of "Biological Residuals" as defined in 15A NCAC 02T .1102(6). 7. Williams WTP residuals are taken to Brown WTP in Durham, mixed, dried and processed. If no distribution events occur during a required sampling period (e.g. no distribution of residuals occur during an entire year when annual monitoring is required), then no sampling data is required during the period of inactivity. The annual report shall include an explanation for missing sampling data. Those required to submit the annual report to EPA may be required to make up the missed sampling, contact the EPA for additional information and clarification.
