REGAN Secretary LINDA CULPEPPER Interim Director Subject: Attachment A & B Certifications Permit No. WQ0034513 Brunswick County RLAP Brunswick County In accordance with your permit modification request received January 12, 2018, and subsequent additional information received March 6, 2018, we are forwarding herewith modified Attachments A and B certified March 8, 2018, to Brunswick County for the subject residuals program. This certification is being issued to modify Attachments A and B by: 1. Addition of one new residual source, Shallotte WWTP (WQ0000798) in Brunswick County totaling 1,500 dry tons (dt) per year. This new source is composed of two Upper Lagoons (798A1 & A2 — 500 dt/year) and one Lower Lagoon (798 B — 1,000 dt/year). 2. Addition of eight new fields totaling 23 5.9 acres in Columbus County, those fields being NC- CO-HH-03 to -07 and NC-CO-HH-12 to -14. Also note that all fields in Brunswick County certified in Brunswick County in the permit issued September 27, 2016 will henceforth have the prefix "NC -BR" added to their name. This allows quick reference to the county in which the field is located. The modified Attachments A and B shall be effective from the date of certification until December 31, 2021 (i.e., expiration date of the current permit), shall void Attachments A and B of Permit No. WQ0034513 issued September 27, 2016, and shall be subject to the conditions and limitations as specified therein, as well as those specified in the above -referenced permit. Please replace Attachments A and B of Permit No. WQ0034513 that was issued September 27, 2016 with the modified Attachments A and B. Please note that only the maps for new land application sites are attached to this letter. It is the Permittee's responsibility to ensure that a complete permit is created by inserting these documents into the previously issued permit. -'Nothing Compares—.. This request shall be in the form of a written petition, conforming to Chapter 150B of the North Carolina General Statutes and filed with the Office of Administrative Hearings at 6714 Mail Service Center, Raleigh, North Carolina 27699-6714. Maximum Dry Tons per Year is the amount of residuals approved for land application from each permitted facility. 2. Analyses to demonstrate that residuals are non -hazardous (i.e., TCLP, ignitability, reactivity, and corrosivity) as stipulated under permit Condition IV.2. 3. Testing of metals and nutrients as stipulated under permit Condition IV.3. 4. Analyses of pathogen and vector attraction reductions as stipulated under permit Condition IVA. 5. Monitoring frequencies are based on the actual dry tons applied per year using the table below, unless specified above. If no land application events occur during a required sampling period (e.g. no land application occur during an entire year when annual monitoring is required), then no sampling data is required during the period of inactivity. The annual report shall include an explanation for missing sampling data. Those required to submit the annual report to EPA may be required to make up the missed sampling, contact the EPA for additional information and clarification.
