Dear Mr.Keller:
In accordance with your permit major modification request received September 20, 2012, we are
forwarding herewith Attachment A of Permit No. W00030580, dated October 24, 2012, to Earthtec of
North Carolina, Inc. for the continued operation of the subject residuals management program.
The modification of this Attachment A is for the addition of 2 new residuals sources (1,600 dry
tons per year from Smithfield Packing and 1,700 dry tons per year from Mebane W W TF) and changing of
the City of Sanford from a one-time occurrence to an annual contribution.
Please note that the`submitted analysis result of residuals from Mebane WWTF did not
pass the ignitability test; therefore, a new analysis result is required and shall be submitted to the
Division prior to acceptance of residuals from the Mebane Facility,
Please replace the existing Attachment A of Permit No. W00030580 that was previously certified
on December 23, 2011, with this modified Attachment A. It is the Pemiittee's responsibility to ensure
that a complete permit is created by inserting this modified Attachment A into Permit No. W00030580,
issued on March 30, 2007.
This modified Attachment A shall be effective from the date of certification; shall void
Attachment A of Permit No W00030580 that was previously (most recently) certified on December 23,
2011; and shall subject to the conditions and limitations as specified therein and well as those specified in
the above -referenced permit. As always, remember to take time to review this modified Attachment A
thoroughly, as some of the conditions contained therein may have been changed since the last
certification. Note that this Attachment A is hereby incorporated by reference into Permit No.
W00030580, issued on March 30, 2007.

if any parts, requirements or limitations contained in this permit are unacceptable, the Permittee
( has the right to request an adjudicatory hearing upon written request within 30 days following receipt of
this permit. This request shall be in the form of a written petition, conforming to Chapter 150B of the
North Carolina General Statutes, and filed with the Office of Administrative Hearings at 6714 Mail
Service Center, Raleigh, NC 27699-6714. Unless such demands are made, this permit shall be final and
binding.
