Because this general permit will renew very soon and all permittees will be getting a new COC next month, I am not
sending a COC out with these changes. Those renewal COCs will be available in early November. Please be sure that
Mr. Hartwood's information is correct because he will be receiving correspondence about that. You or other permit
contacts can also check our website (https://deg.nc.gov/about/divisions/energy-mineral-land-resources/npdes-
stormwater-gps) for updates and a link to those COC requests at that time.

Mr. Fusillo,
We received the attached form requesting a Name Change for the owner entity of the above permit. The form
submitted is actually for the State Storm water permit (post -construction permits) rather than the NPDES program, of
which NCG060168 is a part. However, I can still process the request if I can verify the information. I just need to verify
that ALL OTHER details associated with the original owner of this permit are also correct for Sara Lee Frozen Bakery,
LLC.
We have Mr. Bartlett Hardwood as the designated Owner -Affiliation (the person legally responsible — his email is listed
there with this entity), and the following address and phone information that was used for KSLB, LLC. Is all this
information still correct?
