In accordance with your permit major modification request received November 5, 2012, and
additional information received November 30, 2012, we are forwarding herewith the modified
Attachment A, certified December 5, 2012, to the County of Brunswick Public Utilities for the subject
residuals management program.
This certification is being issued to modify Attachment A by adjusting the residuals annual
drytonage of the West Brunswick Regional WWTP from 250 to 500 and the Northeast Brunswick
Regional WWTP from 188 to 220. Please replace the existing Attachment A 'of Permit No.
WQ0034468 that was previously certified on December 8, 2011, with this modified Attachment A.
This modified Attachment A shall be effective from the date of certification; shall void
Attachment A of Permit No. WQ0034468 that was previously certified on December 8, 2011; and shall be
subject to the conditions and limitations as specified therein and well as those specified in the above -
referenced permit. Note that this modified Attachment A is hereby incorporated by reference into Permit
No. WQ0034468, issued on February 8, 2010.
If any parts, requirements, and/or limitations contained in this certification or modified
Attachment A are unacceptable, you have the right to request an adjudicatory hearing upon written
request within 30 days following receipt of the certification. This request shall be in the form of a written
petition, conforming to Chapter 150B of the North Carolina General Statutes and filed with the Office of
Administrative Hearings at 6714 Mail Service Center, Raleigh, North Carolina 27699-6714. Unless such
demands are made, this permit shall be final and binding.

1. Analyses to demonstrate that residuals are non -hazardous (i.e., TCLP, ignitability, reactivity, and corrosivity) as stipulated under permit Condition IV.2.
2. Testing of metals and nutrients as stipulated under permit Condition IV.3.

The contributions from Carolina Shores WWTP and Sea Trail WWTP will be combined with the residuals at the West Brunswick Regional WWTP and thus included in the maximum permitted
capacity of 250 dry tons per year of the West Brunswick Regional WWTP. Therefore, the total combined capacity of these three source facilities shall not exceed 250 dry tons per year.
6. These sources are to be hauled to and mixed with West Brunswick Regional WWTP residuals, and will be sampled with West Brunswick Regional residuals.
If no land application events occur during a required sampling period (e.g. no land application occur during an entire year when annual monitoring is
required), then no sampling data is required during the period of inactivity. The annual report shall include an explanation for missing sampling data.
Those required to submit the annual report to EPA may be required to make up the missed sampling, contact the EPA for additional information and
clarification.
