HomeMy WebLinkAbout20061429 Ver 2_Application_20070111o~ - I~aG V'2- The following is an abbreviated application form for those applying fora 401 Water Quality Certification to impact FERC (Federal Energy Regulatory Commission) regulated lake shore or bottom under the U.S. Army Corps of Engineers Permit GP30. This Application cannot be used for wetland or stream impacts. Please provide seven (7) copies of this application and supporting information as well as a non-refundable application fee to the Division of Water Quality of $200.00 for impacts of to lake bottom of less than 1 acre and $475.00 for impacts to lake bottom of greater or equal to 1 acre. Applications should be sent to: Division of Water Quality Wetlands Unit 1650 Mail Service Center t'~~ ~ ~ i~ f ~- ~. ; ,~- Raleigh, NC 27699-1650 t ' " ' ` ~ ~ RECEIVED I. Applicant Information 1. Owner/Applicant Information Name: Mailing Address: Telephone Number: E-mail Address: II. Project Information David Averett 1830 Ashway Dr, Cumming GA 30040 770-886-1569 david e.averett.info Attach a vicinity map clearly showing the location of the property with respect to local landmarks such as towns, rivers, and roads. Also provide a detailed site plan showing property boundaries and development plans in relation to surrounding properties. You may use the diagram below to make a hand sketch of your project. Also, see the example below for guidance. Both the vicinity map and site plan must include a scale and north arrow. The footprints of all buildings, impervious surfaces, or other facilities must be included. 1. Location County: Macon Nearest Town: Topton Subdivision name or site address (include phase/lot number): Lot #48 - Nantahala Highlands 2090 Lakeshore Dr. Topton NC 28781 Directions to site please include road numbers, landmarks, etc. (This is not necessary if an adequate vicinity map is included): i '? ~la~ ~~~ ~e~ n .;ri.~ti a ~ ?_Oi ~`fk!! P ~-,MesK~ r ~,,: ~7 qF ~~uia1 Attachment #1 & #2 -Maps 2. Describe the existing land use or condition of the site at the time of this application: Site currently has a completed residence with driveway and parking area. This lot is lakefront with approx. 220ft of shoreline. There is a permitted (Duke Power) dock in place and access steps. With the exception of the house site, the terrain is natural and very steep. Attachment #3 -Site Survey 3. Property size (acres): .5 Acres 4. Nearest body of water (stream/river/sound/ocean/lake): Lake Nantahala (aka. Aquone Lake) S. Describe the purpose of the proposed work: Shoreline Stabilization & Erosion Control 1. Rip-rap or dry stack rock 5-15 inches in diameter. 2. Installed at a 2-1 slope. 3. Erosion control will not change the basic contour of the shoreline. 4. Filter fabric will be used for additional filtration behind the stone. Current shoreline includes some native stone. This stone is of a very porous brittle nature and tends to erode quickly. The majority is not in a solid mass, but rather in stratified layers that breakup under the normal cycle of wave action and winter freeze. Attachment #4 -Site Plan 6. List the type of equipment to be used to construct the project: Due the the steepness of the terrain, most work will have to be done by manpower with the aid of manual tools (wheel barrows, shovels, etc.). Currently, the plan calls for using a chute to bring rip-rap into acatch-basin and distribute the rock via wheel barrow and manually stack. If access becomes available via an adjacent property, the use of a small "bobcat" may be possible to distribute rock. This would not require any excavation. A standard erosion control fence would be used to control the area impacted. Staging of the rock will not occur in the lake, but rather in the parking area of the property. 7. Amount of impact (including all excavation, backfill, rip rap, retaining walls, etc.) below the normal pool lake level in square feet or acres: 2200 sq. ft. (estimated) 8.a. Amount of impact (including all clearing, back fill, excavation, rip rap, retaining walls, etc.) above the normal pool lake level and SO feet land-ward in square feet or acres: 2200 sq. ft. (estimated) 8. b. Please describe vegetation above the normal pool lake level and SO feet landward to be impacted (number of trees for instance): Applicant would prefer to maintain as much of the natural vegetation as possible. Plans are to limit the number of trees impacted currently to only ones directly impacted by severe under-cut bank erosion. Current estimates are that approximately 10 trees of less than 6 inch diameter, and 1 tree (dead) of 12 inch diameter. Additional vegetation impact will be limited to underbrush, laurel, and ferns in approximately 500 sq. ft. All remaining impact will be on the shoreline bank that has previously collapsed due to under-cut erosion. Applicant plans to replant with native species per Duke Power request on all bare surfaces to control runoff.
