This permit operates in conjunction with NPDES Permit NC0020575. This permit shall be effective from the date of issuance until December 11, 2010, and shall be subject to the conditions and limitations as specified therein. Please pay particular attention to the monitoring requirements in this permit. Failure to establish an adequate system for collecting and maintaining the required operational information will result in future compliance problems. The Authorization to Construct No. 020575A01, issued December 12, 2005, authorizes the modification of the Mount Olive WWTP to a design capacity of 2.0 MGD, However, the NPDES Permit No. NC0020575, issued April 21, 2003, limits the discharge flow to 1.0 MGD. This reclaimed water permit authorizes drip irrigation onto 145.5 acres, with an annually averaged disposal capacity of 660,250 gpd (or 0.660 MGD). The combined disposal capacities of the NPDES permit and this reclaimed water permit thus is 1.660 MGD. If any parts, requirements, or limitations contained in this permit are unacceptable, you have the right to request an adjudicatory hearing upon written request within thirty (30) days following receipt of this permit. This request must be in the form of a written petition, conforming to Chapter 150B of the North Carolina General Statutes, and filed with the Office of Administrative Hearings, 6714 Mail Service Center, Raleigh, NC 27699-6714. Unless such demands are made this permit shall be final and binding. SRG NORTH CAROLINA ENVIRONMENTAL MANAGEMENT COMMISSION DEPARTMENT OF ENVIRONMENT AND NATURAL RESOURCES RALEIGH RECLAIMED WATER GENERATION, DISTRIBUTION AND UTILIZATION PERMIT In accordance with the provisions of Article 21 of Chapter 143, General Statutes of North Carolina as amended, and other applicable Laws, Rules, and Regulations 19D1I'MI&Xy[I]i;1C61 a31"1Do IIIB]I-ti] Town of Mount Olive Wayne County FOR THE construction and operation of a 660,250 gpd (annually averaged) reclaimed water generation, distribution and utilization system, consisting of a reclaimed water pump station with two (2) 1,750 gpm, 40 Hp transfer pumps and three (3) 350 gpm, 25 Hp drip irrigation pumps, an in -line automatic backwash strainer, a lined 5.4 MG reclaimed water upset lagoon, a 28.1 MG irrigation storage lagoon, and a drip irrigation system covering 145.5 acres grouped into twelve (12) fields, using 20 mm pressure -compensating drip tubing rated at 0.52 gph per emitter, with emitters spaced on 2-foot centers, with the drip tubing spaced approximately on 10-foot centers, associated header piping, lateral piping and appurtenances, with a vegetative cover of grass and trees at the Town of Mount Olive WWTP to serve the Town of Mount Olive, with no discharge of wastes to the surface waters of the State, pursuant to the application received August 5, 2005, and subsequent additional information received by the Division, and in conformity with the project plan, specifications, and other supporting data subsequently filed and approved by the Department of Environment and Natural Resources and considered a part of this permit. This permit shall be effective from the date of issuance until December 11, 2010, and shall be subject to the following specified conditions and limitations: I. PERFORMANCE STANDARDS Upon completion of construction and prior to operation of this permitted facility, a certification must be received from a professional engineer certifying that the permitted facility has been installed in accordance with this permit, the approved plans and specifications, and other supporting materials. If this project is to be completed in phases and partially certified, you shall retain the responsibility to track further construction approved under the same permit, and shall provide a final Town of Mount Olive Permit WQ0029169 December 12, 2005 Page 2 certificate of completion once the entire project has been completed. Mail the Certification to the Construction Grants & Loans Section, Division of Water Quality, 1633 Mail Service Center, Raleigh, NC 27699-1633. 2. The Washington Regional Office, telephone No. (252) 946-6481, shall be notified at least forty-eight (48) hours in advance of operation of the installed facilities so that an in -place inspection can be made. Such notification to the regional supervisor shall be made during normal office hours from 8:00 a.m. until 5:00 p.m. on Monday through Friday, excluding State Holidays. 3, The Reclaimed Water Drip Irrigation Facility shall be effectively maintained and operated at all times so that there is no discharge to the surface waters, nor any contamination of ground waters, which will render them unsatisfactory for normal use. In the event that the facility fails to perform satisfactorily, including the creation of nuisance conditions or failure of the irrigation area to adequately assimilate the wastewater, the Permittee shall take immediate corrective actions including those actions that may be required by the Division of Water Quality (Division), such as the construction of additional or replacement treatment and disposal facilities. 4. The issuance of this permit shall not relieve the Permittee of the responsibility for damages to groundwater or surface water resulting from the operation of this facility. 5. The residuals generated from these treatment facilities must be disposed of in accordance with General Statute 143-215.1 and in a manner approved by the North Carolina Division of Water Quality. 6. Diversion or bypassing of the untreated wastewater from the treatment facilities is prohibited. 7. The following buffers shall be maintained: a. 100 feet between wetted areas and water supply wells, b. 100 feet between wetted areas and waters classified as SA, C. 25 feet between wetted areas and surface waters not classified as SA, d. 100 feet between wastewater treatment units and wells, e. 50 feet between reclaimed water storage/irrigation ponds and property lines, and f. 50 feet between wastewater treatment units and property lines. 8. Public access to the irrigation sites shall be controlled during active site use. Such controls may include the posting of signs showing the activities being conducted at each site. A sign shall be posted in plain sight showing these activities. Town of Mount Olive Permit WQ0029169 December 12, 2005 Page 3 9. All reclaimed water valves, storage facilities, outlets shall be tagged or labeled to warn the public or employees that the water is not intended for drinking. Where appropriate, such warning shall inform the public or employees to avoid contact with the water. 10. All reclaimed water piping, valves, outlets and other appurtenances shall be color -coded, taped, or otherwise marked to identify the source of the water as being reclaimed water. a. All reclaimed water piping and appurtenances shall be either colored purple (i.e., Pantone 522) and embossed or integrally stamped or marked "CAUTION: RECLAIMED WATER DO NOT DRINK" or be installed with a purple (i.e_, Pantone 522) identification tape or polyethylene vinyl wrap. The warning shall be stamped on opposite sides of the pipe and repeated every three feet or less. b. Identification tape shall be at least three inches wide and have white or black lettering on purple (i.e., Pantone 522) field stating "CAUTION: RECLAIMED WATER DO NOT DRINK." Identification tape shall be installed on top of reclaimed water pipelines, fastened at least every 10 feet to each pipe length and run continuously the entire length of the pipe. C. Existing underground distribution systems retrofitted for the purpose of distributing reclaimed water shall be taped or otherwise identified as in items a. or b. above. This identification need not extend the entire length of the distribution system but shall be incorporated within 10 feet of crossing any potable water supply line or sanitary sewer line. 11. All reclaimed water valves and outlets shall be of a type, or secured in a manner, that permits operation by authorized personnel only. 12. Above -ground hose bibs (i.e., spigots or other hand -operated connections) shall not be present. Hose bibs shall be located in locked below -grade vaults that shall be clearly labeled as being of non -potable quality. As an alternative to the use of locked below -grade vaults with standard hose bibs services, hose bibs, which can only be operated by a special tool may be placed in non -lockable underground services boxes clearly labeled as non -potable water. 13. The Permittee shall maintain an active Cross -connection control program that shall have the following minimum requirements: a. No direct cross -connections shall be allowed between the reclaimed water and potable water systems. b. A reduced pressure principle backflow preventer, an approved air gap separation, or other device approved by the Division of Environmental Health shall be installed at the potable water service connection to the use Town of Mount Olive Permit WQ0029169 December 12, 2005 Page 4 area where both reclaimed water and potable water are supplied to a reclaimed water use area. The installation of the reduced pressure principle backflow prevention device shall allow proper testing. An air gap separation, approved and regularly inspected by the potable water supplier, shall be provided between the potable water and reclaimed water systems where potable water is used to supplement a reclaimed water system. II. OPERATION AND MAINTENANCE REQUIREMENTS The facilities shall be properly maintained and operated at all times_ 2. Upon classification of the wastewater treatment and irrigation facilities by the Water Pollution Control System Operators Certification Commission (WPCSOCC), the Permittee shall designate and employ a certified operator to be in responsible charge (ORC) and one or more certified operator(s) to be back-up ORC(s) of the facilities in accordance with 15A NCAC 8G .0201. The ORC shall visit the facilities in accordance with 15A NCAC 8G .0204 or as specified in this permit and shall comply with all other conditions specified in these rules. The Permittee shall install all appropriate flow measurement devices consistent with approved engineering and scientific practices to ensure accuracy and reliability of influent flow measurement_ Flow measurement devices selected shall be capable of measuring flows with a maximum deviation of less than 10 percent from true flow, accurately calibrated at a minimum of once per year, and maintained to ensure that the accuracy of the measurement is consistent with the accepted capacity of that type of device. Records of flow measurement device calibrations shall be kept on file by the Permittee for a period of at least three years. At a minimum, data to be included in this documentation shall be: a. Date of flow measurement device calibration. b. Name of person performing calibration. C. Percent from true flow. 4. A suitable year round vegetative cover over the drip irrigation fields shall be maintained. 5. An agronomic plan shall be developed which maximizes the assimilative capacity of the nutrients in the wastewater through crop growth, crop harvesting, etc. 6. Irrigation shall not be performed during inclement weather or when the ground is in a condition that will cause runoff. 7. The facilities shall be effectively maintained and operated as a non -discharge system to prevent the discharge of any wastewater resulting from the operation of this facility, unless authorized by an NPDES permit. The application rate shall not exceed an instantaneous rate of 0.4 inches per hour. No type of wastewater other than that from the Town of Mount Olive's wastewater treatment plant shall be irrigated at the Reclaimed Water Drip Irrigation Facility. 10. No traffic or equipment shall be allowed on the disposal area except while installation occurs or while normal maintenance is being performed. 11. Should the limit for fecal coliform (daily maximum concentration of 25 per 100 ml) or turbidity (instantaneous maximum of 10 NTU) be exceeded, all wastewater shall be handled in one of three ways: (a) further treat the wastewater to bring it into compliance with reclaimed water standards without returning the water to the facility's headworks, (b) reroute the wastewater to the headworks of the wastewater treatment facility for treatment to reuse standards, or (c) discharge the wastewater in strict accordance with the conditions of the facility's existing NPDES permit. 111. MONITORING AND REPORTING REQUIREMENTS Any monitoring (including groundwater, surface water, soil or plant tissue analyses) deemed necessary by the Division to insure surface and ground water protection will be established and an acceptable sampling reporting schedule shall be followed. Town of Mount Olive Permit WQ0029169 December 12, 2005 Page 6 2. WWTP reuse effluent flow shall be continuously monitored and daily flow values shall be reported on Form NDMR-I. Irrigation at the Reclaimed Water Drip Irrigation Facility shall be continuously monitored and daily flow values shall be reported on Form NDAR-1. 3. During the remainder of the year, these parameters shall be monitored monthly. ** Triannually sampling shall be conducted during March, July, and November. Town of Mount Olive Permit WQ0029169 December 12, 2005 Page 7 If Groundwater sampling indicates or predicts problems with the compliance with Groundwater Standards, this permit will be modified to include additional and/or more restrictive limitations. 4. Adequate records shall be maintained by the Permittee tracking the amount of wastewater disposed at the Reclaimed Water Drip Irrigation Facility. These records shall include, but are not necessarily limited to, the following information (and reported on Form NDAR-1): a. date of irrigation, b. volume of wastewater irrigated, C. field irrigated, d. length of time field is irrigated, C. continuous weekly, monthly, and year-to-date hydraulic (inches/acre) loadings for each field, f, weather conditions, and g. maintenance of cover crops. 5. Three (3) copies of all monitoring data (as specified in condition I113) on Form NDMR-1 and three (3) copies of all operation and disposal records (as specified in condition III 4) on Form NDAR-I shall be submitted monthly on or before the last day of the following month. A maintenance log shall be maintained at this facility including but not limited to the following items: a. Daily sampling results including residual chlorine, settleable matter, and dissolved oxygen in the aeration basin and at the clarifier weir. b. Visual observations of the plant and plant site. C. Visual observations of the spray irrigation site. d. Record of preventative maintenance (changing of filters, adjusting belt tensions, alarm testing, diffuser inspections and cleanings, irrigation pump maintenance, repairing broken drip irrigation lines, etc.) - el Date of calibration of flow measurement device. f. Date and results of power interruption testing on alternate power supply. 7. Noncompliance Notification: The Permittee shall report by telephone to the Washington Regional Office, telephone number (252) 946-6481, as soon as possible, but in no case more than 24 hours or on the next working day following the occurrence or first knowledge of the occurrence of any of the following: Town of Mount Olive Permit WQ0029169 December 12, 2005 Page 8 a. Any occurrence at the wastewater treatment facility which results in the treatment of significant amounts of wastes which are abnormal in quantity or characteristic, such as the dumping of the contents of a sludge digester; the known passage of a slug of hazardous substance through the facility; or any other unusual circumstances. b. Any process unit failure, due to known or unknown reasons, that render the facility incapable of adequate wastewater treatment such as mechanical or electrical failures of pumps, aerators, compressors, etc. C. Any failure of a pumping station, sewer line, or treatment facility resulting in a by-pass directly to receiving waters without treatment of all or any portion of the influent to such station or facility. d. Any time that self -monitoring information indicates that the facility has gone out of compliance with its permit limitations. Occurrences outside normal business hours may also be reported to the Division's Emergency Response personnel at telephone number (800) 662-7956, (800) 858- 0368, or (919) 733-3300. Persons reporting such occurrences by telephone shall also file a written report in letter form within five (5) days following first knowledge of the occurrence. This report must outline the actions taken or proposed to be taken to ensure that the problem does not recur. IV. GROUNDWATER REQUIREMENTS 1. Prior to beginning waste disposal operations, monitor wells MW-1, MW-2 and MW-3 shall be installed to monitor groundwater quality. The wells shall be constructed such that the water level in the well is never above or below the screened (open) portion of the well at any time during the year. The general location and name for each well is marked on attached Sheets I-1, I-2 and 1-5. Each monitoring well shall be constructed in accordance with this permit, and approved by the Washington Regional Office. 2. All wells that are constructed for purposes of groundwater monitoring shall be constructed in accordance with 15A NCAC 2C .0108 (Standards of Construction for Wells Other than Water Supply) and any other state and local laws and regulations pertaining to well construction. 3. The Washington Regional Office, telephone No. (252) 946-6481, shall be notified at least forty-eight (48) hours prior to the construction of any monitoring well so that an inspection can be made of the monitoring well location. Such notification to the regional groundwater supervisor shall be made during the normal office hours from 8:00 a.m. until 5:00 p.m. on Monday through Friday, excluding state holidays. Within sixty (60) days of completion of all monitoring wells, the permittee shall submit two original copies of a scaled topographic map (scale no greater than 1 ":100') signed and sealed by a professional engineer or a state licensed land surveyor that indicates all of the following information: a. the location and identity of each monitoring well (MW-1 through MW-3), b. the location of all components of the waste disposal system, C. the location of property boundaries within 500 feet of the disposal area(s), d. the latitude and longitude of the established horizontal control monument, e. the elevation of the top of the well casing (which shall be known as the "measuring point") relative to a common datum, £ the depth of water below the measuring point at the time the measuring point is established, and g. the date the map is prepared and/or revised. Control monuments shall be installed in such a manner and made of such materials that the monument will not be destroyed due to activities that may take place on the property. The permittee is responsible for the geographic accuracy of any map submitted, however produced.The maps and any supporting documentation shall be sent to: NC DENR/DWQ Information Processing Center 1617 Mail Service Center Raleigh, NC, 27699-1617 Within thirty (30) days of completion of all well construction activities, a certification must be received from a professional engineer certifying that the monitoring wells are located and constructed in accordance with the Well Construction Standards (15A NCAC 2C) and this permit. This certification should be submitted with copies of the Well Completion Form (GW-1) for each well. Mail this certification and the associated GW-1 forms to: NC DENR/DWQ Information Processing Center 1617 Mail Service Center Raleigh, NC, 27699-1617 For the initial sampling of the well as specified elsewhere in the permit, the permittee shall submit a copy of the GW-1 Form (Well Completion Form) with the Compliance Monitoring Form (GW-59) for that well. Compliance Monitoring Forms that do not include copies of the GW-1 form will be returned to the permittee without being processed. Failure to submit these forms as required by this permit may result in the initiation of enforcement activities pursuant to NC General Statutes §143-215.6. Groundwater monitor wells MW-1, MW-2, and MW-3 shall be sampled initially after construction, and thereafter every March, July and November for the following parameters: Water Level* Chloride Total Ammonia (as N) Total Dissolved Solids (TDS) pH Fecal Coliform Nitrate (NO3-N) Total Organic Carbon (TOC) Volatile Organic Carbon (VOC) analysis Only in November *The measurement of water levels must be made prior to sampling for the remaining parameters. The depth to water in each well shall be measured from the surveyed point on the top of the casing. The measuring points (top of well casing) of all monitoring wells shall be surveyed to provide the relative elevation of the measuring point for each monitoring well. For Volatile Organic Compounds (VOCs) sampled in November, use only one of the following methods: a. Standard Method 6230D, Practical Quantitation Limit (PQL) at 0.5 ug/L or less b. Standard Method 6210, PQL at 0.5 ug/L or less C. EPA Method 8021, Low Concentration, PQL at 0.5 ug/L or less d. EPA Method 8260, Low Concentration, PQL at 0.5 ug/L or less C. Another method with prior approval by the Aquifer Protection Section Chief. Any of the referenced methods used for VOCs must at a minimum, include all the constituents listed in Table VIII of Standard Method 6230D. Any method used must provide a PQL of 0.5 ug/L or less, which must be supported by laboratory proficiency studies as required by the DWQ Laboratory Certification Unit. Any constituents detected above the Method Detection Limit (MDL) but below the PQL of 0S ug/L must be qualified (estimated) and reported. Any laboratory selected to analyze parameters must be Division of Water Quality (DWQ) certified for those parameters required. The results of the sampling and analysis must be received on Form GW-59 (Groundwater Quality Monitoring: Compliance Report Form) on or before the last working day of the month following the sampling month. The compliance boundary and review boundary for wastewater disposal systems are specified by 15A NCAC 2L. For systems utilizing reclaimed water, 15A NCAC 2H .0219(k) specifies that both the compliance boundary and review boundary shall be established at the property line. An exceedance of groundwater quality standards at or beyond the compliance boundary is subject to immediate remediation action in addition to the penalty provisions under North Carolina General Statute § 143-215.6A(a)(1). V. INSPECTIONS 1. Adequate inspection, maintenance, and cleaning shall be provided by the Permittee to insure proper operation of the subject facilities. 2. The Permittee or his designee shall inspect the wastewater treatment and disposal facilities to prevent malfunctions and deterioration, operator errors and discharges which may cause or lead to the release of wastes to the environment, a threat to human health, or a nuisance. The Permittee shall keep an inspection log or summary including at least the date and time of inspection, observations made, and any maintenance, repairs, or corrective actions taken by the Permittee. This log of inspections shall be maintained by the Permittee for a period of three years from the date of the inspection and shall be made available upon request to the Division or other permitting authority. 3. Any duly authorized officer, employee, or representative of the Division may, upon presentation of credentials, enter and inspect any property, premises or place on or related to the disposal site or facility at any reasonable time for the purpose of determining compliance with this permit; may inspect or copy any records that must be maintained under the terms and conditions of this permit, and may obtain samples of groundwater, surface water, or leachate. GENERAL CONDITIONS This permit shall become voidable unless the facilities are constructed in accordance with the conditions of this permit, the approved plans and specifications, and other supporting data. 2. This permit is effective only with respect to the nature and volume of wastes described in the application and other supporting data. 3. This permit is not transferable. In the event there is a desire for the facilities to change ownership, or there is a name change of the Permittee, a formal permit request must be submitted to the Division accompanied by an application fee, documentation from the parties involved, and other supporting materials as may be appropriate. Failure to abide by the conditions and limitations contained in this permit may subject the Permittee to an enforcement action by the Division in accordance with North Carolina General Statute 143-215.6A to 143-215.6C. 5. The issuance of this permit does not exempt the Permittee from complying with any and all statutes, rules, regulations, or ordinances which may be imposed by other government agencies (local, state, and federal) which have jurisdiction, including but not limited to applicable river buffer rules in 15A NCAC 213 .0200, erosion and sedimentation control requirements in 15A NCAC Chapter 4 and under the Division's General Permit NCG010000, and any requirements pertaining to wetlands under 15A NCAC 213.0200 and 2H .0500. 6. The Permittee, within 90 days following issuance of this permit, shall request modification of NPDES Permit No. NCO020575 issued to the Town of Mount Olive wastewater treatment facility. The modification shall amend the existing NPDES permit to allow the dedicated use of reclaimed water as provided for in this permit. 7. A set of approved plans and specifications for the subject project must be retained by the Permittee for the life of the project. 8. The annual administering and compliance fee must be paid by the Permittee within thirty (30) days after being billed by the Division. Failure to pay the fee accordingly may cause the Division to initiate action to revoke this permit as specified by 15A NCAC 2H .0205 (c)(4). 9. The Permittee, at Ieast six (6) months prior to the expiration of this permit, shall request its extension. Upon receipt of the request, the Commission will review the adequacy of the facilities described therein, and if warranted, will extend the permit for such period of time and under such conditions and limitations as it may deem appropriate. 10. This permit in no way negates, precludes, or invalidates the most recent re issuance of NPDES Permit No. NC0020575, and the Permittee shall continue to comply with all conditions provided for therein.
