In accordance with your permit modification request received on August 2, 2010, we are
forwarding herewith a modified Attachment A, certified August 25, 2010, to the Synagro Central, LLC
for the subject land application of residuals solids program.
On August 5, 2009, Session Law 2009-406, entitled "An Act to Extend Certain Government
Approvals Affecting the Development of Real Property Within the State," was enacted by the General
Assembly and signed into law. The Act, known as the Permit Extension Act of 2009, extends the
expiration date of certain government approvals and permits. On August 2, 2010, this Act was extended
an additional year. The subject permit falls within the scope of this Act and is therefore being
extended to December 31, 2016.
This certification is being issued to approve additional tonnage for an approved residuals source,
which is the Lake Townsend WTP, a facility that is owned and operated by the City of Greensboro, North
Carolina. The tonnage from this facility will be 1,500 dry tons per year, and the total tonnage for this
permit from all sources is being increased from 1,400 dry tons per year to 2,900 dry tons per year.
Please replace the existing Attachment A of Permit No. WQ0034195 that was previously
certified on September 9, 2009, with this modified Attachment A. It is the Permittee's responsibility
to ensure that a complete permit is created by inserting this modified Attachment A into Permit No.
WQ0034195, issued on September 9, 2009.
This modified Attachment A shall be effective from the date of certification; shall void
Attachment A of Permit No. WQ0034195 that was previously (most recently) certified on September 9,
2009; and shall be subject to the conditions and limitations as specified therein and well as those specified
in the above -referenced permit. As always, remember to take the time to review this modified
Attachment A thoroughly, as ,some of the conditions contained therein may have been changed since the
last certification. Note that this modified Attachment A is hereby incorporated by reference into Permit
No. WQ0034195, issued on September 9, 2009,

If any parts, requirements, and/or limitations contained in this certification or modified
Attachment A are unacceptable, you have the right to request an adjudicatory hearing upon written
request within 30 days following receipt of the certification. This request shall be in the form of a written
petition, conforming to Chapter 150B of the North Carolina General Statutes and filed with the Office of
Administrative Hearings at 6714 Mail Service Center, Raleigh, North Carolina .27699-6714. Unless such
demands are made, this permit shall be final and binding.

No residuals other than those generated by the following residuals source -generating facilities shall be approved for acceptance for storage or treatment in accordance with this
permit:
Monitoring frequencies are based on the actual dry tons applied per year, unless otherwise specified. The Monitoring Frequency Requirements Table below shall be used
to determine appropriate sampling frequency. The Permittee is responsible for managing the program: to assure that adequate sampling is performed.
b Provided that the residuals from this residuals source -generating facility are not mixed with any other residuals authorized by this permit, this facility shall be exempt
from complying with the performance standards stipulated in Condition II.6., as well as the monitoring and reporting requirements as stipulated in Condition IVA
