Dear Mr. Puryear:
In accordance with your permit modification request received on March 22, 2013, we are
forwarding herewith a modified Attachment A, certified May 8, 2013, to the Synagro Central, LLC for
the subject land application of residuals solids program.
This certification is being issued to modify Attachment A to add the Mebane Wastewater
Treatment )dant as a source of residuals. This addition will effectively increase the increase the maximum
annual dry tonnage for the permit from the current 4,700 dry tons per year to 5,000 dry tons per year.
Please note that to apply all of the permitted dry tons (5,000) will require a permit modification to
annex additional fields that will serve to supplement the currently permitted acreage (889.3 acres).
.Since all of the permitted fields may not be available for use every year, care must be taken to assure
there are sufficient lands available for the generated residuals.
Please replace the existing Attachment A of Pgrmit No. WQ0034195 that was previously
certified on February 25, 2011, with this modified Attachment A.
This modified Attachment A shall be effective from the date of certification; shall void
Attachment A of Permit No. WQ0034195 that was previously certified on February 25, 2011; and shall
be subject to the conditions and limitations as specified therein as well as those specified in the above -
referenced permit. Note that this modified Attachment A is hereby incorporated by reference into Permit
No. WQ0034195, issued on September 9, 2009.
If any parts, requirements, and/or limitations contained in this certification or modified
Attachment A are unacceptable, you have the right to request an adjudicatory hearing upon written
request within 30 days following receipt of the certification. This request shall be in the form of a written
petition, conforming to Chapter 150B of the North Carolina General Statutes and filed with the Office of
Administrative Hearings at 6714 Mail Service Center, Raleigh, North Carolina 27699-6714. Unless such
demands are made, this permit shall be final and binding.

SYNAGRO CENTRAL, LLC — LAND APPLICATION OF RESIDUALS DISTRIBUTION PROGRAM
No residuals other than those generated by the following residuals source -generating facilities shall be approved for acceptance for storage or treatment in accordance with this
hermit:
NOTE: In the event that the total yearly amount permitted herewith is land applied, additional acreage that is not yet included in this
permit will be required to support the PAN nutrient loading.
Monitoring frequencies are based on the actual dry tons applied per year, unless otherwise specified. The Monitoring Frequency Requirements Table below shall be used
to determine appropriate sampling frequency. The Permittee is responsible for managing the program to assure that adequate sampling is performed.
b Provided that the residuals from this residuals source -generating facility are not mixed with any other residuals authorized by this permit, this facility shall be exempt
from complying with the performance standards stipulated in Condition II.6., as well as the monitoring and reporting requirements as stipulated in Condition IVA.
