In accordance with your permit modification application received on ,lune 21, 2010, we are
forwarding herewith a modified Attachment B, certified August 13, 2010, to Case Farms, LLC for the
subject wastewater irrigation system.
This certification is, being issued to modify Attachment B by adjusting the net application areas of
Zone 2 from 1.20 to 1.13 acres and Zone 3 from 1.17 to 1.I 2 acres, to reflect the change of two irrigation
heads from full to partial which better position the monitoring well (MW -5) at the review boundary.
Please replace the existing Attachment B and an updated site map of Permit No.
WQ0033677 that was previously certified on duly 18, 2009, with this modified Attachment B.
This modified Attachment B shall be effective from the date of certification; shall void
Attachment B of Permit No. WQ0033677 that was previously certified on July 18, 2009; and shall be
subject to the conditions and limitations as specified therein and well as those specified in the above -
referenced permit. Note that this modified Attachment B is hereby incorporated by reference into Permit
No. WQ0033677, issued on July 18, 2009.
On August 5, 2009, Session Law 2009-406, entitled "An Act to Extend Certain Government
Approvals Affecting the Development of Real Property within the State," was enacted by the
General Assembly and signed into law. The Act, known as the Permit Extension Act of 2009,
extends the expiration date of certain government approvals and permits.
Non -discharge permit, WQ0033677, issued by the Division of Water Quality falls within the
scope of this Act and is therefore being extended until December 31, 2016. Please note per your
permit, a renewal application must still be submitted six months in advance of the extended
expiration date.

If any parts, requirements, and/or limitations contained in this certification or modified
Attachment B are unacceptable, you have the right to request an adjudicatory hearing upon written
request within 30 days following receipt of the certification. This request shall be in the form of a written
petition, conforming to Chapter 150B of the North Carolina General Statutes and filed with the Office of
Administrative Hearings at 6714 Mail Service Center, Raleigh, North Carolina 27699-6714. Unless such
demands are made, this permit small be final and binding.
