JAY ZIMMERMAN Director Subject: Attachments A & B Certification Permit No. WQ0030998 Triple S Farms & Cleaning RLAP Duplin County In accordance with your permit modification request received September 14, 2017, we are forwarding herewith modified Attachments A & B certified November 2, 2017, to Triple S Farms & Cleaning, Inc. for the subject residuals program. This certification is being issued to modify Attachments A & B by: 1. Removal of two residual sources totaling 67.5 tons/year, those sources being Webb Creek WWTP and Maysville WWTP. 2. Addition of one residual source totaling 80.0 tons/year, that source being Little Creek WWTP. Modification of the acreage of five existing fields, those fields being Fields 1, 3, 7, 8 and 9. This modified Attachments A & B shall be effective from the date of certification until February 28, 2021 (i.e., expiration date of the current permit), shall void Attachments A & B of Permit No. WQ0030998 issued March 9, 2016, and shall be subject to the conditions and limitations as specified therein, as well as those specified in the above -referenced permit. Please replace the Attachments A & B of Permit No. WQ0030998 that was issued March 9, 2016, with these modified Attachments A & B. If any parts, requirements, and/or limitations contained in this certification or modified Attachments A & B are unacceptable, you have the right to request an adjudicatory hearing upon written request within 30 days following receipt of the certification. This request shall be in the form of a written petition, conforming to Chapter 150B of the North Carolina General Statutes and filed with the Office of Administrative Hearings at 6714 Mail Service Center, Raleigh, North Carolina 27699-6714.
