This form must be completed by Permit Writers for all expedited permits which do not require
full Fact Sheets. Expedited permits are generally simple 100% domestics (e.g., schools, mobile
home parks, etc) that can be administratively renewed with minor changes, but can include
facilities with more complex issues (Special Conditions, 303(d) listed water, toxicity testing,
instream monitoring, compliance concerns).

Comments received on Draft Permit?
Permittee not eligible for General Permit
NCG59 because of required monitoring for
TN/TP, Neuse River Basin Strategy
The permit was drafted in accordance with the Greensand Filter section of the 2012 WTP
permitting guidance. Changes to the existing permit included:
1. Discontinued monitoring for Total Iron [no standard/no longer a
parameter of concern (POC)].
2. Per potable water treatment plant (WTP) strategy (2012), continued
quarterly monitoring for Total Manganese (no limit) as required of
greensand filter treatment technology.
3. Total Nitrogen / Total Phosphorus (TN/TP) remain in the permit,
monitoring semi-annually per current Neuse River Basin Nutrient
Management Guidelines.
4. Because TN/TP monitoring is required, this facility is not eligible for WTP
General Permit NCG590000.
5. Updated site map and the Supplement to Permit Cover Sheet narrative.
