Triple S Farms & Cleaning Land Application of Residual Solids (503) Program Duplin County Dear Mr. Smith: In accordance with your permit modification request received on July 24, 2012, we are forwarding herewith a modified Attachment A, certified August 1, 2D12, to Triple S Farms & Cleaning, Inc. for the subject land application of residuals solids program. This certification is being issued to approve additional tonnage for an approved residuals source, which is the Town of Beulavilie WWTP, a facility that is owned and operated by the Town of Beulaville. The tonnage from this facility will be 50 dry tons per year, and the total tonnage for this permit from all sources is therefore being increased from 226.51 dry tons per year to 276.51 dry tons per year. Please replace the existing Attachment A of Permit No. WQ0030998 that was previously certified on November 18, 2010, with this modified Attachment A. It is the Permittee's responsibility to ensure that a complete permit is created by inserting this modified Attachment A into Permit No. WQ0030998, issued on December 9, 2009. This modified Attachment A shall be effective from the date of certification; shall void Attachment A of Permit No. WQ0030998 that was previously (most recently) certified on November 18, 2010; and shall be subject to the conditions and limitations as specified therein and well as those specified in the above -referenced permit. As always, remember to take the time to review this modified Attachment A thoroughly, as some of the conditions contained therein may have been changed since the last certification. Note that this modified Attachment A is hereby incorporated by reference into Permit No. WQ0030998, issued on December 9, 2009. If any parts, requirements, and/or limitations contained in this certification or modified Attachment A are unacceptable, you have the right to request an adjudicatory hearing upon written request within 30 days following receipt of the certification. This request shall be in the form of a written petition, conforming to Chapter 150B of the North Carolina General Statutes and filed with the Office of Administrative Hearings at 6714 Mail Service Center, Raleigh, North Carolina 27699-6714.
