Dear Mr. and Mrs. Cantu:
In accordance with your permit modification request received April 3, 2008, we are forwarding herewith Permit No.
WQ00325YI, dated April 11, 2008, to Nazario (Chuck) and Mary Cantu for the construction and operation of the
subject single family wastewater treatment and surface irrigation facilities.
This permit shall be effective from the date of issuance until February 28, 2013 and shall be subject to the conditions
and limitations as specified therein. Please pay particular attention to the monitoring requirements in this permit.
Failure to establish an adequate system for collecting and maintaining the required operational information will
result in future compliance problems. Please pay particular attention to permit condition I.14 which specifies
procedures to be followed in the addition of fill material to the irrigation area and to permit condition 1.15 which
requires preparation and submittal of a soils report following completion of the fill operation.
This modification approves the use of an 8 foot diameter 11,126 gallon chlorine contact/pump tank in place of the
previous 10 foot diameter 10,265 gallon tank. This modification also approves changes in the piping to relocate the
chlorinator, which is between the Whitewater DF-50 Aerobic Treatment Unit and the chlorine contact/pump tank.
An extension shall be requested, by the permittee, at least six (6) months prior to the expiration of this permit. Upon
receipt of the request the Commission will review the adequacy of the facility described therein and, if warranted,
will extend the permit for such period of time and under such conditions and limitations as it may deem appropriate.
If any parts, requirements, or limitations contained in this permit are unacceptable, you have the right to request an
adjudicatory hearing upon written request within thirty (30) days following receipt of this permit. This request must
be in the form of a written petition, conforming to Chapter 150B of the North Carolina General Statutes, and filed
with the Office of Administrative Hearings, 6714 Mail Service Center, Raleigh, NC 27699-6714. Unless such
demands are made this permit shall be final and binding,

SINGLE FAMILY RESIDENCE SURFACE IRRIGATION PERMIT
In accordance with the provisions of Article 21 of Chapter 143, General Statutes of North Carolina as
amended, and other applicable Laws, Rules, and Regulations
PERMISSION IS HEREBY GRANTED TO

FOR THE
construction and operation of a 360 gallon per day (GPD) surface irrigation wastewater treatment and disposal
system consisting of a 1,200 gallon baffled septic tank with effluent filter, an in -ground Whitewater DF-50
Aerobic Treatment Unit with dual air compressors, dual tablet chlorinators in parallel, a 11,126 gallon
chlorine contact/pump tank, dual 20 GPM field dosing pumps with a high water alarm light and buzzer, a
standby 8.5 KW generator with transfer switch, a 21,840 square foot drip irrigation area consisting of 4 zones
of 5,460 square feet each with 7 drip lines 390 feet long on a 2 foot spacing and all associated piping and
appurtenances, to serve the Chuck and Mary Cantu residence located at 525-A Grants Creek Road,
Jacksonville, North Carolina, 28540, with no discharge of wastes to the surface waters, pursuant to the request
for modification received April 3, 2008, and in conformity with the project plan, specifications, and other
supporting data subsequently filed and approved by the Department of Environment and Natural Resources
and considered a part of this permit.
This permit shall be effective from the date of issuance until February 28, 2013 and shall be subject to the
following specified conditions and limitations:
I. PERFORMANCE STANDARDS
I. Upon completion of construction and prior to operation of this permitted facility, a certification must
be received from a professional engineer certifying that the permitted facility has been installed in
accordance with this permit, the approved plans and specifications, and other supporting materials. If
this project is to be completed in phases and partially certified you shall retain the res onsibili to
track further construction a roved under the same permit, and shall provide a final certificate of
completion once the entire project has been com leted. Mail the Certification to the Groundwater
Protection Unit, 1636 Mail Service Center, Raleigh, NC 27699-1636.
2. The Wilmington Regional Office, telephone number (910) 796-7215, shall be notified at least forty-
eight (48) hours in advance of operation of the installed facilities so that an in -place inspection can be
made. Such notification to the regional supervisor shall be made during the normal office hours from
8:00 a.m. until 5:00 p.m. on Monday through Friday, excluding State Holidays,
The surface irrigation facilities shall be effectively maintained and operated at all times so that there
is no discharge to surface waters, nor any contamination of ground waters, which will render them
unsatisfactory for normal use. In the event that the facilities fail to perform satisfactorily, including
the creation of nuisance conditions or failure of the irrigation area to adequately assimilate the
wastewater, the Permittee shall take immediate corrective actions including those actions that may be
required by the Division of Water Quality, such as the construction of additional or replacement
wastewater treatment and disposal facilities.
4. The issuance of this permit shall not relieve the Permittee of the responsibility for damages to surface
or ground waters resulting from the operation of this facility.
5. A leakage test shall be performed on the septic tank and storage/pump tank(s) to insure that any
exfiltration occurs at a rate, which does not exceed twenty (20) gallons per twenty-four (24) hours per
1,000 gallons of tank capacity. The Engineer's Certification will serve as proof of compliance with
this condition.
6. If excavation into bedrock is required for the installation of the septic tank, Whitewater DF-50 Unit,
pump tank or other components, the excavation shall be lined with a 10-mil synthetic liner.
7. A sanitary tee and filter shall be installed on the effluent pipe of the septic tank. The Engineer's
Certification will serve as evidence that these specifications have been met.
8. The residuals generated from these treatment facilities must be disposed in accordance with General
Statute 143-215.1 and in a manner approved by the Division.
9. Diversion or bypassing of the untreated wastewater from the treatment facilities is prohibited.
10. The following setbacks shall be maintained:
a. 100 feet between the wetted area and any residence or places of public assembly under separate
ownership,
b. 50 feet between the wetted area and property lines,
c. 100 feet between wetted area and wells,
d. 100 feet between the wetted area and drainage ways or surface water bodies,
e. 50 feet between the wetted area and public right of ways,
£ 100 feet between the wetted area and any swimming pool,
g. 15 feet between the wetted area and any building foundation or basement,
h. 100 feet between the treatment/storage units and any wells, with the exception of monitoring
wells, and
i. 50 feet between the treatment units and property lines.
11. The pump tank(s) shall have functional audible and visual high water alarms that are external to any
structure.
12. The disposal system shall be connected to a rain or moisture sensor that shall indicate when
wastewater application is not appropriate in accordance with Condition 11(4).
13. All tank access risers shall extend at lest six (6) inches above finished grade and shall be provided
with properly secured covers.

14. The installation of fill material over the wetted area as specified in the plans shall only occur when
soil moisture conditions are sufficiently low to ensure that compaction of the underlying subsoil and
rutting of the soil surface will not occur. Fill material shall have USDA soil textures classified as
sand, sandy loam or loam. Fill material shall not contain fibrous organics, building rubble or other
debris. The fill material shall be placed over the application area only after proper preparation of the
original ground surface. Heavy vegetative cover or organic litter shall be removed before the fill
material is added or incorporated. The first layer of fill material and the existing soil shall be mixed
to a depth of six inches below the interface. The type of fill material shall be approved by a licensed
soil scientist prior to placement. The fill area shall be shaped to shed surface water and shall be
stabilized with a vegetative cover to prevent erosion. The side slope of the fill shall not exceed a rise
to run ratio of 1:4. After placement, a soil test of the fill material shall be performed to determine the
addition of lime and fertilizer required for the specific type of vegetation to be established as a
vegetative cover.
15. Within 15 days from the date of completion of the addition of fill material, a post -construction soils
report shall be conducted to confirm that the fill addition was done appropriately and that the
hydraulic loading rates specified in Permit Condition 11.6 remain appropriate. The completed, signed,
sealed soils report shall be submitted to the Groundwater Protection Unit of the Aquifer Protection
Section, at 1636 Mail Service Center, Raleigh, North Carolina 27699-1636 prior to the start of
construction of the wastewater disposal system under this permit.
II. OPERATION AND MAINTENANCE REQUIREMENTS
1. The facilities shall be properly maintained and operated at all times.
2. Upon classification of the wastewater treatment and surface irrigation facilities by the Water
Pollution Control System Operators Certification Commission (WPCSOCC), the Permittee shall
designate and employ a certified operator to be in responsible charge (ORC) and one or more
certified operator(s) to be back-up ORC(s) of the facilities in accordance with 15A NCAC 8G .0201.
The ORC shall visit the facilities in accordance with 15A NCAC 8G .0204 or as specified in this
permit and shall comply with all other conditions specified in these rules.
3. A suitable year round vegetative cover shall be maintained over the disposal area.
4. Irrigation shall not be performed during inclement weather or when the ground is in a condition that
will cause runoff.
5. Adequate measures shall be taken to prevent wastewater runoff from the irrigation field.
6. The application rate shall not exceed a cumulative loading of 13.0 inches over any twelve (12) month
period at an instantaneous application rate not to exceed 0.25 inches per hour.
7. The facilities shall be effectively maintained and operated as a non -discharge system to prevent the
discharge of any wastewater resulting from the operation of this facility.
No type of wastewater other than that from the Chuck and Mary Cantu residence at 525-A Grants
Creek Road, Jacksonville, North Carolina 28540, shall be emitted onto the irrigation area.
No traffic or equipment shall be allowed on the disposal area except while installation occurs or
while normal maintenance is being performed.
10. The irrigation field shall be fenced with at least a two -strand wire fence.
11. Wastewater treatment units open to the atmosphere shall have restricted access and pump control
panels shall be locked.
12. The Permittee is responsible for the operation and maintenance of the entire treatment and disposal
system including, but not limited to, the following items:
a. The septic tank and Whitewater DF-50 Aerobic Treatment Unit shall be checked twice
annually for solids build-up and pumped out as needed. The septic tank effluent filter shall be
checked and cleaned at the same frequency of the septic tank.
b. The tablet chlorinators shall be checked every week. Tablets shall be added to provide proper
chlorination. Only tablets manufactured for wastewater treatment shall be applied through the
chlorinator. Swimming pool chlorination tablets shall NOT be used.
c. The storage, pump, and alarm systems shall be inspected monthly. Any sludge or scum layer
accumulating in the pump/storage tank(s) shall be removed at the same interval as the septic tank
is cleaned out.
d. The Whitewater DF-50 Aerobic Treatment Unit shall be inspected monthly to ensure the
housing and access covers are intact, that the unit has adequate ventilation and that the air filters
are clean.
e. The irrigation system shall be inspected monthly to make certain of the proper operation of
the drip line, that the vegetative growth allows a proper drip pattern, that the soil is assimilating
the disposed treated wastewater with no surface runoff, and that no objectionable odors are being
generated.
f. The standby generator shall be operated at least quarterly to ensure proper starting and operation.
The fuel shall be drained out and replaced with fresh fuel at least twice yearly.
III. MONITORING AND REPORTING REQUIREMENTS
1. Any monitoring deemed necessary by the Division to insure surface and ground water protection will
be established and an acceptable sampling reporting schedule shall be followed.
2. The Permittee shall maintain records of all maintenance performed on the system and irrigation area,
as required in Condition II(12) for a minimum of five years. This information shall be provided to
the Division upon request.
3. Noncompliance Notification:
The Permittec shall report by telephone to the Wilmington Regional Office, telephone number (910)
796-7215, as soon as possible, but in no case more than 24 hours or on the next working day
following the occurrence or first knowledge of the occurrence of any of the following:
a. Any process unit failure, due to known or unknown reasons, that renders the facility incapable of
adequate wastewater treatment, such as mechanical or electrical failures of pumps, air
compressors, emitters, etc.; or
b. Any failure of a pumping station or treatment facility resulting in a by-pass directly to receiving
waters without treatment of all or any portion of the influent to such station or facility.

Occurrences outside normal business hours may also be reported to the Division's Emergency
Response personnel at telephone number (800) 858-0368 or (919) 733-3300. Persons reporting such
occurrences by telephone shall also file a written report in letterform within five (5) days following
first knowledge of the occurrence. This report must outline the actions taken or proposed to be taken
to ensure that the problem does not recur.
IV, INSPECTIONS
1. Adequate inspection, maintenance, and cleaning shall be provided by the Permittee to insure proper
operation of the subject facilities.
2. Any duly authorized officer, employee, or representative of the Division may, upon presentation of
credentials, enter and inspect any property, premises or place on or related to the disposal site or
facility at any reasonable time for the purpose of determining compliance with this permit, may
inspect or copy any records that must be maintained under the terms and conditions of this permit,
and may obtain samples of groundwater, surface water, or leachate.
V. GENERAL CONDITIONS
1. This permit shall become void unless the facilities are constructed in accordance with the conditions
of this permit, the approved plans and specifications, and other supporting data.
2. This permit is effective only with respect to the nature and volume of wastes described in the
application and other supporting data.
3. This permit is not transferable. In the event there is a desire for the facilities to change ownership, or
there is a name change of the Permittee, a formal permit request must be submitted to the Division
accompanied by documentation from the parties involved, and other supporting materials as may be
appropriate. The approval of this request will be considered on its merits and may or may not be
approved.
4. Upon the availability of a municipal or regional sewerage collection system, the subject wastewater
treatment facilities shall be abandoned and all wastewater discharged into the municipal or regional
sewerage system.
5. Failure to abide by the conditions and Iimitations contained in this permit may subject the Permittee
to an enforcement action by the Division in accordance with North Carolina General Statute 143-
215.6A to 143-215.6C.
6. The issuance of this permit does not preclude the Permittee from complying with any and all statutes,
rules, regulations, or ordinances, which may be imposed by other government agencies (local, state,
and federal), which have jurisdiction.
7. This permit may be revoked if the Permittee .fails to abide by the conditions of the "Operation and
Maintenance Agreement" previously signed by the Permittee.
8. The Pennittee shall retain a set of approved plans, specifications and operation and maintenance
manual of the subject facility for the life of the project.
9. The Permittee shall pay the annual administering and compliance fee within 30 days of being billed
by the Division. Failure to pay the fee accordingly may'cause the Division to initiate action to revoke
this permit as specified by 15A NCAC 2T .0105 (e)(3).

10. The Perraittee, at least six (6) months prior to the expiration of this permit, shall request its extension.
Upon receipt of the request, the Commission will review the adequacy of the facilities describer)
therein, and if warranted, will extend the Permit for such period of time and under such conditions
and limitations, -as it may deem appropriate,
