continued operation of the subjml wastewater treatment and surface irrigation facilities.
This permit shall be effective from the date of issuance until May 31, 2010, slull void Permit No.
WQ0024053 issued Januauy 24, 2005, and shall be subject to the conditions and limitations as specified
therein. Please note that modifhcabons to file permit require tbat the buffer setback requirement from
drailrageways and surface water bodies (Permit Condition I(9xd)) has now been reduced from 50 feat to
10 feet, that harvested grass shall now be removed from the treatment area (Permit Condition 11(3)), and
that the requirement for having an autornatically activated standby power source (Permit Condition.U(9))
has now been alminged_ Please: pay particular attention to the monitoring requirements in this permit.
Failure to establish an adequate system for collecting and maintaining the required operational
information will result in future compliance problems.
If any parts, requirements, or limitations contained in this permit are unacceptable, you have the
right to request an 4udicatory hearing upon written request within thirty (30) days following receipt of

construction and operation of a 1,200 gallon per day (GPD) surface irrigation treatment and disposal
facility consisting of two (2) 1,200 gallon interconnected baiffied septic tanks with effluent filters, a 2,500
gallon Env&ofilter fiberglass tank with a 32 gallon per rninate (GPM) recirculation pump, a 33 GPM sand
filter dosing pump, fiewmeter and audio/visual high water alarms, chlorine disinfection, a fiberglass
covered high rate sand .filter, a 30 gallon purnp basin with a 34 GPM pump, a 6,000 gallon storage/pump
tank with a 1.5 horsepower (hp) field dosing pump rated at 45 GPM for dosing and 40 GPM for flushing
and audiohnsuat high water alarms, a rain sensor, a 1.307 aoxe drip irrigation field consisting of tln-ee (3)
zones and approximately 5,994 emitters rated at 0.42 GPM to serve the Cinot nati Thermal Spray facility,
with x)o discharge of wastes to the surface waters, parsuant to the application rmeived June 1, 2005, and
subsequent additional information received by the Division, and in conforrni,ty with the project plan,
specifications, and other suppoiting data. subsequently filed and approved by the Department of
Environment and Namml Resources and considered a part of this permit,
This permit shall be effective from the date of issuance until May 31, 2010, shall void Permit NO.

limitations:

1. The I?ertraittee leases the prcrjeet siir property from Glenmoranie Limited 1'zatnership, and has a lease
agree=nt in effect until Qotober 31,,2012. Texmination or extension of the lease agreement, as well
as change of ownership, shall require the Permittee to submit for modification of the permit,
2. The Permittee shall have onsite at least one (1) back-up pump for each pump in use 0.e., back-up for

4, The Wilmington Regional Office, telephone number [phone], shall be notified at least fo ty-
eight (48) hours in advance of operation of the installed facilities so that an in -place ixaspection can be

5_ The surface irrigation facilities shall be effectively mainlWned and operated at all times so that them
is no discharge to the surface waters, nor any contamination of ground waters, wMeh will render them
uum isfactory for normal, use. In the event that the facilities fail to perfarm sati,sf'actorily, including
the, creation of nifisance conditions or failure of the irrigation axes to adequately assimilate the
wastewater, the Permittee shall tame immediate corrective actions including those actions that may be
required by the Division of Water Quality, such as the construction of additional oT replacement
wastewater treatment and disposal facilities.
6. The issuance of'this permit shall not relieve the Fena i'ttee of the responsibility for damkges to surface
or grouudwatexs Tesultlag from the operation of this facility.
7. The residuals generated from these tLcatnent facilities must be disposed in accordance with General
Statute 143 215.1 and in a manner approved by the Division.
8, Diversion or bypassing of the untreated wastewater ftm the treatment facilities is prohibited,

ownership,

Some of the buffers specified above may not have bmm included in previous permits for this waste
treatment and disposal sy'sten-.L These buffers are not izzteaaded to prohibit or prevent modifinatiorss,
which arc required by the Division, to improve perform== of &c existing treatment facility. These
buffers do, however, apply to modifications of the, treatment and disposal facilities that are for the
purpose of tssarea,sing the flow that is tributary to the facility. These buffers do apply to any
expansion or modification of the irrigation areas and apply in instances in which the sale of prop rty
would cause any of the bu CRcrs now campli.eci with, for the. treatment and disposal facilities, to be
violated. The applicant is advi.5ed that any modifications to the existing, facilities will require a permit
modification,
l0. A leakage test shall be performed can the septic tank and dosing task to imure that any exfiltration
occurs at a rate, which does not 5xceed twenty (20) gallons per twenty -faux (24) hours peT 1,000
gallons of Wmk capacity. The engineer"s certifioarion will serve as proof of compliance with this

suitable for waste disposal, either in its natural state or 'which. has been modified by planting a
vegetative cover of grasses or low growing shrubbery.
H. OPERATION AND MAINTENANCE REQ�IRElI'iENTS
1. The facilities shall be properly maintained and operated at all tithes.
2_ Upon classification of the wastewater treatment and irzigation facilities by the Water Pollution
Control Systexii Operators Certification Commission (WPCSOCC), the Puttee shall designate and
employ a certified operator to be in responsible charge (ORC) and one or more certified opmtor(s) to
be back-up ORC(s) of the facilities in arcordanee with 15A NCAC 8G .0201. The -ORC shall visit
the facilities in accordance with 15A NCAC WG .0204 or as specified in this permit and shall crnnply
with all other conditions speWi fed in these rules.
3. A suitable year round vegetative cover shall be maintained. Harvested grass shall be removed from
the treatment area.

the irrigation area.
9- A portable standby generator tmit shall be available at all times to serve as au alternate power supply
in the event of a long --term power interruption. Short-term power intemptions will, require that the
plant be shut down until power is restored.

10. No traffic or equipment shall be allowed on the disposal area except while installation occurs or while
normal znainten.-wee is being performed.
11. Public access to the land application sites shall be controlled during activt site use. Such controls
rtaay include the posting of signs showing the aetavities being conducted at each site.
12. The chlorine tablets used in the disinfwtion facility shall be ofthe kind and type specified in the plans
and specifications approved by the Division.
M. MONrrORING AND REPORTING RE TTLRF.1br MIS
1. Any monitoring (including groundwater, surface water, soil or plant tissue analyses) deomed
necessary by the Division to insure surface and ground water protection will be established and an
acceptable sampling reporting schedule shall be followed,
2. Effluent flow shall be continuously monitored and daily average flow values shall be reported on
Forra NDMR. Flow may be estimated from water use records provided 'water use is 1a ctered. Daily
average values shall be calculated by dividing monthly flow volume by the number of days in the
month.
3. The effluent from the subject facilities shall be monitored by the Permittee at a point prior to
irrigation every March, July, and November far the following parameters.

4. The Permittee tracking the amotmt of wastewater disposed shall maintain adequate records. These
.records shall include, but are not necessarily limited to, the following information:
a. Data of irrigatiotx,

d. Length of time field is irrigated,
e.. Continuous weekly, monthly, and year-to-date hydmulio (incheslacre) loadings for each field,

5. Three (3) copies of all manitrn'ing data [as specified in Conditions UK2) and M(3)] on Form NDMR
1 and three (3) copies of all operation and disposal records [as specified in Conditions =4) and

1. Well Coinstruction / A.ba donmerrt Crteris.
a. Prior to begixrning waste disposal opeara-ions, three monitor wells shall be installed to monitor
groundwater quality. The wells shall be constructed such that the water level in the well is ney'cr
above or below the screened (open) portion of the well at any tine during the year. The general.
location and name for each well is xnarked on Figures 1 &2. Each monitoring well shall be located
at the Review boundary, constructed in accoz^ciance with this permit, and approved by the
Wilmington Regional Office.

b. A North Carolina Certified Well Contraator, the property owner, or the property lessee according
to General Statues 87-98.4(b) (2) mWt construct the vti+ell(s}. If a certified well eootrsctor [lees
not perfmm the construction, the property owner or lessee, pro -Ailed that they are a natural
persona, -inust physically perform the actual well construction activities. All well(s) must be
constructed according to the North Carolina Well Construction Standards (15A NCA.0 2C .0108)
and my local county roles.
o_ Within sixty (60) clays of permit issuance, all water supply wells on -site shall be permanently
abandoned. Within thirty (30) days of abandonment:, a Well Abando=nmt Rzoord (OW-30 form)
shall be complmd for each well abandoned and mailed to the address listed iia the "Reporting I
Documentation" section of the Groundwater.Requirements. The weli(s) must be, abandoned by a
North Carolina Certified Well Contractror according to the North Carolina. Well Construction
Standards (15A NCAC 2C .01.13) and local county rules-

2. Sampling Criteria.
a. Mian:itor wells MW-1, MW-2, and MW-3 shall be sarnpled initially after construction and prior to
waste disposal operations, and thereafter every Mxrclr„ July, and November for the parameters
listed below. Prior to sampling the parameters, the measurement of Water levels must be taken.
The depth to water in each well shall be measured franc the surveyed point on the top of the
casing- The meawxing; points (top of well casing) ofall monitorin8 wells shall be surveyed
relative to a common datum.

Monitoring: Compliance Report Form) by the Groundwater Section (address listed in the
"Relaorting / DocumenTata=" section, of the Grounclwattr Requirers .lt.$), on or before the last
working day of the month following the sampling month- Tha data of all groundwater sampling
analyses required by the permit rondhions must be reported using the most :recent OW 59 farm
along with attached copies of the laboratory analyses.
d. Wiazin sixty (60) mays of completion of all unonitoring wells, the pern-dttee shall submit two
original copies of scaled site romp (scale no greaterthan 1"=100); however, special provisions
rimy be granted upon prior approval for large propczties. The map(s) must include the foIlow#xg

1) The location and identity of each monitoring well.

2) The location of property boundaries within 500 feet of the dispersal area(s).
3) The latitude and longitude of the established horizontal control monuacent.

relative to a common datum.
5) The depth of water below the Measuring pout at the ti= the meastaxn point is established.
6) The location of Review and C=Vliance boundaries.
7) The date the map is prepared and/or revised.

a. Waste shall not be applied or discharged onto or below the land surface when the vertical
sepaxation between the waste and the seasonal high water table is less dun one (1) foot.
5, Applkable Boundaries:
a. The COMFULkNCE, ! C)OUNDARY for the disposal system is specified by regulations in 15A
NCAC 2L, Groundwater Classifications and Staudards. The Compliance Boundary for the
disposal system individually permitted on or after December 30, 19K is established at either 250
feet froxa d= waste disposal area., or 50 feet within tho property boundary, whichever is olosest to
thce waste disposal area. Ati exceedance of Groundwater Quality Standards at or beyond the
Compliance Boundary is subject to re=diatiou action according to 15A N- CAC 2L .UlWd)(2),

b. The REVIEW RO NDAItSt is established around the disposal systerns =dway between the
Compliance BOou ochry and the perimeter of the waste disposal area. Any exceedance of Fstandazds
at the Review Boundary shall require action in accordance with 15A NCAC 2L .0106(d)(1).
V_ INSPECTIONS
1. Adequate inspection, maiutenance, and cleauii7,g shall be prodded by the Pennittee to insure proper
operation of the subject facilities_
2. The Permittee or his designee shall inspect; the wastewater treatment and disposal laoi lities to prevent
malfunctions and deterioration, operator errors aril discharges which may cause or lead to the release
of wastes to the cnvirom ent, a threat to human health, or a. nuisance. The Pennittee shall keep an
inspection log or summary including at least the date and time of inspectian, observations made, and
aray maintananoa, repairs, or corrective actions taken by the Pe=Jtten. This log of inspections shall
be maintained by the Permittee for a period of three years from the date of the inspection and shall be
made aysdabie upon request to the Divi.siori or pother pe rit6ng authority.
3. Any duly authorized officer, employee, or representative of the Division may, upon presentation of
eredentials, enter and inspect any property, premises or place on or related to the disposal site or
facility at any reasonable time for the purpose of determining compliance with this permit; may
inspect or copy arty records that xzmst be rnainta.iiXed under the terans and conditions of this permit,
and may obtain samples of groundwater, surface watar, or lewlaate.
`VI. GENERAL CONDITIONS
1.. T.Ws pest shall beco= voidable unless the facilities are constructed in accordanca with the
conditions of this permit, the approved plans and specifications, and otlux supporting data.
2. This permit is effective only with respect to the nature and volume of wastes described in the
application and other supporting data-
3. This permit is not transkTable. In the event there is a desire for the facilities tp changg ownership, or
there is a name chalage of the Permaittee, a fannal permit request must be submitted to the Divisi❑n
accompanied by an application free, documentation from the parties i avolveed, and other Supporting
materials as may be appropriate. The approval of tbzs request will be considered on its merits and
may or may not be approved.
4. Failure to abide by the conditions and limitations contained in this permit rzay subject the Permittee
to an enforcement action by the Division in accordance with Nortb Carolina General Statute 143-
215.6Ato 143-215AC,
S. The issuance of fts permit does riot exempt the Permittee from complying with any and all statutes,
rules, regulatiox-is, or ordinances wbieh may be imposed by other gov'eraroent agencies (local, state,
and federal) which have jurisdiction, including but not limited to applicable river buffex rules an 1SA
NCAC 2B .0200, erosion and sedimentation control requiT ents in 15A. NCAC Chapter 4 and under
the Division's General Permit MG010000, and any requirements pertaining to WWands under 15A,
NCAC 2D .0200 and 2H .4500.
6_ The 'Pezxxdttee shall retain a set of approved plazas and specification for tfe life of the project.
i. The Permittee must pay the annual administering and cornpliauce fee within thirty (30) days afar
beiuz billed by the Division. Failure to pay the fee accordingly i=Y cause the Division to midate
action to revoke this permit as specified try ISA NCAC 2H .0205 (c)(4)_

8. The Pertnittee, at hmst six (6) months prior to the expiration of this permit, shall request its
ex.tenaion. Upon receipt of the request, the Commission will review the adequacy of the
facilities described therein, and if warranted, will extend the permit for such period of time
and under such co:aditioas and limitatioRs as it may deem appropriate.
9. The subject wastewater tmatcnent and disposal facilities shall he comected to an operational publicly
owned wastewater collection system within Igo days of its availability to the wbsect facilities, if the
subject wastewater- treatrnmt or disposal facilities are in nomompliance with the terms and conditions
of this non-rliscliarge pem it or the governing statutes or regulations. Prior to the initiation of these
connection activities, appropriate approval -gist be received from this Division.
