In accordance with your permit application request received September 25, 2009, and subsequent
additional information received February 19, 2010 and June 7, 2010, we are forwarding herewith Permit
No. WQ0034367 dated July 2, 2010, to the Onslow Water and Sewer Authority for the construction and
operation of the subject wastewater treatment, bulk conjunctive reclaimed water and high -rate infiltration
facilities.
This permit shall be effective from the date of issuance until December 31, 2015, and shall be
subject to the conditions and limitations as specified therein. Please pay particular attention to the
monitoring and use requirements listed in Attachments A, B and C. Failure to establish an adequate
system for collecting and maintaining the required operational information shall result in future
compliance problems.
Please note the following conditions are unique to your permit and require your attention:
➢ Condition I.1. —This condition requires the Permittee to record the executed setback waiver
agreements with the Onslow County Register of Deeds prior to operation of the subject
facilities. Documentation that the waivers have been recorded shall be submitted to the
Division.
➢ Condition I.2. — This condition requires that post construction and prior to operation of the 10
high -rate infiltration basins, the Permittee shall have a licensed Professional Geologist inspect
and verify that the proposed 10 high -rate infiltration basins are capable of assimilating
effluent at the permitted rates. Documentation of this inspection shall be submitted to the
Division.

➢ Condition I.3. — This condition requires the Permittee to provide the Division with a copy of
the education program prior to operation of the bulk conjunctive reclaimed utilization system.
➢ Condition I.4. — This condition requires the Permittee to address how dredged soil will be
stored, treated and disposed of prior to operation of the subject facility.
Please note in accordance with Condition VL4., the issuance of this permit does not exempt the
Permittee from complying with any and all statutes, rules, regulations, or ordinances, which may be
imposed by other jurisdictional government agencies (e.g., local, state, and federal). Accordingly, the
Permittee shall comply with the requirements in the Finding of No Significant Impact document dated
June 1, 2009 and submit the appropriate documentation to the Office of State Archaeology through the
State Historic Preservation Office prior to any ground disturbing activities.
If any parts, requirements or limitations contained in this permit are unacceptable, the Permittee
has the right to request an adjudicatory hearing upon written request within 30 days following receipt of
this permit. This request shall be in the form of a written petition, conforming to Chapter 150B of the
North Carolina General Statutes, and filed with the Office of Administrative Hearings at 6714 Mail
Service Center, Raleigh, NC 27699-6714. Unless such demands are made, this permit shall be final and
binding.

operation of a 1,000,000 gallon per day (GPD) wastewater treatment, bulk conjunctive reclaimed water
and high -rate infiltration facility consisting of:
the construction and operation of a wastewater treatment plant consisting of. an influent flow splitter box;
one 6 millimeter (mm) mechanical fine screen and one auxiliary manual bar screen; an influent composite
sampler; a Parshall flume; one grit removal chamber with two 220 gallon per minute (GPM) centrifugal
grit slurry pumps feeding two grit classifiers and associated dumpsters; an equalization flow splitter box;
two 500,000 gallon aerated flow equalization tanks with two 3,000 GPM centrifugal jet mixing pumps
and served by three 500 cubic feet per minute (CFM) positive displacement blowers; a flow equalization
pump station with three 350 GPM centrifugal return pumps capable of sending flow to the 5-day upset
basin or to the treatment plant headworks; an ultra fine screen flow splitter box; two 2-mm ultra fine
screens; a headworks flow splitter box diverting flow to Trains 1 and 2; eight 12,560 gallon anaerobic
basins (four per train) with eight 2 horsepower (hp) submersible mixers (one per basin) and two 100 GPM
submersible fermentation recycle pumps (one per train); two 84,500 gallon anoxic basins (one per train)
with four 3 hp submersible mixers (two per basin) and two 350 GPM submersible anaerobic recycle
pumps (one per basin) returning flow to their respective anaerobic basins; two 159,000 gallon aeration
basins (one per train) with six 1,111 GPM submersible jet mixing pumps (three per basin), two 1,400
GPM submersible nitrate recycle pumps (one per basin) returning flow to their respective anoxic basins,
and served by three 310 CFM positive displacement blowers; two 42,680 gallon post -anoxic basins (one
per train) with two 550 GPM submersible jet mixing pumps and served by the three aforementioned
aeration basin process positive displacement blowers; four 3,075 gallon membrane bioreactors (MBAs)
(two per train) with four 700 GPM centrifugal membrane recycle pumps (one per bioreactor) returning
flow to their respective aerobic basins, two 470 GPM rotary waste activated sludge (WAS) pumps
wasting to the aerobic digester or fine screens, four rotary permeate pumps (one per bioreactor) sending
flow to the ultraviolet (UV) disinfection system or 5-day upset basin, and served by three 1,670 CFM
positive displacement blowers (or as sized by membrane manufacturer); a clean -in -place chemical feed
system serving the four MBR basins consisting of sodium hypochlorite and citric acid each with their
own chemical feed pumps;

four turbidimeters (one per MBR basin); a one channel UV disinfection system with two in -series banks
with a total of 96 lamps (or as sized by manufacturer); an effluent composite sampler, a chlorine residual
chemical feed system with three 55 gallon NaOCI drums, a 500 gallon NaOCI day tank and two 3 GPM
chemical feed pumps; a scum digester with a 300 GPM rotary scum pump and served by a 50 CFM
positive displacement blower; two 628,300 gallon aerobic sludge digesters with two 9,500 GPM
centrifugal jet mixing pumps (one per basin), a 500 GPM submersible solids transfer pump and served by
three 2,940 CFM positive displacement blowers; a digested sludge pump station with two 1,000 GPM
centrifugal sludge loading pumps; a North Plant drain pump station with two 300 GPM submersible
pumps sending drain water back to the flow equalization splitter box; a South Plant drain pump station
with two 300 GPM submersible pumps sending drain water back to the flow equalization splitter box; an
effluent distribution pump station with three 1,740 GPM centrifugal pumps serving the 10 high -rate
infiltration basins; an approximately 5,013,000 gallon synthetically lined 5-day upset basin with a reject
pump station with two 350 GPM submersible pumps returning reject water back to the treatment
headworks; and all associated piping, valves, electrical systems, instrumental/control systems, and
appurtenances; the
construction and operation of a non -potable reuse system for internal use within the treatment plant works
deemed permitted under 15A NCAC 02T .1003(a)(4) consisting of: a non -potable reuse system pump
station with two 350 GPM centrifugal pumps; a hydropneumatic tank; an air compressor; the
aforementioned chlorine residual feed system; and all associated piping, valves, electrical systems,
instrumental/control systems, and appurtenances; the
construction and operation of a bulk conjunctive reclaimed utilization system consisting of: a bulk
reclaimed water pump station with two 300 GPM centrifugal pumps; a bulk reclaimed water truck loading
station; and all associated piping, valves, electrical systems, instrumental/control systems, and
appurtenances: the
corlstruction and operation of a high -rate infiltration 'system consisting of: 10 effluent flow meters each
serving an individual high -rate infiltration basin; 10 high -rate infiltration basins each approximately 0.9
acres in area and with a loading rate of 2.55 gallons per day per square foot (GPD/ft2); and all associated
piping, valves, electrical systems, instrumental/control systems, and appurtenances; and the
construction and operation of a groundwater lowering system consisting of: a continuous 8-inch
corrugated HDPE perforated piping network enclosing all 10 high -rate infiltration basins and located 50
feet from the basins' high level water elevation; a groundwater lowering pump station #1 with two 139
GPM submersible pumps; a groundwater lowering pump station #2 with two 209 GPM submersible
pumps; a groundwater lowering pump station #3 with two 209 GPM submersible pumps; a groundwater
lowering pump station #4 with two 139 GPM submersible pumps; an irrigation pond receiving lowered
groundwater effluent with a overflow level spreader; and all associated piping, valves, electrical systems,
instrumental/control systems, and appurtenances;
to serve the Northwest Regional Water Reclamation Facility, with no discharge of wastes to surface
waters, pursuant to the application received September 25, 2009, and subsequent additional information
received by the Division of Water Quality, and in conformity with the project plans, specifications, and
other supporting data subsequently filed and approved by the Department of Environment and Natural
Resources and considered a part of this permit.
This permit shall be effective from the date of issuance until December 31, 2015, and shall be subject to
the following specified conditions and limitations:

I.
1. Prior to operation of the subject facilities, the Permittee shall record the Setback Waiver Agreement
executed January 14, 2010 with the Onslow County Register of Deeds, thus allowing effluent high -
rate infiltration and high -rate infiltration basins within 0 feet of the following parcel property lines:
PIN #444103140904; PIN #444103027242; PIN #444103220913; and PIN #444103209366.
The Permittee shall submit copies of the recorded setback waiver to the Division of Water Quality,
Aquifer Protection Section, 1636 Mail Service Center, Raleigh, NC 27699-1636.
2. Upon completion of construction and prior to operation of any high -rate infiltration basins, the basins
shall be inspected by a licensed Professional Geologist to verify that the constructed basins are
capable of receiving the wastewater at the permitted loading rates. The results of this inspection,
along with the results of any testing performed shall be signed, sealed and submitted to the Aquifer
Protection Section, Land Application Unit, 1636 Mail Service Center, Raleigh, NC 27699-1636. The
requested, information must be received and acknowledged in writing by the Aquifer Protection
Section prior to operation of the subject facilities.
3. Prior to operation of the bulk conjunctive reclaimed utilization system, the Permittee shall submit
documentation of a bulk reclaimed water education program in accordance with 15A NCAC 02T
.0910(a)(3), .0910(b)(4) and .0911(d). The Permittee shall submit copies of the education program to
the Division of Water Quality, Aquifer Protection Section, 1636 Mail Service Center, Raleigh, NC
27699-1636.
4. Prior to operation of the high -rate infiltration system and in accordance with 15A NCAC 02T .0707,
the Permittee shall submit documentation of how the Permittee intends to stage, process for
pathogen/vector attraction reduction and dispose of all dredge spoils produced during high -rate
infiltration basin activities, as dredge spoils are classified as a residual per 15A NCAC 02T .0103(30).
The Permittee shall submit copies of this documentation to:

5. In accordance with 15A NCAC 02T .0116, upon completion of construction and prior to operation of
this permitted facility, a certification (attached) shall be submitted from a licensed North Carolina
Professional Engineer certifying that the permitted facility has been installed in accordance with this
permit, Division approved plans and specifications, and other supporting documentation, including
the location of all monitoring wells as applicable. If this project is to be completed in phases and
under the same permit, and shall provide a final certificate of completion once the entire project has
been completed. Mail the Certification to the Division of Water Quality, Aquifer Protection Section,
1636 Mail Service Center, Raleigh, NC 27699-1636.
6. The Wilmington Regional Office, telephone number (910) 796-7215, shall be notified at least 48
hours in advance (excluding weekends and holidays) of operation of the installed facilities such that
an in -place inspection can be made. Notification to the Aquifer Protection Section's regional
supervisor shall be made from 8:00 a.m. until 5:00 p.m. on Monday through Friday, excluding State
Holidays.

7. The Wilmington Regional Office, telephone number (910) 796-7215, shall approve monitoring wells
MW-1, MW-2, MW-3, MW-4, MW-5 and MW-6 prior to installation, and the monitoring wells shall
be installed prior to beginning waste disposal operations. The regional office shall be notified at least
48 hours prior to the construction of any monitoring well, and such notification to the Aquifer
Protection Section's regional supervisor shall be made from 8:00 a.m. until 5:00 p.m. on Monday
through Friday, excluding State Holidays. The monitoring wells shall be constructed such that the
water level in the well is never above or below the screened (open) portion of the well at any time
during the year, and in accordance with 15A NCAC 02C .0108. The general location and name for
each monitoring well is marked on Figure 1.
8. Within 60 days of completion of the monitoring wells, the Permittee shall submit two original copies
of a site map with a scale no greater than 1-inch equals 100 feet; however, special provisions may be
granted upon prior approval for large properties. At a minimum, the map shall include the following
information:
a. The location and identity of each monitoring well.
b. The location of major components of the waste disposal system.
c. The location of property boundaries within 500 feet of the disposal areas.
d. The latitude and longitude of the established horizontal control monument.
e. The elevation of the top of the well casing (i.e., measuring point) relative to a common datum.
f. The depth of water below the measuring point at the time the measuring point is established.
g. The location of compliance and review boundaries.
h. The date the map is prepared and/or revised.
Control monuments shall be installed in such a manner and made of such materials that the
monument will not be destroyed due to activities taking place on the property. The map and any
supporting documentation shall be sent to the Division of Water Quality, Aquifer Protection Section,
1636 Mail Service Center, Raleigh, NC 27699-1636,
9. Gauges to monitor waste levels in the 10 high -rate infiltration basins and the 5-day upset basin shall
be installed prior to operation. Caution shall be taken not to damage the integrity of the liner (if
present) when installing the gauge.
10. No later than six months prior to the expiration of this permit, the Permittee shall request renewal of
this permit on official Division forms. Upon receipt of the request, the Division will review the
adequacy of the facilities described therein, and if warranted, will renew the permit for such period of
time and under such conditions and limitations as it may deem appropriate. Please note Rule 15A
NCAC 02T .0105(d) requires an updated site map to be submitted with the permit renewal
application.
H. PERFORMANCE STANDARDS
The subject non -discharge facilities shall be effectively maintained and operated at all times so there
is no discharge to surface waters, nor any contravention of groundwater or surface water standards.
In the event the facilities fail to perform satisfactorily, including the creation of nuisance conditions
due to improper operation and maintenance, failure of the infiltration areas to adequately assimilate
the effluent, or failure of the bulk conjunctive use distribution system, the Permittee shall take
immediate corrective actions including Division required actions, such as the construction of
additional or replacement wastewater treatment, disposal and/or utilization facilities.
2. This permit shall not relieve the Permittee of their responsibility for damages to groundwater or
surface water resulting from the operation of this facility.
3. All wells constructed for purposes of groundwater monitoring shall be constructed in accordance with
15A NCAC 02C .0108 (Standards of Construction for Wells Other than Water Supply), and any other -
jurisdictional laws and regulations pertaining to well construction.

4. Effluent limitations shall not exceed those specified in Attachment A.
5. Application rates, whether hydraulic, nutrient or other pollutant, shall not exceed those specified in
Attachment B.
6. Reclaimed water distributed through the bulk conjunctive reclaimed utilization system shall be
limited to the uses specified in Attachment D.
7. The following shall be requirements for the bulk conjunctive reclaimed water distribution, storage
and utilization facilities:
a. All reclaimed water valves, storage facilities and outlets shall be tagged or labeled to warn the
public or employees that reclaimed water is not intended for drinking. Where appropriate, such
warning shall inform the public or employees to avoid contact with reclaimed water.
b. All reclaimed water piping, valves, outlets and other appurtenances shall be color -coded, taped or
otherwise marked to identify the source of the water as being reclaimed water.
i. All reclaimed water piping and appurtenances shall be either colored purple (i.e., Pantone
522) and embossed or integrally stamped or marked "CAUTION: RECLAIMED WATER —
DO NOT DRINK" or be installed with a purple (i.e., Pantone 522) identification tape or
polyethylene vinyl wrap. The warning shall be stamped on opposite sides of the pipe and
repeated every three feet or less.
ii. Identification tape shall be at least three inches wide and have white or black lettering on
purple (i.e., Pantone 522) field stating "CAUTION: RECLAIMED WATER — DO NOT
DRINK." Identification tape shall be installed on top of reclaimed water pipelines, fastened
at least every 10 feet to each pipe length and run continuously the entire length of the pipe.
iii. Existing underground distribution systems retrofitted for the purpose of distributing
reclaimed water shall be taped or otherwise identified as noted above. This identification
need not extend the entire length of the distribution system, but shall be incorporated within
10 feet of crossing any potable water supply line or sanitary sewer line.
c. All reclaimed water valves and outlets shall be of a type, or secured in a manner, that permits
operation by authorized personnel only.
d. Above -ground hose bibs (i.e., spigots or other hand -operated connections) shall not be present.
Hose bibs shall be located in locked below -grade vaults that shall be clearly labeled non -potable.
As an alternative to the use of locked below -grade vaults with standard hose bibs services, hose
bibs, which can only be operated by a special tool or connected to a special hose connection, may
be placed in non -lockable underground services boxes clearly labeled non -potable.
8. The compliance boundary for the high -rate infiltration disposal system shall be specified in
accordance with 15A NCAC 02L .0107(b). This disposal system was individually permitted on or
after December 30, 1983; therefore, the compliance boundary is established at either 250 feet from
the effluent disposal area, or 50 feet within the property boundary, whichever is closest to the effluent
disposal area. An exceedance of groundwater standards at or beyond the compliance boundary is
subject to remediation action according to 15A NCAC 02L .0106(d)(2) as well as enforcement
actions in accordance with North Carolina General Statute 143-215.6A through 143-215.6C.
9. In accordance with 15A NCAC 02L .0108, the review boundary is established midway between the
compliance boundary and the high -rate infiltration effluent disposal area. Any exceedance of
groundwater standards at the review boundary shall require action in accordance with 15A NCAC
02L .0106.
10. The compliance and review boundaries for bulk conjunctive reclaimed water utilization activities
have not been established pursuant to 15A NCAC 02T .0910(a)(5). Any exceedance of standards
shall require action in accordance with 15A NCAC 02L .0106.

11. The Permittee shall apply for a permit modification to establish a new compliance boundary prior to
any sale or transfer of property affecting a compliance boundary.
12. In accordance with 15A NCAC 02L .0107(d), no wells, excluding Division approved monitoring
wells, shall be constructed within the compliance boundary except as provided for in 15A NCAC 02L
.0107(g).
13. Except as provided for in 15A NCAC 02L .0107(g), the Permittee shall ensure any landowner who is
not the Permittee and owns land within the compliance boundary shall execute and file with the
Onslow County Register of Deeds an easement running with the land containing the following items:
a. A notice of the permit and number or other description as allowed in 15A NCAC 02L .0107(f)(1);
b. Prohibits construction and operation of water supply wells within the compliance boundary; and
c. Reserves the right of the Permittee or the State to enter the property within the compliance
boundary for purposes related to the permit.
The Director may terminate the easement when its purpose has been fulfilled or is no longer needed.
14. The facilities permitted herein shall be constructed according to the following setbacks:
a. The setbacks for high -rate infiltration sites permitted after September 1, 2006 shall be as follows

The Permittee has elected to comply with the high -rate infiltration system design policy dated
October 27, 2006. Therefore, in accordance with meeting the Policy's criteria, the setback in
Condition II.14.a.i. has been reduced from 400 feet to 100 feet, and the setbacks in
Conditions II.14.a.ii. and II.14.a.ix. have been reduced from 200 feet to 50 feet.
2 Setbacks to non -SA surface waters, groundwater lowering ditches and subsurface
groundwater lowering drainage systems have been reduced from 200 feet to 50 feet, and the
setback to SA surface waters has been reduced from 200 feet to 100 feet because the
treatment units are designed to meet a Total Nitrogen of 4 mg/1 and a Total Phosphorus of 2
mg/1 in accordance with 15A NCAC 02T .0706(c).

M. OPERATION AND MAINTENANCE REOUIREMENTS
The facilities shall be properly maintained and operated at all times. The facilities shall be effectively
maintained and operated as a non -discharge system to prevent the discharge of any wastewater
resulting from the operation of this facility. The Permittee shall maintain an Operation and
Maintenance Plan pursuant to 15A NCAC 02T .0707 and .0913, which at a minimum shall include
operational functions, maintenance schedules, safety measures and a spill response plan.
2. Upon the Water Pollution Control System Operators Certification Commission's (WPCSOCC)
classification of the subject non -discharge facilities, in accordance with 15A NCAC 08G .0200 the
Permittee shall designate and employ a certified operator in responsible charge (ORC) and one or
more certified operator(s) as back-up ORC(s). The ORC or their back-up shall visit the facilities in
accordance with 15A NCAC 08G .0200, and shall comply with all other conditions specified in the
previously cited rules.
3. Only effluent from the Northwest Regional Water Reclamation Facility shall be infiltrated on the sites
listed in Attachment B.

4. Only reclaimed water from the Northwest Regional Water Reclamation Facility shall be utilized for
the approved bulk conjunctive reclaimed water uses listed in Attachment D.
5. For bulk conjunctive reclaimed utilization operations the following requirements shall be met:
a. The Permittee shall develop and implement an operation and maintenance plan, which includes
but is not limited to the following
i. Description of operation;
ii. Description of anticipated maintenance;
iii. Safety measures, including site restrictions;
iv. Spill prevention and spill response plan;
v. Contact information for plant personnel, emergency responders, and regulatory agencies;
vi. Approval procedures of users;
vii. Operational requirements for users; and
viii.If reclaimed water is to be stored for greater than 72 hours prior to use, include a description
of how compliance with reclaimed water quality standards will be ensured at the time of use.
b. An education program shall be developed and implemented to inform bulk conjunctive reclaimed
utilization users of the proper use of reclaimed water for each approved use (Attachment D).
Educational material shall be provided to all individuals accepting the bulk reclaimed water.
c. Educational material shall be maintained consistent with the approved reclaimed water uses listed
in Attachment D. Educational material shall be made available to the Division upon request.
d. The Permittee shall approve tank truck drivers for participation in the bulk reclaimed water
distribution program prior to distribution of reclaimed water. In addition, the Permittee shall
provide bilingual (English and Spanish) training to all tank truck drivers approved to participate
in the bulk reclaimed water distribution program at a regular frequency. These sessions shall
provide training on, but not limited to, the following topics:
i. Reclaimed water description and safety;
ii. Allowable and prohibited uses of reclaimed water;
iii. Procedures for the application (including buffer maintenance), uses, and storage of reclaimed
water as well as the disposal of unused reclaimed water;
iv. Procedures for spill prevention, countermeasures, and control as well as notification
requirements;
v. Personal safety and hygiene issues; and
vi. Interaction with the general public.
e. Tank trucks used to transport reclaimed water shall meet the following requirements:
i. Tank trucks and other equipment used to distribute reclaimed water shall be clearly identified
with advisory signs. Signage could include but is not limited to "CAUTION: RECLAIMED
WATER — DO NOT DRINK", "NON -POTABLE WATER", or a combination of similar
phrases.
ii. Tank trucks used to transport reclaimed water shall not be used to transport potable water that
is used for drinking or other potable purposes.
iii. Tank trucks used to transport reclaimed water shall not be filled through on -board piping or
removable hoses that may subsequently be used to fill tanks with water from a potable water
supply. Tank trucks owned and operated by the Permittee and dedicated to be filled by
reclaimed water only may be filled through on -board piping.
iv. Tank trucks shall not have been used to haul septage, biosolids, or other waste or chemical
transport.

v. Tank trucks shall not have any type of removable container or portable bladder except if the
tank/bladder is permanently labeled for reclaimed water use only and a letter from the truck
owner is submitted to the Permittee indicating that in no case shall any removable container
or portable bladder be used for both reclaimed and potable water at any time.
6. An automatically activated standby power source capable of powering all essential treatment units
shall be on site and operational at all times. If a generator is employed as an alternate power supply,
it shall be tested weekly by interrupting the primary power source.
7. No automobiles or machinery shall be allowed on the infiltration sites except during equipment
installation or while maintenance is being performed.
8. Public access to the infiltration sites and wastewater treatment facilities shall be prohibited.
9. Public access to bulk conjunctive reclaimed water utilization sites shall be controlled during active
site use. Such controls may include the posting of signs indicating the use of reclaimed water and
appropriate restrictions.
10. The residuals generated from the wastewater treatment facilities shall be disposed or utilized in
accordance with 15A NCAC 02T .1100. The Permittee shall maintain a residual management plan
pursuant to 15A NCAC 02T .0708.
11. Diversion or bypassing of untreated or partially treated wastewater from the treatment facilities is
prohibited.
12. Freeboard in the 10 high -rate infiltration basins and the 5-day upset basin shall not be less than two
feet at any time.
13. Gauges to monitor waste levels in the 10 high -rate infiltration basins and the 5-day upset basin shall
be provided. These gauges shall have readily visible permanent markings indicating the following
elevations: maximum liquid level at the top of the temporary liquid storage volume; minimum liquid
level at the bottom of the temporary liquid storage volume; and the lowest point on top of the dam.
14. A protective vegetative cover shall be established and maintained on all earthen embankments (i.e.,
outside toe of embankment to maximum allowable temporary storage elevation on the inside of the
embankment), berms, pipe runs, erosion control areas, and surface water diversions. Trees, shrubs,
and other woody vegetation shall not be allowed to grow on the earthen dikes or embankments.
Earthen embankment areas shall be kept mowed or otherwise controlled and accessible.
15. All effluent shall be routed to the five day upset basin should the limit for fecal coliform (e.g., daily
maximum concentration of 25 colonies per 100 ml) or turbidity (e.g., instantaneous maximum of 10
NTU) be exceeded, until the problems associated with the wastewater treatment plant have been
corrected. The wastewater in the five day upset basin shall be pumped back to the treatment plant
headworks for re -treatment or treated in the five day upset basin prior to infiltration and/or bulk
conjunctive reclaimed utilization.
16. The high -rate infiltration basins shall be periodically dredged to remove deposited materials that may
impede the infiltration process. Dredging records shall be maintained at the facility for a period of no
less than five years, and shall be made available to the Division upon request. The Wilmington
Regional Office's Aquifer Protection Section, telephone number (910) 796-7215, shall be notified
prior to dredging.

IV. MONITORING AND REPORTING REQUIREMENTS
1. Any Division required monitoring (including groundwater, plant tissue, soil and surface water
analyses) necessary to ensure groundwater and surface water protection shall be established, and an
acceptable sampling reporting schedule shall be followed.
2. Per 15A NCAC 02H .0800, a Division certified laboratory shall conduct all laboratory analyses for
the required effluent, groundwater or surface water parameters.
3. Flow through the treatment facility and distributed to the high -rate infiltration system shall be
continuously monitored, and daily flow values shall be reported on Form NDMR.
The Permittee shall install and maintain an appropriate flow measurement device to ensure the
accuracy and reliability of flow measurement consistent with accepted engineering and scientific
practices. Selected flow measurement devices shall be capable of measuring flows with a maximum
deviation of less than ten percent from true flow; accurately calibrated at a minimum of once per year;
and maintained to ensure the accuracy of measurements is consistent with the selected device's
accepted capability. The Permittee shall maintain records of flow measurement device calibration on
file for a period of at least five years. At a minimum, documentation shall include:
a. Date of flow measurement device calibration,
b. Name of person performing calibration, and
c. Percent from true flow.
4. Flow through the bulk conjunctive reclaimed utilization distribution operation shall be continuously
monitored, and daily flow values shall be reported on Form NDMR.
The Permittee shall install and maintain an appropriate flow measurement device to ensure the
accuracy and reliability of flow measurement consistent with accepted engineering and scientific
practices. Selected flow measurement devices shall be capable of measuring flows with a maximum
deviation of less than ten percent from true flow; accurately calibrated at a minimum of once per year;

5. The Permittee shall monitor the effluent from the subject facilities at the frequencies and locations for
the parameters specified in Attachment A.
6. The Permittee shall maintain adequate records tracking the amount of effluent infiltrated. At a
minimum, these records shall include the following information for each infiltration site listed in
Attachment B:
a. Date of infiltration;
b. Volume of effluent infiltrated;
c. Loading rates to each infiltration site listed in Attachment B; and
d. Weather conditions

7. The Permittee shall maintain adequate records of all bulk conjunctive reclaimed water distributed.
These records shall include, but are not necessarily limited to:
a. Date, time, and volume of each bulk distribution event;
b. Name and drivers license number for tank truck operators;
c. Bulk fill certification number,
d. Intended use of the reclaimed water;
e. Turbidity measurement result; and
f. Total volume of bulk reclaimed water distributed each month.
8. Freeboard (i.e., waste level to the lowest embankment elevation) in the 10 high -rate infiltration basins
and 5-day upset basin shall be recorded weekly. Weekly freeboard records shall be maintained at the
facility for a period of no less than five years, and shall be made available to the Division upon
request.
9. Three copies of all monitoring data (as specified in Conditions IV.3., IVA. and IV.5.) on Form
NDMR for each PPI, three copies of all operation and disposal records (as specified in Conditions
IV.6 and IV.8.) on Form NDAR-2 for every site in Attachment B, and three copies of all bulk
distribution records (as specified in Condition IV.7.) on Form NDAR-3 shall be submitted on or
before the last day of the following month. If no activities occurred during the monitoring month,
monitoring reports are still required documenting the absence of the activity. All information shall be
submitted to the following address:

10. Pursuant to § 143-215.1C., the Permittee shall provide to its users and the Division of Water Quality
an annual report summarizing the performance of the wastewater treatment, bulk conjunctive
reclaimed utilization and high -rate infiltration facilities and the extent to which the facility has
violated this permit, or federal or State laws, regulations, or rules related to the protection of water
quality. This report shall be prepared on either a calendar -or fiscal year basis and shall be provided
no later than 60 days after the end of the calendar or fiscal year. Two copies of the annual report
provided to the Permittee's users shall be submitted to:

11. A record shall be maintained of all residuals removed from this facility. This record shall be
maintained at the facility for a period of no less than five years, and shall be made available to the
Division upon request. At a minimum, this record shall include:
a. Name of the residuals hauler;
b. Non -Discharge permit number authorizing the residuals disposal, or a letter from a municipality
agreeing to accept the residuals;
c. Date the residuals were hauled; and
d. Volume of residuals removed.
12. A maintenance log shall be maintained at this facility. This log shall be maintained at the facility for
a period of no less than five years, and shall be made available to the Division upon request. At a
minimum, this log shall include:
a. Date and results of power interruption testing on alternate power supply;
b. Date of calibration of flow measurement device;
c. Visual observations of the plant and plant site; and
d. Record of preventative maintenance (e.g., changing of equipment, adjustments, testing,
inspections and cleanings, etc.).

13. A bulk conjunctive reclaimed water user inspection log shall be maintained at this facility. This log
shall be maintained at the facility for a period of no less than five years, and shall be made available
to the Division upon request. At a minimum, this log shall include:
a. Visual observations of the reclaimed water user facilities; and
b. Record of preventative maintenance (e.g., pump and valve maintenance, cross connection control,
etc.).
14. Monitoring wells shall be sampled after construction and within 3 months prior to initiating non -
discharge disposal operations. Monitoring wells shall be sampled thereafter at the frequencies and for
the parameters specified in Attachment C. All mapping, well construction forms, well abandonment
forms and monitoring data shall refer to the permit number and the well nomenclature as provided in
Attachment C and Figure 1.
15. For initial sampling of monitoring wells, the Permittee shall submit a Compliance Monitoring Form
(GW-59) and a Well Construction Record Form (GW-1) listing this permit number and the
appropriate monitoring well identification number. Initial Compliance Monitoring Forms (GW-59)
without copies of the Well Construction Record Forms (GW-1) are deemed incomplete, and may be
returned to the Permittee without being processed.
16. Two copies of the monitoring well sampling and analysis results shall be submitted on a Compliance
Monitoring Form (GW-59), along with attached copies of laboratory analyses, on or before the last
working day of the month following the sampling month. The Compliance Monitoring Form (GW-
59) shall include this permit number, the appropriate well identification number, and one GW-59a
certification form shall be submitted with each set of sampling results. All information shall be
submitted to the following address:

17. Noncompliance Notification:
The Permittee shall report by telephone to the Wilmington Regional Office, telephone number (910)
796-7215, as soon as possible, but in no case more than 24 hours, or on the next working day
following the occurrence or first knowledge of the occurrence of any of the following:
a. Any occurrence at the facility resulting in the treatment'of significant amounts of wastes that is
abnormal in quantity or characteristic, including the known passage of a hazardous substance.
b. Any process unit failure (e.g., mechanical, electrical, etc.), due to known or unknown reasons,
rendering the facility incapable of adequate wastewater treatment.
c. Any facility failure resulting in a by-pass directly to receiving surface waters.
d. Any time self -monitoring indicates the facility has gone out of compliance with its permit
limitations.
e. Effluent breakout from the infiltration basins.
Any emergency requiring immediate reporting (e.g., discharges to surface waters, imminent failure of
a storage structure, etc.) outside normal business hours shall be reported to the Division's Emergency
Response personnel at telephone number (800) 662-7956, (800) 858-0368, or (919) 733-3300,
Persons reporting such occurrences by telephone shall also file a written report in letter form within
five days following first knowledge of the occurrence. This report shall outline the actions taken or
proposed to be taken to ensure the problem does not recur.

1. The Permittee shall provide adequate inspection and maintenance to ensure proper operation of the
wastewater treatment, bulk conjunctive reclaimed utilization and high -rate infiltration facilities.
2. The Permittee or their designee shall inspect the wastewater treatment, bulk conjunctive reclaimed
utilization and high -rate infiltration facilities to prevent malfunctions, facility deterioration and
operator errors resulting in discharges, which may cause the release of wastes to the environment, a
threat to human health or a public nuisance. The Permittee shall maintain an inspection log that
includes, at a minimum, the date and time of inspection, observations made, and any maintenance,
repairs, or corrective actions taken. The Permittee shall maintain this inspection log for a period of
five years from the date of the inspection, and this log shall be made available to the Division upon
request.
3. Any duly authorized Division representative may, upon presentation of credentials, enter and inspect
any property, premises or place on or related to the wastewater treatment, bulk conjunctive reclaimed
utilization and high -rate infiltration facilities permitted herein at any reasonable time for the purpose
of determining compliance with this permit; may inspect or copy any records required to be
maintained under the terms and conditions of this permit, and may collect groundwater, surface water
or leachate samples.
4. In accordance with 15A NCAC 02T .0911(0 and to ensure compliance with the conditions of this
permit, the Permittee shall inspect the utilization sites of all reclaimed water users who obtain more
than 10,000 gallons per day of bulk distributed reclaimed water. Permittee inspection of these bulk
distribution reclaimed water utilization sites does not need to exceed one inspection per quarter. The
Permittee shall maintain records of these inspections for at least five years, and shall make them
available to the Division upon request.
VI. GENERAL CONDMONS
1. Failure to comply with the conditions and limitations contained herein may subject the Permittee to
an enforcement action by the Division in accordance with North Carolina General Statutes 143-
215.6A to 143-215.6C.
2. This permit shall become voidable if the permitted facilities are not constructed in accordance with
the conditions of this permit, the Division approved plans and specifications, and other supporting
documentation.
3. This permit is effective only with respect to the nature and volume of wastes described in the permit
application, Division approved plans and specifications, and other supporting documentation. No
variances to applicable rules governing the construction or operation of the permitted facilities are
granted, unless specifically requested and approved in this permit pursuant to 15A NCAC 02T
.0105(n).
4. The issuance of this permit does not exempt the Permittee from complying with any and all statutes,
rules, regulations, or ordinances, which may be imposed by other jurisdictional government agencies
(e.g., local, state, and federal). Of particular concern to the Division are applicable river buffer rules
in 15A NCAC 02B .0200; erosion and sedimentation control requirements in 15A NCAC Chapter 4
and under the Division's General Permit NCGO10000; any requirements pertaining to wetlands under
15A NCAC 02B .0200 and 02H .0500; and documentation of compliance with Article 21 Part 6 of
Chapter 143 of the General Statutes.

5. In the event the permitted facilities change ownership or the Permittee changes their name, a formal
permit modification request shall be submitted to the Division. This request shall be made on official
Division forms, and shall include appropriate property ownership documentation and other supporting
documentation as necessary. The Permittee of record shall remain fully responsible for maintaining
and operating the facilities permitted herein until a permit is issued to the new owner.
6. The Permittee shall retain a set of Division approved plans and specifications for the life of the
facilities permitted herein.
7. The Permittee shall maintain this permit until all permitted facilities herein are properly closed or
permitted under another permit issued by the appropriate permitting authority pursuant to 15A NCAC
02T .01050).
8. This permit is subject to revocation or unilateral modification upon 60 days notice from the Division
Director, in whole or part for the requirements listed in 15A NCAC 02T .0110.
9. Unless the Division Director grants a variance, expansion of the permitted facilities contained herein
shall not be granted if the Permittee exemplifies any of the criteria in 15A NCAC 02T .0120(b).
10. The Permittee shall pay the annual fee within 30 days after being billed by the Division. Failure to
pay the annual fee accordingly shall be cause for the Division to revoke this permit pursuant to 15A
NCAC 02T .0105(e)(3).

1. 3 x Year monitoring shall be conducted in March, July & November; Annual monitoring shall be conducted every November.
2. The measurement of water levels shall be made prior to purging the wells. The depth to water in each well shall be measured from the surveyed point on the top of the casing. The measurement
of pill shall be made after purging and prior to sampling for the remaining parameters.
3. The measuring points (top of well casing) of all monitoring wells shall be surveyed to provide the relative elevation of the measuring point for each monitoring well. 'The measuring points (top
of casing) of all monitoring wells shall be surveyed relative to a common datum.
4. Volatile Organic Compounds (VOC) - In November only, analyze by one of the following methods:
a. Standard Method 6230D, PQL at 0.5 pg/L or less
b. Standard Method 62I0D, PQL at 0.5 pg/L or less
C. EPA Method 8021, Low Concentration, PQL at 0.5 pg/L or less
d. EPA Method 8260, Low Concentration, PQI, at 0.5 pg/L or less
e. Another method with prior approval by the Aquifer Protection Section Chief
Any method used must meet the following qualifications:
a. A laboratory must be DWQ certified to run any method used.
b. The method used must, at a minimum, include all the constituents listed in Table Vill of Standard Method 6230D.
c. The method used must provide a PQL. of 0.5 µg/L or less that must be supported by laboratory proficiency studies as required by the DWQ Laboratory Certification Unit. Any constituents
detected above the MDL but below the PQL of 0.5 pg/1, must be qualified (estimated) and reported.
5. If any volatile organic compounds (VOC) are detected as a result of monitoring as provided in Attachment C. then the Wilmington Regional Office Aquifer Protection Supervisor, telephone
number (910) 796-7215, must be contacted immediately for further instructions regarding any additional follow-up analyses required.
6. if TOC concentrations greater than 10 mg/1 are detected in any downgradient monitoring well, additional sampling and analysis must be conducted to identify the individual constituents
Comprising this TOC concentration. lr the TOC concentration as measured in the background monitor well exceeds 10 mg/1, this concentration will be taken to represent the naturally occurring
TOC concentration. Any exceedances of this naturally occurring'rOC concentration in the downgradient wells shall be subject to the additional sampling and analysis as described above.
