Dear Mr. Fleming:
On September 27, 2018 Mitch Hayes of the Division of Water Resources conducted the subject
inspection. The assistance of Wayne Pore, Kyle Smith, Christopher Sandt, and yourself with this
inspection was appreciated. Below is a list of findings developed from the inspection and review
of the permit:
1. The permit was reissued, became effective May 01, 2013 and will expire April 30, 2021.
BIMS lists the subject collection system as a Class 2 facility.
2. The current Capital Improvement Plan projects through the year 2022. Monies are
approved yearly for projects.
3. The Phase 2 extension project is expected to be completed by the end of October. This
project would divert all flow from the Efland Cheek Collection system to the Town of
Mebane Wastewater Treatment Plant, NC0021474 for treatment and disposal. Once the
Town of Mebane assumes responsibility of all aspects of the Efland Cheek Wastewater
Collection System, please send a request to rescind the Efland Cheek wastewater
collection permit to Deborah Gore, NC Division of Water Resources, Water Quality
Permitting Section, Wastewater Branch- PERCS Unit, 1617 Mail Service Center, Raleigh,
NC 27699-1617. If you have any further questions concerning rescinding the permit,
please contact Deborah Gore at 919.707.3624 or at de borah.gore(a�ncdenr.gov
4. Orange Alamance Water System (OAWS) is responsible for the Fats Oils and Grease
(FOG) program Currently, there are no restaurants on the collection system. Proper
disposal of FOG is available on their web page. Please ensure that some type of
information of FOG disposal is mailed out to customers at least once a year.
5. The map of the collection system is complete and contains all permit requirements. The
map is available on GIS and paper.
6. The following records are documented: right of way maintenance, pump station visitation
and maintenance, annual observation of collection system, high priority line observation
once every six months, and line cleaning. A review of all records is conducted weekly.

7. There are two pump stations in the wastewater collection system. Both pump stations
have secured access, telemetry, audible/visual alarms, notification/information signage,
and on-site generators. Audible / visual alarms, telemetry, and generators were
operational at the time of inspection.
8. An 8" DI creek crossing was inspected. There are no piers, and no indication of bank
erosion. The line appeared straicht. You stated that there are several other sub-water way
crossings. These lines are considered high-priority lines and must be inspected once per
every six-month period.
