Triple S Farms & Cleaning Land Application of Residual Solids (503) Program Duplin County Dear W. Smith: In accordance with your permit modification request received on September 14, 2012, we are forwarding herewith a modified Attachment A and a modified Attachment B, both certified October 18, 2012, to Triple S Farms & Cleaning, Inc: for the subject land application of residuals solids program. This certification of Attachment A is being; issued to approve additional tonnage for an approved residuals source, which is the Town of Wallace WWTP, a facility that is owned and operated by the Town of Wallace. The tonnage from this facility .will be increased from 100 dry tons per year to 380 drytons per year, and the total tonnage for this permit from all sources is therefore being increased from 276.51 dry tons per year to 556.51 dry tons per year. Please note that residuals sampling at the Town of Wallace WWTP has been changed from annually to quarterly in accordance with the sampling rules. The sampling of residuals for TCLP analyses from the Town of Beulaville has been changed from annual sampling to sampling once every permit cycle, based on an evaluation of historic TCLP monitoring data. This certification of Attachment B is being issued to modify the acreage of the existing application areas and to add a new application area to the program. Fields 1, 2, 3, 4 and 5 are having their acreages changed and Fields 6 and 7 are being added to the program. These changes will result in the total acreage being increased from 68,24 applicable acres to 96.79 applicable acres. Please replace the existing Attachment A of Permit No, WQ0030998 that was previously certified on August 1, 2012 with this modified Attachment A. Please also replace the existing Attachment B that was certified on December 9, 2009 with this modified Attachment B. It is the Permittee's responsibility to ensure that a complete permit is created by inserting this modified Attachment A into Permit No. WQ0030998, issued on December 9, 2009 and by inserting this modified Attachment B into Permit No. WQ0030998, issued on December 9, 2009. This modified Attachment A and Attachment B shall be effective from the date of certification; shall void Attachment A and Attachment B of Permit No. WQ0030998 that were previously (most recently) certified on August 1, 2012 and on December 9, 2009, respectively; and shall be subject to the AQUIFER PROTECTION SECTION 1636 Mail Serv[ce Center, Raleigh, North Carolina 27699-1636 Locaiiom 512 N. If any parts, requirements, and/or Iimitations contained in this certification or modified Attachment A are unacceptable, you have the right to request an adjudicatory hearing upon written request within 30 days following receipt of the certification. This request shall be in the form of a written petition, conforming to Chapter 150E of the North Carolina General Statutes and filed with the Office of Administrative Hearings at 6714 Mail Service Center, Raleigh, North Carolina 27699-6714. Analyses to demonstrate that residuals are non -hazardous (i.e., TCLP, ignitability, reactivity, and corrosivity) as stipulated under permit Condition IV-2. The monitoring frequency for TCLP analysis for the Town of Beulaville has been decreased to once every permit cycle. 3. Testing of metals and nutrients as stipulated under permit Condition IV.3. 4. Analyses of pathogen and vector attraction reductions as stipulated under permit Condition WA. Permit application indicated that a fecal Coliform density demonstration and lime stabilization will be performed to meet pathogen reduction requirements, and that alkaline stabilization, injection, and incorporation will be performed to meet vector attraction reductions_ However, other methods listed under 15A NCAC 02T .1106(b) and .1107(a) can also be used to demonstrate compliance with this permit requirement 5. Monitoring frequencies are based on the actual dry tons applied per year using the table below, unless specified above.
