Such notification to the regional supervisor shall be made during the normal office hours froxn 8:00 a.rn, until 5:00 p.m on Monday through Friday, exeluciing State Holidays. 5_ The surface irrigation facilities shall be effectively mainlWned and operated at all times so that them is no discharge to the surface waters, nor any contamination of ground waters, wMeh will render them uum isfactory for normal, use. In the event that the facilities fail to perfarm sati,sf'actorily, including the, creation of nifisance conditions or failure of the irrigation axes to adequately assimilate the wastewater, the Permittee shall tame immediate corrective actions including those actions that may be required by the Division of Water Quality, such as the construction of additional oT replacement wastewater treatment and disposal facilities. 6. The issuance of'this permit shall not relieve the Fena i'ttee of the responsibility for damkges to surface or grouudwatexs Tesultlag from the operation of this facility. 7. The residuals generated from these tLcatnent facilities must be disposed in accordance with General Statute 143 215.1 and in a manner approved by the Division. 8, Diversion or bypassing of the untreated wastewater ftm the treatment facilities is prohibited, 9. These buffers do, however, apply to modifications of the, treatment and disposal facilities that are for the purpose of tssarea,sing the flow that is tributary to the facility. These buffers do apply to any expansion or modification of the irrigation areas and apply in instances in which the sale of prop rty would cause any of the bu CRcrs now campli.eci with, for the. treatment and disposal facilities, to be violated. The applicant is advi.5ed that any modifications to the existing, facilities will require a permit modification, l0. H. OPERATION AND MAINTENANCE REQ�IRElI'iENTS 1. The facilities shall be properly maintained and operated at all tithes. 2_ Upon classification of the wastewater treatment and irzigation facilities by the Water Pollution Control Systexii Operators Certification Commission (WPCSOCC), the Puttee shall designate and employ a certified operator to be in responsible charge (ORC) and one or more certified opmtor(s) to be back-up ORC(s) of the facilities in arcordanee with 15A NCAC 8G .0201. The -ORC shall visit the facilities in accordance with 15A NCAC WG .0204 or as specified in this permit and shall crnnply with all other conditions speWi fed in these rules. 3. A suitable year round vegetative cover shall be maintained. No traffic or equipment shall be allowed on the disposal area except while installation occurs or while normal znainten.-wee is being performed. 11. Public access to the land application sites shall be controlled during activt site use. Such controls rtaay include the posting of signs showing the aetavities being conducted at each site. 12. The chlorine tablets used in the disinfwtion facility shall be ofthe kind and type specified in the plans and specifications approved by the Division. M. MONrrORING AND REPORTING RE TTLRF.1br MIS 1. Any monitoring (including groundwater, surface water, soil or plant tissue analyses) deomed necessary by the Division to insure surface and ground water protection will be established and an acceptable sampling reporting schedule shall be followed, 2. Effluent flow shall be continuously monitored and daily average flow values shall be reported on Forra NDMR. Flow may be estimated from water use records provided 'water use is 1a ctered. Daily average values shall be calculated by dividing monthly flow volume by the number of days in the month. 3. The effluent from the subject facilities shall be monitored by the Permittee at a point prior to irrigation every March, July, and November far the following parameters. Length of time field is irrigated, e.. Continuous weekly, monthly, and year-to-date hydmulio (incheslacre) loadings for each field, is Weather conditions, and g. Maintenance of cover Cups. 5. Three (3) copies of all manitrn'ing data [as specified in Conditions UK2) and M(3)] on Form NDMR 1 and three (3) copies of all operation and disposal records [as specified in Conditions =4) and I f(5)] on Form NDAR-1 shall be submitted on or before the last day of the following month. This report must outline the actions taken or proposed to be taken to umsure that tyre problem does not recur. TV. G1x.OIll"WATM RE UHIEMENTS 1. Well Coinstruction / A.ba donmerrt Crteris. a. Prior to begixrning waste disposal opeara-ions, three monitor wells shall be installed to monitor groundwater quality. The wells shall be constructed such that the water level in the well is ney'cr above or below the screened (open) portion of the well at any tine during the year. The general. location and name for each well is xnarked on Figures 1 &2. A North Carolina Certified Well Contraator, the property owner, or the property lessee according to General Statues 87-98.4(b) (2) mWt construct the vti+ell(s}. All well(s) must be constructed according to the North Carolina Well Construction Standards (15A NCA.0 2C .0108) and my local county roles. o_ Within sixty (60) clays of permit issuance, all water supply wells on -site shall be permanently abandoned. Within thirty (30) days of abandonment:, a Well Abando=nmt Rzoord (OW-30 form) shall be complmd for each well abandoned and mailed to the address listed iia the "Reporting I Documentation" section of the Groundwater.Requirements. The weli(s) must be, abandoned by a North Carolina Certified Well Contractror according to the North Carolina. Well Construction Standards (15A NCAC 2C .01.13) and local county rules- d. Such notification to the regional aquifer protection supervisor shall be made during the normal office hours ftrorn 8:00 a.m- until 5:00 p.m. on Monday through Friday, exclading state holidays_ e_ Within tlsirty (30) days of cotrnpletion of all well constration activities, a certification rant be received from a professional engineer or a licensed geologist certifying that the monitoring wells are located according to this permit. 2. Sampling Criteria. a. Mian:itor wells MW-1, MW-2, and MW-3 shall be sarnpled initially after construction and prior to waste disposal operations, and thereafter every Mxrclr„ July, and November for the parameters listed below. Prior to sampling the parameters, the measurement of Water levels must be taken. The depth to water in each well shall be measured franc the surveyed point on the top of the casing- The meawxing; points (top of well casing) ofall monitorin8 wells shall be surveyed relative to a common datum. The results of th-- sampling. and analysis must be received on Farm OW59 (Groundwater Quality Monitoring: Compliance Report Form) by the Groundwater Section (address listed in the "Relaorting / DocumenTata=" section, of the Grounclwattr Requirers .lt.$), on or before the last working day of the month following the sampling month- Tha data of all groundwater sampling analyses required by the permit rondhions must be reported using the most :recent OW 59 farm along with attached copies of the laboratory analyses. d. Any exceedance of Fstandazds at the Review Boundary shall require action in accordance with 15A NCAC 2L .0106(d)(1). V_ INSPECTIONS 1. Adequate inspection, maiutenance, and cleauii7,g shall be prodded by the Pennittee to insure proper operation of the subject facilities_ 2. The Permittee or his designee shall inspect; the wastewater treatment and disposal laoi lities to prevent malfunctions and deterioration, operator errors aril discharges which may cause or lead to the release of wastes to the cnvirom ent, a threat to human health, or a. nuisance. The Pennittee shall keep an inspection log or summary including at least the date and time of inspectian, observations made, and aray maintananoa, repairs, or corrective actions taken by the Pe=Jtten. This log of inspections shall be maintained by the Permittee for a period of three years from the date of the inspection and shall be made aysdabie upon request to the Divi.siori or pother pe rit6ng authority. 3. Any duly authorized officer, employee, or representative of the Division may, upon presentation of eredentials, enter and inspect any property, premises or place on or related to the disposal site or facility at any reasonable time for the purpose of determining compliance with this permit; may inspect or copy arty records that xzmst be rnainta.iiXed under the terans and conditions of this permit, and may obtain samples of groundwater, surface watar, or lewlaate. `VI. GENERAL CONDITIONS 1.. T.Ws pest shall beco= voidable unless the facilities are constructed in accordanca with the conditions of this permit, the approved plans and specifications, and otlux supporting data. 2. This permit is effective only with respect to the nature and volume of wastes described in the application and other supporting data- 3. This permit is not transkTable. In the event there is a desire for the facilities tp changg ownership, or there is a name chalage of the Permaittee, a fannal permit request must be submitted to the Divisi❑n accompanied by an application free, documentation from the parties i avolveed, and other Supporting materials as may be appropriate. The approval of tbzs request will be considered on its merits and may or may not be approved. 4. Failure to abide by the conditions and limitations contained in this permit rzay subject the Permittee to an enforcement action by the Division in accordance with Nortb Carolina General Statute 143- 215.6Ato 143-215AC, S. The issuance of fts permit does riot exempt the Permittee from complying with any and all statutes, rules, regulatiox-is, or ordinances wbieh may be imposed by other gov'eraroent agencies (local, state, and federal) which have jurisdiction, including but not limited to applicable river buffex rules an 1SA NCAC 2B .0200, erosion and sedimentation control requiT ents in 15A. NCAC Chapter 4 and under the Division's General Permit MG010000, and any requirements pertaining to WWands under 15A, NCAC 2D .0200 and 2H .4500. 6_ The 'Pezxxdttee shall retain a set of approved plazas and specification for tfe life of the project. i.
