Dear Mr. Smith:
In accordance with your permit modification application package received on December 6, 2007,
we are forwarding herewith a modified Attachment A and B, certified February 26, 2008, to Triple S
Farms & Cleaning, Inc. for the subject residuals land application program..
This certification is being issued to modify Attachment A and B by adding the Town of
Kenansville WWTP as one of the residuals source -generating facilities, combining field lA & I and
adding 8.14 acres of new land with a new total of 37.64 acres of permitted land. The maximum annual
volume of residuals is also being increased from 51.51 Dry tons to 111,51 Dry tons.
Please replace the existing Attachment A and B of Permit No. WQ0030998 that was
previously certified on February 9, 2007, with this modified Attachment A and B. Note that only the
vicinity and buffer maps for any new and modified land application sites are attached to this modified
Attachment B. It is the Permittee's responsibility to ensure that a complete permit is created by inserting
this modified Attachment B and vicinity and buffer maps into Permit No. WQ0030998, issued on
February 9, 2007, and making sure to retain any vicinity and buffer maps for any existing and unmodified
land application sites.
This modified Attachment A and B shall be effective from the date of certification; shall void
Attachment A and B of Permit No. WQ0030998 that was previously certified on February 9, 2007 and
shall be subject to the conditions and limitations as specified therein and well as those specified in the
above -referenced permit. As always, remember to take the time to review this modified Attachment A
and B thoroughly, as some of the conditions contained therein may have been changed since the last
certification. Note that this modified Attachment A and B is hereby incorporated by reference into Permit
No. WQ0030998, issued on February 9, 2007,
If any parts, requirements, and/or limitations contained in this certification or modified
Attachment A and/or Attachment B are unacceptable, you have the right to request an adjudicatory
hearing upon written request within 30 days following receipt of the certification. This request shall be in
