In accordance with the permit modification request submitted on your behalf by The Wooten
Company and your confirmatory email received on May 18, 2011, we are forwarding herewith a modified
Attachment B, certified May 20, 2011, to the Town of La Grange for the subject conjunctive reclaimed
water irrigation system.
This certification is being issued to modify Attachment B to:
♦ Permit an increase in the hourly application rate from 0.2 inches per hour to 0.3 inches per
hour on all three permitted application fields.
Please replace the existing Attachment B of Permit No. WQ0034386 that was previously
certified on January 27, 2010, with this modified Attachment B. It is the Permittee's responsibility to
ensure that a complete permit is created by inserting this modified Attachment B into Permit No.
WQ0034386, issued on January 27, 2010.
This modified Attachment B shall be effective from the date of certification; shall void
Attachment B of Permit No. WQ0034386 that was previously certified on January 27, 2010; and shall be
subject to the conditions and limitations as specified therein and well as those specified in the above -
referenced permit. As always, remember to take the time to review this modified Attachment B
thoroughly, as some of the conditions contained therein may have been changed since the last
certification. Note that this modified Attachment B is hereby incorporated by reference into Permit No.
WQ0034386, issued on January 27, 2010.

If any parts, requirements, and/or limitations contained in this certification or modified
Attachment B are unacceptable, you have the right to request an adjudicatory hearing upon written
request within 30 days following receipt of the certification. This request shall be in the from of a written
petition. conforming to Chapter 150B of the North Carolina General Statutes and filed with the Office of
Administrative Hearings at 6714 Mail Service Center, Raleigh, North Carolina 27699-6714. Unless such
demands are made, this permit shall be final and binding.
On August 5, 2009, Session Law 2009-406, entitled "An Act to Extend Certain Government
Approvals Affecting the Development of Real Property Within the State," was enacted by the General
Assembly and signed into law. The Act, known as the Permit Extension Act of 2009, extends the
expiration date of certain government approvals and permits. On August 2, 2010, this Act was extended
an additional year. The subject permit falls within the scope of this Act and is therefore being
extended to December 31, 2416.
