In accordance with your permit renewal request received July 21, 2008, we are forwarding
herewith Permit No. WQ0023693, dated October 20, 2008, to Brunswick County for the construction and
continued operation of the subject wastewater treatment, reclaimed water utilization, and high -rate
infiltration facilities. Construction activities permitted in March 3, 2008 have not been completed as of
the time of this permit issuance and are hereby included in this permit. These construction activities
include: a 3 million gallon per day (MGD) treatment works expansion, the addition of a septage receiving
station, the addition of eleven (11) high -rate infiltration basins, and the addition of approximately 88.74
acres of wetted acreage for reclaimed irrigation
This permit shall be effective from the date of issuance until September 30, 2013, shall void
Permit No. WQ0023693 issued March 3, 2008, and shall be subject to the conditions and limitations as
specified therein. Please pay particular attention to the monitoring requirements in this permit. Failure to
establish an adequate system for collecting and maintaining the required operational information will
result in future compliance problems.
If any parts, requirements, or limitations contained in this permit are unacceptable, you have the
right to request an adjudicatory hearing upon written request within 30 days following receipt of this
permit. This request must be in the form of a written petition, conforming to Chapter 150B of the North
Carolina General Statutes, and filed with the Office of Administrative Hearings, 6714 Mail Service
Center, Raleigh, NC 27699-6714. Unless such demands are made this permit shall be final and binding.

continued operation of the existing 600,000 GPD reclaimed water distribution and disposal system
located at St. James Plantation and the existing 500,000 reclaimed water distribution and disposal system
located at Winding River Plantation, which consist of the following:
The St. James Plantation Distribution and Disposal Facilities, consisting of:
200 GPM pump station with dual pumps, high water alarms; approximately 600 linear feet of 4-inch force
main and approximately 4,100 linear feet of 10-inch force main from. the WWTP effluent pumping
chambers to the storage pond; a 3,000,000 gallon storage/irrigation lagoon; a 1,050 GPM irrigation pump
station with a 30 HP pump, a 75 HP pump and high water alarms; and approximately 102.4 acres of spray
irrigation area;
The Winding River Plantation Distribution and Disposal Facilities, consisting of:
a 13,000,000 gallon storage and irrigation pond, and approximately 150 acres of irrigation area;
continued operation of the 3.0 MGD West Brunswick Regional Water Reclamation Facility, the
reclaimed water distribution system, and the West Brunswick Drip Irrigation Facility, which consist of
the following:
The 3.0 MGD West Brunswick Regional Water Reclamation Facility, consisting of:
• approximately 7,000 linear feet of 24-inch sanitary sewer force main, approximately 6,000 linear
feet of 24-inch reclaimed water force main, and approximately 2,000 linear feet of 8-inch water
force main, two air release valves and associated appurtenances,
• two influent magnetic flowmeters,
• plant headworks facility, to include one manual and two mechanical bar screens, vortex grit
removal with associated dual 220 GPM grit pumps, screening washer and grit classifier,
• two unequal -sized equalization basins totaling 750,000 gallons, with dual 1,400 GPM, 10 Hp
pumps, jet aeration using dual 1,620 scfm and dual 540 scfm air blowers (Blower Building No. 1)
in addition to one (1) 2,900 GPM, 25 Hp pump and one (1) 1,100 GPM, 10 Hp pump,
• oxidation ditch flow splitter box,

• dual oxidation ditches, with each ditch specified as: 76 feet wide, 10-foot side water depth,
950,000 gallon volume, and each ditch includes three (3) 4.9 Hp submersible mixers, three (3) 75
Hp 30-foot brush aerators, and one (1) adjustable effluent weir,
• two (2) 70-foot diameter, 16-foot side water depth center -fed secondary clarifiers with dual 20
GPM, 7.5 Hp scum pumps,
• dual rotating disk package filters, with a combined rating of 3.0 MGD design flow and 7.5 MGD
peak flow, with each filter system consisting of a stainless steel tank with cloth rotating disk
filters rated at 3 gpm/ftz with automatic backwash, miscellaneous electrical and piping work
associated with the rotating filters,
• a 78,000 gallon chlorine contact tank with sodium hypochlorite disinfection,
• a sodium hypochlorite chemical feed and storage building,
• effluent reclaimed water pump station consisting of two (2) 2,800 GPM, 125 Hp vertical turbine
pumps and two (2) 1,400 GPM, 100 Hp vertical turbine pumps with magnetic flowmeters,
• a 15 million gallon (approximate) upset pond lined with 40 mil polypropylene synthetic Iiner, and
a dual 2,100 GPM, 40 Hp upset pond pump station,
• RAS/WAS pump station consisting of three (3) 1,400 GPM, 15 Hp centrifugal RAS pumps and
two (2) 100 GPM, 7.5 Hp centrifugal WAS pumps,
• a 238,000 gallon WAS storage tank with jet aeration using dual 320 sefm air blowers (Blower
Building No. 2) and one (1) 1,465 GPM, 15 Hp pump,
• dual 250 GPM WAS thickener feed pumps,
• a WAS thickening building, consisting of a 1-meter gravity belt thickener with approximately
2,000 lbs/hour capacity, polymer tank and feed system, with dual 250 GPM, 15 Hp positive
displacement thickened WAS pumps,
• an autothermal thermophilic aerobic digestion (ATAD) facility for producing Class A biosolids
consisting of a dual 20-foot diameter, 21-foot covered concrete ATAD tanks, a 70-font diameter,
17-foot covered concrete storage tank, a 2,495 sq. foot ATAD Building housing dual 250 GPM,
15 Hp ATAD feed pumps, dual 40 Hp jet aeration pumps, one 50 Hp jet aeration pump, dual 5
Hp sludge transfer pumps, 250,000 BTU/hr heat -exchanger, five (5) 15 Hp blowers, ammonia
scrubber and biofilter, and associated piping, appurtenances, and control systems,
• a sludge loading station (for trucks) with 250 GPM sludge transfer pump,
• plant sanitary sewer drain pump station consisting of two (2) 2,100 GPM, 50 Hp submersible
PUMPS,
• two odor control systems, one rated at 2,000 efm and one rated at 8,400 cfm,
• three (3) non potable water pumps, one rated at 200 GPM, 15 Hp pump and two (2) rated at 75
GPM, T 5 Hp pumps,
• a 3,456 sq. foot administration building,
• a 2,240 sq. foot maintenance building,
• associated electrical work, including two (2) 1,600 kW emergency generators with automatic
transfer switches,
• SCADA instrumentation and control systems,
• associated yard piping and appurtenances, and
The reclaimed water distribution system, consisting of-

geomembrane protective liner in inside berm walls,
• Pump Station effluent reclaimed water pump station consisting of a manual bar screen, two (2)
3,300 GPM, 150 Hp vertical turbine pumps and one (1) 1,600 GPM, 75 Hp vertical turbine pump
with two (2) magnetic flowmeters, disk filtration system, a sodium hypochlorite chemical storage
and feed system, .
• a drip irrigation system covering 497.32 acres grouped into the Southern Management Area
(Zones 1 — 13) and the Northern Management Area (Zones 14-15 and 17-29), with drip tubing
spaced approximately on 10-foot centers, associated header and lateral piping and associated
appurtenances, associated forest clearing and planting of Loblolly Pine seedlings, and access
roads,
• SCADA instrumentation and control systems, and
• associated yard piping and appurtenances
construction and operation of the 3.0 MGD West Brunswick Regional Water Reclamation Facility
expansion, reclaimed water distribution system, and high -rate infiltration system, which consist of the
following: one (1) 15 MGD influent flow meter; one (1) mechanical bar screen with a 0.25 inch spacing;
vortex grit removal with associated Grit Chamber, a 220 GPM grit pump screening washer and grit
classifier; an alkalinity feed system with a 5,000 gallon chemical storage tank, two (2) 0.5 GPM alkalinity
feed pumps, two (2) 22 GPM alkalinity recirculation pumps and two (2) 0.68 GPM alum feed pumps; two
(2) unequal -sized equalization basins totaling 840,700 gallons with two (2) 1,400 GPM transfer pumps,
one (1) 1,100 GPM jet aeration pump, one (1) 2,900 GPM jet aeration pump, two (2) 320 scfm air
blowers, two (2) 790 scfrn air blowers; an oxidation ditch 15 MGD influent flow meter and a flow splitter
box; two (2) 0.95 million gallon Oxidation Ditches each with three (3) 5 hp submersible mixers, three (3)
60-hp 30-foot brush aerators, and one (1) adjustable effluent weir; two (2) clarifiers with two (2) 20 GPM
scum pumps; a RAS / WAS pump station with one (1) 4,000 GPM RAS ultrasonic flow meter, one (1)
WAS 250 GPM ultrasonic flow meter, three (3) RAS 1,400 GPM vertical non -clog centrifugal pumps,
two (2) WAS 100 GPM pumps; two (2) rotating disk package filters, with a combined rating of 3.0 MGD
design flow and 7.5 MGD peak flow, with each filter system consisting of a stainless steel tank with cloth
rotating disk filters rated at 3 gpm/ftz with automatic backwash; two (2) 30 ft x 60 ft chlorine contact
tanks ,a sodium hypochlorite chemical feed and storage building with four (4) 0.5 GPM chemical feed
pumps; a turbidimeter; one (1) 238,000 gallon aerated WAS storage tank with one (1) 1,465 jet aeration
pump, and two (2) 510 scfm blowers; two 141,000 gallon ATAD digesters each with dual 3,500 jet
aeration mixing pumps, dual 750 GPM foam control pumps, and dual 275 scfm blowers; one (1) 400
GPM gravity thickener belt; an effluent pumping station with two (2) 2,800 GPM vertical turbine effluent

(2) 75 scfm blowers and two (2) 50 GPM sludge pumps, a 6 ft diameter x 6-foot long vacuum drum filter,
a 510 scfm 40 hp vacuum pump, a receiver tank, a filter tank, an air / liquid separator, a 80 GPM receiver
pump, a control panel; the reconstruction of the 40 mil lined 5-day upset pond to increase the storage
from 15 MG to 30MG; a 20,000 gallon infiltration pump tank; approximately 10,460 linear -feet of 16-
inch reclaimed force main to North 1P Dispersal Site, and all associated piping, valves and appurtenances,

construction of the IP Tract effluent disposal site consisting of: approximately 50.88 acres for reclaimed
water irrigation and two (2) 1,200 GPM spray irrigation pumps; one 1.94 acre high -rate infiltration basin
with a loading rate of 1.4 gpd/ft (IP #1), one 1.09 acre high -rate infiltration basin with a loading rate of
1.4 gpd/ft (IP #2), one 1.35 acre high -rate infiltration basin with a loading rate of 1.4 gpd/ft (IP #3), one
1.85 acre high -rate infiltration basin with a loading rate of 1.4 gpd/ft (IP #4), one 1.66 acre high -rate
infiltration basin with a loading rate of 1.4 gpd/ft (IP #5), one 0.98 acre high -rate infiltration basin with a
loading rate of 1.4 gpd/ft (IP #6), one 1.54 acre high -rate infiltration basin with a loading rate of 1.4
gpd/ft (IP # 7), one 1.84 acre high -rate infiltration basin with a loading rate of 1.4 gpd/ft (IP #8), and one
1.63 acre high -rate infiltration basin with a loading rate of 1.4 gpd/ft (IP #9); approximately 14,167 linear
feet of 8-inch perforated PVC groundwater lowering drain with clay anti -seep collars and in -lime clean
outs, and a PVC curtain membrane, three (3) groundwater lowering pump stations each with two (2) 650
GPM submersible dewatering pumps and magnetic flow meter; and all associated piping, valves, and
appurtenances;
construction of the Mercer Mill Tract effluent disposal site consisting of: approximately 37.86 acres for
reclaimed water irrigation and two (2) 1,300 GPM spray irrigation pumps; one 739 acre high -rate
infiltration basin with a loading rate of 2.0 gpd/ft (MM ##1), one 5.16 acre high -rate infiltration basin with
a loading rate of 2.0 gpd/ft (MM #1); approximately 6,088 linear feet of 10-inch perforated PVC
groundwater lowering drain with clay anti -seep collars and in -line clean outs, two (2) groundwater
lowering pump stations each with two (2) 1,300 GPM submersible dewatering pumps and magnetic flow
meter; and all associated piping, valves, and appurtenances
to serve Brunswick County, with no discharge of wastes to the surface waters, pursuant to the application
received July 21, 2008 and subsequent additional information received by the Division of Water Quality
(Division), and in conformity with the project plan, specifications, and other supporting data subsequently
filed and approved by the Department of Environment and Natural Resources and considered a part of
this permit.
This permit shall be effective from the date of issuance until September 30, 2013, shall void Permit
No. WQ0023693 issued March 3, 2008, and shall be subject to the following specified conditions and
limitations:
I. SCHEDULES [please review the entire permit for reporting, monitoring, and other on -going activities]
At least 120 days prior to completion of construction and prior to operation of this permitted facility's
modifications, a Wetlands Monitoring Plan shall be submitted to the Division for written approval.
This plan shall include monitoring of nearby, reference wetland sites in addition to wetlands near the
spray fields and groundwater lowering devices for comparative purposes. Pursuant to paragraph (b)
of Rule 15A NCAC 02B .0231, the Wetlands Monitoring Plan shall outline the monitoring program
to support the biological and physical characteristics naturally present in wetlands to protect and
prevent adverse impacts on: water currents, erosion or sedimentation patterns; natural water
temperature variations; the chemical, nutrient and dissolved oxygen regime of the wetland; the
movement of aquatic fauna; the pH of the wetland; and water levels or elevations. Mail 4 copies of
the Monitoring Plan to the Aquifer Protection Section, Division of Water Quality, 1636 Mail Service
Center, Raleigh, NC 27699-1636. Please note that any diversion from the Wetlands Monitoring Plan
or any adverse impact on the surrounding wetlands shall result in immediate corrective action
including the removal of the groundwater lowering discharge from the permitted facility.

2. Upon completion of construction and prior to operation of this permitted facility's modifications, a
certification (see attached form) :trust be received from a professional engineer certifying that the
permitted facility's modifications have been installed in accordance with this permit, the approved
plans and specifications, and other supporting materials including the location of all monitoring wells
as applicable. If this project is to be completed in phases and partially certified, you shall retain the
responsibility to track further construction approved under the same permit,, and shall provide a final
certificate of completion once the entire proLt has been completed. Mail the Certification to the
Aquifer Protection Section, Division of Water Quality, 1636 Mail Service Center, Raleigh, NC
27699-1636.
3. The Wilmington Regional Office, telephone number (910) 796-7215 shall be notified at least 48
hours in advance (excluding weekends and holidays) of operation of the installed facilities so that an
in -place inspection can be made. Such notification to the Aquifer Protection Section regional
supervisor shall be made during the normal office hours from 8.00 a.m. until 5:00 p.m. on Monday
through Friday, excluding State Holidays.
4. No later than six months prior to the expiration of this permit, the Permittee shall request renewal of
this permit on official Division forms. Upon receipt of the request, the Division will review the
adequacy of the facilities described therein, and if warranted, will renew the permit for such period of
time and under such conditions and limitations as it may deem appropriate. Please note that Rule
15A NCAC 02T .0105(d) requires an updated site map to be submitted with the permit renewal
application.
11. PERFORMANCE STANDARDS
1. The reclaimed water utilization and high -rate infiltration facilities shall be effectively maintained and
operated at all times so that there is no discharge to the surface waters, nor any contamination of
ground waters, which will render them unsatisfactory for normal use. In the event that the facilities
fail to perform satisfactorily, including the creation of nuisance conditions due to improper operation
and maintenance or failure of the utilization area to adequately assimilate the wastewater, the
Permittee shall take immediate corrective actions including those actions that may be required by the
Division of Water Quality (Division), such as the construction of additional or replacement
wastewater treatment and disposal facilities.
2. The issuance of this permit shall not relieve the Permittee of the responsibility for damages to ground
or surface waters resulting from the operation of this facility.
All wells that are constructed for purposes of groundwater monitoring shall be constructed in
accordance with 15A NCAC 02C .0108 (Standards of Construction for Wells Other than Water
Supply) and any other state and local laws and regulations pertaining to well construction.
4, Effluent limitations shall not exceed those specified in Attachment A.
5. Application rate(s), whether hydraulic, nutrient, or other pollutant shall not exceed those specified in
Attachment B.
6. The COMPLIANCE and REVIEW BOUNDARIES for the reclaimed utilization areas are established
at the utilization area boundaries. Any exceedance of standards at the Compliance or Review
Boundary shall require action in accordance with 15A NCAC 02L .0106.

7. The compliance boundary for the high -rate disposal system is specified by rules in 15A NCAC 02L,
Groundwater Classifications and Standards. This disposal system was individually permitted on or
after December 30, 1983; therefore, the compliance boundary is established at either 250 feet from
the waste disposal area, or 50 feet within the property boundary, whichever is closest to the waste
disposal area. An exceedance of groundwater standards at or beyond the compliance boundary is
subject to remediation action according to 15A NCAC 02L .0106(d)(2) as well as enforcement
actions in accordance with North Carolina General Statute 143-215.6A through 143-215.6C.
8. The review boundary is established around the disposal systems midway between the compliance
boundary and the perimeter of the waste disposal area. Any exceedance of standards at the review
boundary shall require action in accordance with 15A NCAC 02L .0106
9. The Permittee shall apply for a permit modification prior to any sale or transfer of property that
affects a compliance boundary to establish a new compliance boundary.
10. In accordance with 15A NCAC 02L .0107(d), no wells, other than monitoring wells, shall be
constructed within the compliance boundary except as provided by 15A NCAC 02L .0107(g).
11. The facilities permitted herein must be constructed according to the following setbacks:

The storage/irrigation ponds located at Winding River Plantation do not comply with this
condition, but were previously approved by the Division. Therefore, this condition does not
apply to those previously constructed ponds
The following shall be requirements for the reclaimed water distribution, storage, and utilization
facilities:
a. All reclaimed water valves, storage facilities, and outlets shall be tagged or labeled to warn the
public or employees that the water is not intended for drinking. Where appropriate, such warning
shall inform the public or employees to avoid contact with the water.
b. All reclaimed water piping, valves, outlets, and other appurtenances shall be color -coded, taped,
or otherwise marked to identify the source of the water as being reclaimed water.
All reclaimed water piping and appurtenances shall be either colored purple (i.e., Pantone
522) and embossed or integrally stamped or marked "CAUTION: RECLAIMED WATER —
DO NOT DRINK" or be installed with a purple (i.e., Pantone 522) identification tape or
polyethylene vinyl wrap. The warning shall be stamped on opposite sides of the pipe and
repeated every three feet or less.
ii. Identification tape shall be at least three inches wide and have white or black lettering on
purple (i.e., Pantone 522) field stating "CAUTION: RECLAIMED WATER — DO NOT
DRINK." Identification tape shall be installed on top of redlaimed water pipelines, fastened
at least every 10 feet to each pipe length and run continuously the entire length of the pipe.
c. All reclaimed water valves and outlets shall be of a type, or secured in a manner, that permits
operation by authorized personnel only.
d. Above -ground hose bibs (i.e., spigots or other hand -operated connections) shall not be present.
Hose bibs shall be located in locked below -grade vaults that shall be clearly labeled as being of
non -potable quality. As an alternative to the use of locked below -grade vaults with standard hose
bibs services, hose bibs, which can only be operated by a special tool or connected to a special
hose connection, may be placed in non -lockable underground services boxes clearly labeled as
non -potable water.
13. The Perrnittee shall maintain an active cross -connection control program that shall have the following
minimum requirements:
a. No direct cross -connections shall be allowed between the reclaimed water and potable water
systems.
b. A reduced pressure principle backflow preventer, an approved air gap separation, or other device
approved by the Division of Environmental Health shall be installed at the potable water service
connection to the use area where both reclaimed water and potable water are supplied to a
reclaimed water use area. The installation of the reduced pressure principle backflow prevention
device shall allow proper testing_
c. An air gap separation, approved and regularly inspected by the Permittee shall be provided
between the potable water and reclaimed water systems where potable water is used to
supplement a reclaimed water system.
14. Reclaimed water distribution lines shall be located 10 feet horizontally from and 18 inches below any
water line where practicable. Where these separation distances cannot be met, the piping and integrity
testing procedures shall meet water main standards in accordance with 15A NCAC 18C.

15. Reclaimed water distribution lines shall not be less than 100 feet from a well unless the piping and
integrity testing procedures meet water main standards in accordance with 15A NCAC 18C, but no
case shall they be less than 25 feet from a private well or 50 feet from a public well.
16. Reclaimed water distribution lines shall meet the separation distances to sewer lines in accordance
with Rule .0305 of Subchapter 02T.
17. A usable green area shall be maintained for wastewater disposal. The green area shall have the
capability of accommodating the average daily flow of the facility being served without exceeding the
loading rates of the green area. A "green area", as defined in 15A NCAC 02H .0404 (g)(7), is an area
suitable for waste disposal, either in its natural state or which has been modified by planting a
vegetative cover of grasses or low growing shrubbery.
III. OPERATION AND MAINTENANCE REQUIREMENTS
I. The facilities shall be properly maintained and operated at all times. The facilities shall be effectively
maintained and operated as a non -discharge system to prevent the discharge of any wastewater
resulting from the operation of this facility. The Permittee shall maintain an Operation and
Maintenance Plan pursuant to 15A NCAC 02T .0913 including operational functions, maintenance
schedules, safety measures, and a spill response plan.
Upon classification of the wastewater treatment, reclaimed utilization, and high -rate infiltration
facilities by the Water Pollution Control System Operators Certification Commission (WPCSOCC),
the Permittee shall designate and employ a certified operator to be in responsible charge (ORC) and
one or more certified operator(s) to be back-up ORC(s) of the facilities in accordance with 15A
NCAC 0 8 G .0200. The ORC shall visit the facilities in accordance with 15A NCAC 0 8 G .0200 or as
specified in this permit and shall comply with all other conditions specified in these rules.
3. A suitable year round vegetative cover shall be maintained such that crop health is optimized, allows
for even distribution of effluent, and allows inspection of the reclaimed water utilization systems.
4. Adequate measures shall be taken to prevent ponding or runoff from the reclaimed water utilization
and high -rate infiltration areas.
5. Reclaimed water application shall not be performed during inclement weather or when the ground is
in a condition that will cause ponding or runoff.
6. All reclaimed water and high -rate application equipment must be tested and calibrated at least once
per permit cycle. Records of the calibration must be maintained for five years.
No type of wastewater other than that from Brunswick County West Regional. WW'11, shall be
applied to the reclaimed water utilization and high -rate infiltration area(s).
8. An automatically activated standby power source shall be on site and operational at all times capable
of powering all essential treatment units. If a generator is employed as an alternate power supply, it
shall be tested weekly by interrupting the primary power source.
9. Public access to the reclaimed water utilization sites and high -rate infiltration areas shall be controlled
during active site use. Such controls may include the posting of signs showing the activities being
conducted at each site.
10. The residuals generated from these treatment facilities must be disposed / utilized in accordance with
15A NCAC 02T .I100. The Permittee shall maintain a residual management plan pursuant to 15A
NCAC 02T .0914.
11. Diversion or bypassing of the untreated wastewater from the treatment facilities is prohibited.

12. Freeboard in the all irrigation and storage ponds shall not be less than two feet at any time.
13. A waste -level gauge, to monitor waste levels in all irrigation and storage ponds shall be provided.
This gauge shall have readily visible permanent markings indicating the maximum liquid level at the
top of the temporary liquid storage volume, minimum liquid level at the bottom of the temporary
liquid storage volume, and the lowest point on top of the dam elevations.
14. A protective vegetative cover shall be established and maintained on all earthen basin embankments
(outside toe of embankment to maximum allowable temporary storage elevation on the inside of the
embankment), berms, pipe runs, erosion control areas, and surface water diversions. Trees, shrubs,
and other woody vegetation shall not be allowed to grow on the earthen basin dikes or embankments,
Earthen basin embankment areas shall be kept mowed`or otherwise controlled and accessible.
15. All wastewater shall be routed to the 5-day bolding pond should the limit for fecal coliform (daily
maximum concentration of 25 per 100 ml) or turbidity (instantaneous maximum of 10 NTU) be
exceeded, until such time that the problems associated with the treatment capability of the wastewater
treatment plant have been corrected. The wastewater in the 5-day holding pond shall be pumped back
to the treatment plant for re -treatment or treated in the 5-day pond prior to discharge to the storage
ponds.
IV. MONITORING AND REPORTING REQUIREMENTS
1. Any monitoring (including groundwater, surface water, soil or plant tissue analyses) deemed
necessary by the Division to ensure surface and ground water protection will be established and an
acceptable sampling reporting schedule shall be followed.
2. All laboratory analyses for effluent, ground waters, or surface waters shall be made by a laboratory
certified by the Division for the required parameter(s) under 15A NCAC 02H .0800.
3. Flow through the treatment facility shall be continuously monitored and daily flow values shall be
reported on Form NDMR.
The Permittee shall install and maintain an appropriate flow measurement device consistent with
approved engineering and scientific practices to ensure the accuracy and reliability of flow
measurement. Flow measurement devices selected shall be capable of measuring flows with a
maximum deviation of less than 10 percent from true flow, accurately calibrated at a minimum of
once per year, and maintained to ensure that the accuracy of the measurements is consistent with the
accepted capability of that type of device. The Permittee shall keep records of flow measurement
device calibration on file for a period of at least three years. At a minimum, data to be included in
this documentation shall be:
a. Date of flow measurement device calibration,
b. Name of person performing calibration, and
c. Percent from true flow.
4. The effluent from the subject facilities shall be monitored by the Permittee at the frequency(ies) and
location(s) for the parameter(s) specified in Attachment A.
5. The Permittee tracking the amount of reclaimed water applied shall maintain adequate records. These
records shall include, but are not necessarily limited to, the following information:
a. Date of reclaimed water application,
b. Volume of reclaimed water applied,
c. Field irrigated,
d. Length of time field is irrigated,
e. Continuous weekly, monthly, and year-to-date hydraulic (inches/acre) loadings for each field,
f. Continuous monthly and year-to-date loadings for any non -hydraulic parameter specifically
limited in Attachment B for each field,

g. Weather conditions, and
h. Maintenance of cover crops.
6. Freeboard (i.e., waste level to the lowest elevation on the top of the embankment) in all irrigation,
infiltration, and storage ponds shall be recorded weekly.
7. The Permittee tracking the amount of wastewater disposed shall maintain adequate records. These
records shall include, but are not necessarily limited to, the following information:
a. Date of high -rate infiltration,
b. Volume of wastewater infiltrated,
c. Loading rates to each infiltration site list in Attachment B
d. Weather conditions,
8. Monitor wells shall be sampled at the frequencies and for the parameters specified in Attachment C.
All ma mg,well construction forms well abandonment forms and monitoring data shall refer to the
permit number and the well nomenclature as provided in Figure_1 and Attachment C.
Two copies of the results of the sampling and analysis must be received on Form GW-59
(Groundwater Quality Monitoring: Compliance Report Form), along with attached copies of
laboratory analyses, by the Division of Water Quality, Information Processing Unit, 1617 Mail
Service Center, Raleigh, North Carolina 27699-1617 on or before the last working day of the month
following the sampling month. Form GW-59 shall include the number of this permit and the
appropriate well identification number. One Form GW-59a certification form shall be provided for
each set of sampling results submitted.
10. Three copies of all monitoring data [as specified in Conditions IV.3. and IV.4.] on Form NDMR for
each point prior to irrigation (PPI), three copies of all operation and disposal records [as specified in
Conditions IV.5, and IV.6. on Form NDAR-1] for every utilization area and three copies of all
operation and disposal records [as specified in Conditions IV.7] for every infiltration area shall be
submitted on or before the last day of the following month. If no activities occurred during the
monitoring month, monitoring reports are still required documenting the absence of the activity. All
information shall be submitted to the following address:

11. A record shall be maintained of all residuals removed from this facility. This record shall include the
name of the hauler, permit authorizing the disposal or a letter from a municipality agreeing to accept
the residuals, date the residuals were hauled, and volume of residuals removed.
12. A maintenance log shall be maintained at this facility including but not limited to the following items:
a. Visual observations of the plant and plant site.
b. Record of preventative maintenance (changing of filters, adjusting belt tensions, alarm testing,
diffuser inspections and cleanings, etc.),
c. Date of calibration of flow measurement device.
d. Date and results of power interruption testing on alternate power supply.

13. Noncompliance Notification:
The Permittee shall report by telephone to the 'Wilmington Regional Office, telephone number (910)
796-7215, as soon as possible, but in no case more than 24 hours or on the next working day
following the occurrence or first knowledge of the occurrence of any of the following:
a. Any occurrence at the wastewater treatment facility which results in the treatment of significant
amounts of wastes which are abnormal in quantity or characteristic, such as the dumping of the
contents of a sludge digester; the known passage of a slug of hazardous substance through the
facility; or any other unusual circumstances including ponding in the reclaimed utilization area or
runoff from the reclaimed utilization areas.
b. Any process unit failure, due to known or unknown reasons, that render the facility incapable of
adequate wastewater treatment such as mechanical or electrical failures of pumps, aerators,
compressors, etc.
c. Any failure of disposal system resulting in a by-pass directly to receiving waters.
d. Any time that self -monitoring information indicates that the facility has gone out of compliance
with its permit limitations including, but not limited to, freeboard measurements, effluent
limitations, exceedances of groundwater standards, or overloading of any utilization area.
For any emergency that requires immediate reporting (e.g., discharges to surface waters, imminent
failure of a storage structure, etc.) outside normal business hours must be reported to the Division's
Emergency Response personnel at telephone number (800) 662-7956, (800) 858-0368, or (919) 733-
E 300. Persons reporting such occurrences by telephone shall also file a written report in letter form
within five (5) days following first knowledge of the occurrence. This report must outline the actions
taken or proposed to be taken to ensure that the problem does not recur.
1. Adequate inspection and maintenance shall be provided by the Permittee to ensure proper operation
of the subject facilities.
2. The Permittee or his designee shall inspect the wastewater treatment and disposal facilities to prevent
malfunctions and deterioration, operator errors and discharges which may cause or lead to the release
of wastes to the environment, a threat to human health, or a nuisance. The Permittee shall keep an
inspection log or summary including at least the date and time of inspection, observations made, and
any maintenance, repairs, or corrective actions taken by the Permittee. This log of inspections shall
be maintained by the Permittee for a period of five years from the date of the inspection and shall be
made available upon request to the Division or other permitting authority.
3. Any duly authorized officer, employee, or representative of the Division may, upon presentation of
credentials, enter and inspect any property, premises or place on or related to the disposal site or
facility at any reasonable time for the purpose of determining compliance with this permit; may
inspect or copy any records that must be maintained under the terms and conditions of this permit,
and may obtain samples of groundwater, surface water, or leachate.
VI. GENERAL CONDITIONS
1. Failure to abide by the conditions and limitations contained in this permit may subject the Permittee
to an enforcement action by the Division in accordance with North Carolina General Statute 143-
215.6A to 143-215.6C.

2. This permit shall become voidable unless the facilities are constructed in accordance with the
conditions of this permit, the approved plans and specifications, and other supporting data.
3. This permit is effective only with respect to the nature and volume of wastes described in the
application and other supporting data. No variances to applicable rules governing the construction
and / or operation of the permitted facilities are granted unless specifically requested and granted in
this permit.
4. The issuance of this permit does not exempt the Permittee from complying with any and all statutes,
rules, regulations, or ordinances, which may be imposed by other government agencies (local, state,
and federal) that have jurisdiction. Of particular concern to the Division are applicable river buffer
rules in 15A NCAC 02B .0200, erosion and sedimentation control requirements in 15A NCAC
Chapter 4 and under the Division's General Permit NCG010000, and any requirements pertaining to
wetlands under 15A NCAC 02B .0200 and 02H .0500.
In the event there is a desire for the facilities to change ownership, or there is a name change of the
Permittee, a formal permit request must be submitted to the Division on official Division forms,
documentation from the parties involved, and other supporting materials as may be appropriate. The
approval of this request will be considered on its merits and may or may not be approved. The
Permittee of record shall remain fully responsible for compliance until a permit is issued to the new
owner.
6. The Permittee shall retain a set of approved plans and specifications for the life of the facilities
permitted herein.
7. The Permittee shall maintain this permit until all permitted facilities herein are properly closed or
permitted under another permit issued by the appropriate permitting authority.
8. The Permittee must pay the annual fee within 30 days after being billed by the Division. Failure to
pay the fee accordingly may cause the Division to initiate action to revoke this permit pursuant to
15A NCAC 02T .0105(e).

2. The measurement of water levels shall be made prior to purging the wells. The depth to water in each well shall be measured from the surveyed point on the top of the casing. The measurement
of pH shall be made after purging and prior to sampling for the remaining parameters.
3. The measuring points (top of well casing) of all monitoring wells shall be surveyed to provide the relative elevation of the measuring point for each monitoring well. The measuring points (top
of casing) of all monitoring wells shall be surveyed relative to a common datum.
4. Volatile Organic Compounds (VOC) - In November only, analyze by one of the following methods:
a. Standard Method 6230D, PQL at 0.5 µg/L or less
b. Standard Method 6210D, PQL at 0.5 µg/L or less
C. EPA Method 8021, Low Concentration, PQL at 0.5 µg/L or less
d. EPA Method 8260, Low Concentration, PQL at 0.5 µg/I, or less
e_ Another method with prior approval by the Aquifer Protection Section Chief
Any method used must meet the following qualifications:
a. A laboratory must be DWQ certified to run any method used.
b. The method used must, at a minimum, include all the constituents listed in Table VIII of Standard Method 6230D.
c. The method used must provide a PQL of 0.5 µglL or less that must be supported by laboratory proficiency studies as required by the DWQ Laboratory Certification Unit. Any constituents
detected above the MDL but below the PQL of 0.5 µg/L must be qualified (estimated) and reported.
5. If any volatile organic compounds (VOC) are detected as a result of monitoring as provided in Attachment C, then the Wilmington Regional Office Aquifer Protection Supervisor, telephone
number (910) 796-7215 must be contacted immediately for further instructions regarding any additional follow-up analyses required_
6. If TOC concentrations greater than 10 mg/1 are detected in any downgradient monitoring well, additional sampling and analysis must be conducted to identify the individual constituents
comprising this TOC concentration. If the TOC concentration as measured in the background monitor well exceeds 10 mg/l, this concentration will be taken to represent the naturally occurring
TOC concentration. Any exceedances of this naturally occurring TOC concentration in the downgradient wells shall be subject to the additional sampling and analysis as described above.
7. Monitoring wells shall be reported consistent with the nomenclature and location information provided in Figure I and this attachment.
