Dear Mr. Lawing:
In accordance with your modification request received September 14, 2004 to add the last nine
holes at Winding River Plantation, and subsequent additional information received May 13, 2005 and
August 22, 2005, we are forwarding herewith Permit Number. WQ0023693, dated August 23, 2005, to
Brunswick County for the construction and operation of the subject wastewater treatment and reclaimed
water utilization system_
This permit shall be effective from the date of issuance until April 15, 2009, shall void Permit No.
WQ0023693 issued April 1, 2005, shall rescind permit WQ0012151 for St. James Plantation and permit
WQ0013785 for Winding River Plantation, and shall be subject to the conditions and limitations as
specified therein. Please pay particular attention to the monitoring requirements in this permit.
Monitoring requirements apply and must be submitted for all three wastewater treatment facilities until
documentation has been received by the Division that the treatment facilities located at St. James and
Winding River Plantations have been properly abandoned. Failure to establish an adequate system for
collecting and maintaining the required operational information will result in future compliance problems.
Please note there are several new/modified conditions not included in your permit issued April 16,
❖ Condition I.4 — requires that you provide the Division with an accurate system description upon
completion of all construction and demolition activities, and submit a minor permit
modification at that time.
❖ Condition 1.5 — requires that the wastewater treatment facilities at St. James and Winding River
Plantations be properly abandoned within six months of the start of operations at the West
Brunswick Regional Water Reclamation Facility.
❖ Condition II.8 — requires that freeboard in all irrigation/storage ponds not be less than 2 feet at
any time.
❖ Condition 11.9 —requires the installation of waste -level gauges, if not already installed.

right to request an adjudicatory hearing upon written request within thirty (30) days following receipt of

continued operation of the existing 600,000 GPD wastewater treatment and reclaimed water distribution
and disposal system located at St. James Plantation and the existing 500,000 GPD wastewater treatment
and reclaimed water distribution and disposal system located at Winding River Plantation, which consist
of the following:
The St. James Plantation Wastewater Treatment Facility, consisting of:
a 150,000 gallon flow equalization basin with dual 450 GPM pumps, a 600 CFM blower and high water
alarms; manually cleaned bar screen; a flow splitter box; dual 410,000 gallon coarse bubble aeration
tanks; dual 77,900 gallon clarifiers; a 150,000 gallon aerated sludge holding tank; five 1,000 CFM
blowers; a traveling bridge filter with dual 340 square foot filters; gas or hypo -chlorinator with an 18,800
gallon contact chamber; UV disinfection; continuous on-line monitoring and recording for turbidity;
continuous on-line monitoring and recording for flow; a 17,400 gallon effluent dosing tank with three 525
GPM pumps and high water alarms; and a 350 kW stand-by generator; and
The St. James Plantation Distribution and Disposal Facilities, consisting of:
a 3,000,000 gallon clay lined 5-day holding pond; 200 GPM pump station with dual pumps, high water
alarms; approximately 600 linear feet of 4-inch force main and approximately 4,100 linear feet of 10-inch
force main from the WWTP effluent pumping chambers to the storage pond; a 3,000,000 gallon
storage/irrigation lagoon; a 1,050 GPM irrigation pump station with a 30 HP pump, a 75 HP pump and
high water alarms; and approximately 134 acres of spray irrigation area; and
The Winding River Plantation Wastewater Treatment Facility, consisting of:
the Phase I wastewater treatment system (100,000 GPD) that consists of a 25,000 gallon flow equalization
chamber with duplex 100 gallon per minute (GPM) submersible pumps, one (1) 80 cubic feet per minute
(CFM) blower, and high water alarms, a manually cleaned bar screen, flow splitter box, two (2) 62,500
gallon fine bubble aeration basins, two (2) 10,418 gallon clarifiers, one (1) 20,000 gallon aerated sludge
holding tank, three (3) 290 CFM blowers, two (2) 34.5 square foot (ft2) tertiary filters (a total of 69.0 ft2)
with duplex 518 GPM backwash pumps, duplex 40 GPM wastewater return pumps, one (1) 55 CFM air
scour blower, and high water alarms, ultraviolet disinfection, one (1) 1,885 gallon effluent dosing
chamber with duplex 190 GPM submersible pumps, approximately 5,700 linear feet of 8-inch force main,
and approximately 170 linear feet of 3-inch force main (to eventually pump to Phase II), a continuous
monitoring and recording flow meter, and a turbidity meter; and
the Phase II and III wastewater treatment systems (both 200,000 GPD) that consist of a 50,000 gallon
flow equalization chamber with duplex 200 GPM submersible pumps, one (1) 160 CFM blower, and high
water alarms, a manually cleaned bar screen (in Phase II only however it will also be utilized in Phase
III), flow splitter box, two (2) 127,700 gallon fine bubble aeration basins, two (2) 27,000 gallon clarifiers,
one (1) 50,000 gallon aerated sludge holding tank, three (3) 540 CFM blowers, dual, two -filter tertiary
filters (each cell having a filter area of 34.75 square feet for a total of 139 square feet) with duplex 1,036
GPM backwash pumps, duplex 80 GPM wastewater return pumps, one (1) 110 CFM air scour blower,
and high water alarms, ultraviolet disinfection, one (1) 4,950 gallon effluent dosing chamber with duplex
870 GPM submersible pumps (for Phase U) and duplex 350 GPM submersible pumps (for Phase III
which pump effluent to Phase II), a continuous monitoring and recording flow meter, and a turbidity
meter; and
The Winding River Plantation Distribution and Disposal Facilities, consisting of:
a 500,000 gallon lined upset pond (for Phase 1), a 2,000,000 gallon lined upset pond (for Phase 11),
13,000,000 gallon storage and irrigation pond, and approximately 150 acres of irrigation area;
construction and operation of the 3.0 MGD West Brunswick Regional Water Reclamation Facility, the
reclaimed water distribution system, and the West Brunswick Drip Irrigation Facility, which consist of
the following:
The 3.0 MGD West Brunswick Regional Water Reclamation Facility, consisting of:
approximately 7,000 linear feet of 24-inch sanitary sewer force main, approximately 6,000 linear
feet of 24-inch reclaimed water force main, and approximately 2,000 linear feet of 8-inch water
force main, two air release valves and associated appurtenances,
• two influent magnetic flowmeters,
plant headworks facility, to include one manual and two mechanical bar screens, vortex grit
removal with associated dual 220 GPM grit pumps, screening washer and grit classifier,
• two unequal -sized equalization basins totaling 750,000 gallons, with dual 1,400 GPM, 10 Hp
pumps, jet aeration using dual 1,620 scfm and dual 540 scfm air blowers (Blower Building No. 1)
in addition to one (1) 2,900 GPM, 25 Hp pump and one (1) 1,100 GPM, 10 Hp pump,
• oxidation ditch flow splitter box,
• dual oxidation ditches, with each ditch specified as: 76 feet wide, 10-foot side water depth,
950,000 gallon volume, and each ditch includes three (3) 4.9 Hp submersible mixers, three (3) 75
Hp 30-foot brush aerators, and one (1) adjustable effluent weir,
• two (2) 70-foot diameter, 16-foot side water depth center -fed secondary clarifiers with dual 20
GPM, 7.5 Hp scum pumps,
+ dual rotating disk package filters, with a combined rating of 3.0 MGD design flow and 7.5 MGD
peak flow, with each filter system consisting of a stainless steel tank with cloth rotating disk
filters rated at 3 gpm/fO with automatic backwash, miscellaneous electrical and piping work
associated with the rotating filters,
• a 78,000 gallon chlorine contact tank with sodium hypochlorite disinfection,
• a sodium hypochlorite chemical feed and storage building,
• effluent reclaimed water pump station consisting of two (2) 2,800 GPM, 125 Hp vertical turbine
pumps and two (2) 1,400 GPM, 100 Hp vertical turbine pumps with magnetic flowmeters,
• a 23 million gallon (approximate) upset pond lined with 40 mil polypropylene synthetic liner, and
a dual 2,100 GPM, 40 Hp upset pond pump station,
0 RAS/WAS pump station consisting of three (3) 1,400 GPM, 15 Hp centrifugal RAS pumps and
two (2) 100 GPM, 7.5 Hp centrifugal WAS pumps,
• a 238,000 gallon WAS storage tank with jet aeration using dual 320 scfm air blowers (Blower
Building No. 2) and one (1) 1,465 GPM, 15 Hp pump,
0 dual 250 GPM WAS thickener feed pumps,
• a WAS thickening building, consisting of a 1-meter gravity belt thickener with approximately
2,000 lbs/hour capacity, polymer tank and feed system, with dual 250 GPM, 15 Hp positive
displacement thickened WAS pumps,
• an autothermal thermophilic aerobic digestion (ATAD) facility for producing Class A biosolids
consisting of a dual 20-foot diameter, 21-foot covered concrete ATAD tanks, a 70-foot diameter,
17-foot covered concrete storage tank, a 2,495 sq. foot ATAD Building housing dual 250 GPM,
15 Hp ATAD feed pumps, dual 40 Hp jet aeration pumps, one 50 Hp jet aeration pump, dual 5
Hp sludge transfer pumps, 250,000 BTU/hr heat -exchanger, five (5) 15 Hp blowers, ammonia
scrubber and biofilter, and associated piping, appurtenances, and control systems,
• a sludge loading station (for trucks) with 250 GPM sludge transfer pump,
• plant sanitary sewer drain pump station consisting of two (2) 2,100 GPM, 50 Hp submersible
pumps,
• two odor control systems, one rated at 2,000 cfm and one rated at 8,400 cfm,
• three (3) non -potable water pumps, one rated at 200 GPM, 15 Hp pump and two (2) rated at 75
GPM, 7.5 Hp pumps,
• a 3,456 sq. foot administration building,
• a 2,240 sq. foot maintenance building,
• associated electrical work, including two (2) 1,600 kW emergency generators with automatic
transfer switches,
• SCADA instrumentation and control systems,
• associated yard piping and appurtenances, and
The reclaimed water distribution system, consisting of:
N.C. 211 to Reclaimed Valve Vault (Section 5):
approximately 15,269 linear feet of 16-inch reclaimed water force main, approximately 3,471 linear feet
of 18-inch reclaimed water force main, approximately 1,020 linear feet of 20-inch reclaimed water force
main, associated gate valves, air release valve assemblies, bypass assemblies and associated
appurtenances,
Bolivia to Reclaimed Valve Vault (Sewer Force Main), and Valve Vault to Effluent Disposal Site
(Reclaimed Water Force Main), (Section 6)
approximately 804 linear feet of 10-inch reclaimed water force main, approximately 15,650 linear feet of
16-inch reclaimed water force main, associated gate valves, air release valve assemblies, bypass
assemblies and associated appurtenances, and
N.C. 211 to Winding River Plantation:
approximately 5,774 linear feet of 10-inch reclaimed water force main, three air release valve assemblies,
and associated appurtenances, and
N.C. 211 to St. James Plantation:
approximately 11,813 linear feet of 12-inch reclaimed water force main, approximately 13 linear feet of
16-inch reclaimed water force main, four air release valve assemblies, and associated appurtenances, and
The West Brunswick Drip Irrigation Facility (rated at 1.9 MGD annual averaged wastewater disposal),
consisting of:
• approximately 2,665 linear feet of 16-inch reclaimed water force main, one air release valve and
associated appurtenances,
• a 98.25 million gallon (approximate) wet weather storage pond lined with 36 tnil polypropylene
geomembrane protective liner in inside berm walls,
• Pump Station effluent reclaimed water pump station consisting of a manual bar screen, two (2)
3,300 GPM, 150 Hp vertical turbine pumps and one (1) 1,600 GPM, 75 Hp vertical turbine pump
with two (2) magnetic flowmeters, disk filtration system, a sodium hypochlorite chemical storage
and feed system,
a drip irrigation system covering 556..03 acres grouped into the Southern Management Area
(Zones I — 13) and the Northern Management Area (Zones 14 — 30), with drip tubing spaced
approximately on 10-foot centers, associated header and lateral piping and associated
appurtenances, associated forest clearing and planting of Loblolly Pine seedlings, and access
roads,
SCADA instrumentation and control systems, and
associated yard piping and appurtenances,
to serve Brunswick County with no discharge of wastes to the surface waters, pursuant to the application
received November 9, 2004 and subsequent additional information received by the Division, and in
conformity with the project plan, specifications, and other supporting data subsequently filed and
approved by the Department of Environment and Natural Resources and considered a part of this permit.
This permit shall be effective from the date of issuance until April 15, 2009, shall void Permit No.
WQ0023693 issued April 16, 2004, and shall be subject to the following specified conditions and
limitations:
PERFORMANCE STANDARDS
I. Upon completion of construction and prior to operation of this permitted facility, a certification
must be received from a professional engineer certifying that the permitted facility has been
installed in accordance with this permit, the approved plans and specifications, and other
supporting materials. If this project is to be completed in phases and partially certifiedyou shall
retain the responsibilityresponsibili1y to track further construction approved under the same permit, and shall
rovide a final certificate of com lesion once the entire project has been completed. Mail the
Certification to the Land Application Unit, 1636 Mail Service Center, Raleigh, NC 27699-1636.
2. The Wilmington Regional Office, telephone number (910) 395-3900, shall be notified at least
forty-eight (48) hours in advance of operation of the installed facilities so that an in -place
inspection can be made. Such notification to the regional supervisor shall be made during the
normal office hours from 8:00 a.m. until 5:00 p.m. on Monday through Friday, excluding State
Holidays.
Upon the completion of construction of the West Brunswick Regional Water Reclamation
Facility and the abandonment of the treatment facilities located at St. James Plantation and
Winding River Plantation, the Pernuttee shall provide the Division with a list detailing which
facility components have been constructed and which have been abandoned. At this time, the
Permittee shall also apply for a .minor permit modification so that the system description can be
corrected accordingly.
4. As specified in a contract between Brunswick County and St. James and Winding River
Plantations, within six (6) months of the time that the West Brunswick Regional Water
Reclamation Facility begins treating wastewater the wastewater treatment facilities Iocated at St.
James Plantation and Winding River Plantation will be properly abandoned. Documentation of
this shall be submitted to the Division.
5. The reclaimed water utilization facilities shall be effectively maintained and operated at all times
so that there is no discharge to the surface waters, nor any contamination of ground waters, which
will render them unsatisfactory for normal use. In the event that the facilities fail to perform
satisfactorily, including the creation of nuisance conditions or failure of the irrigation area to
adequately assimilate the wastewater, the Perrnittee shall take immediate corrective actions
including those actions that may be required by the Division of Water Quality (Division), such as
the construction of additional or replacement wastewater treatment and disposal facilities.
6. The issuance of this permit shall not relieve the Pennittee of the responsibility for damages to
surface water or ground water resulting from the operation of this facility.

7. The residuals generated from these treatment facilities must be disposed in accordance with
General Statute 143-215.1 and in a manner approved by the Division.
8. Diversion or bypassing of the untreated wastewater from the treatment facilities is prohibited.
9. The following buffers shall be maintained:
a. 100 feet between wetted areas and water supply wells,
b. 100 feet between wetted areas and waters classified as SA,
c. 25 feet between wetted areas and surface waters not classified as SA,
d. 100 feet between wastewater treatment units and wells,
e. 50 feet between reclaimed water storage/irrigation ponds and property lines', and
f. 50 feet between wastewater treatment units and property lines.
Sorne of the buffers specified above may not have been included in previous permits for this
waste treatment and disposal system. These buffers are not intended to prohibit or prevent
modifications, which are required by the Division, to improve performance of the existing
treatment facility. These buffers do, however, apply to modifications of the treatment and
disposal facilities that are for the purpose of increasing the flow that is tributary to the facility.
These buffers do apply to any expansion or modification of the irrigation areas and apply in
instances in which the sale of property would cause any of the buffers now complied with, for the
treatment and disposal facilities, to be violated. The applicant is advised that any modifications
to the existing facilities will require a permit modification.
The storage/irrigation ponds located at Winding River Plantation do not comply with this
condition, but were previously approved by the Division. Therefore, this condition does not
apply to those previously constructed ponds.
10. Public access to the irrigation sites shall be controlled during active site use. Such controls may
include the posting of signs showing the activities being conducted at each site. A sign shall be
posted in plain sight in the clubhouse showing these activities.
11. The following shall be requirements for the reclaimed water distribution, storage, and utilization
facilities:
a. All reclaimed water valves, storage facilities, and outlets shall be tagged or labeled to warn
the public or employees that the water is not intended for drinking. Where appropriate, such
warning shall inform the public or employees to avoid contact with the water.
b. All reclaimed water piping, valves, outlets, and other appurtenances shall be color -coded,
taped, or otherwise marked to identify the source of the water as being reclaimed water.
i. All reclaimed water piping and appurtenances shall be either colored purple (i.e., Pantone
522) and embossed or integrally stamped or marked "CAUTION: RECLAIMED
WATER — DO NOT DRINK" or be installed. with a purple (i.e., Pantone 522)
identification tape or polyethylene vinyl wrap. The warning shall be stamped on opposite
sides of the pipe and repeated every three feet or less.
ii. Identification tape shall be at least three inches wide and have white or black lettering on
purple (i.e., Pantone 522) field stating "CAUTION: RECLARVIED WATER -- DO NOT
DRINK." Identification tape shall be installed on top of reclaimed water pipelines,
fastened at least every 10 feet to each pipe length and run continuously the entire length
of the pipe.
iii. Existing underground distribution systems retrofitted for the purpose of distributing
reclaimed water shall be taped or otherwise identified as in items a or b above. This
identification need not extend the entire length of the distribution system but shall be
incorporated within 10 feet of crossing any potable water supply line or sanitary sewer
line.
c. All reclaimed water valves and outlets shall be of a type, or secured in a manner, that permits
operation by authorized personnel only.
d. Above -ground hose bibs' (i.e., spigots or other hand -operated connections) shall not be
present. Hose bibs shall be located in locked below -grade vaults that shall be clearly labeled
as being of non -potable quality. As an alternative to the use of locked below -grade vaults
with standard hose bibs services, hose bibs, which can only be operated by a special tool or
connected to a special hose connection, may be placed in non -lockable underground services
boxes clearly labeled as non -potable water.
12. The Permittee shall maintain an active cross -connection control program that shall have the
following minimum requirements:
a. No direct cross -connections shall be allowed between the reclaimed water and potable water
systems.
b. A reduced pressure principle backflow preventer, an approved air gap separation, or other
device approved by the Division of Environmental Health shall be installed at the potable
water service connection to the use area where both reclaimed water and potable water are
supplied to a reclaimed water use area. The installation of the reduced pressure principle
backflow prevention device shall allow proper testing.
c. An air gap separation, approved and regularly inspected by the potable water supplier, shall
be provided between the potable water and reclaimed water systems where potable water is
used to supplement a reclaimed water system.
II. OPERATION AND MAINTENANCE REQUIREMENTS
1. The facilities shall be properly maintained and operated at all times
2. Upon classification of the wastewater treatment and irrigation facilities by the Water Pollution
Control System Operators Certification Commission (WPCSOCC), the Permittee shall designate
and employ a certified operator to be in responsible charge (ORC) and one or more certified
operator(s) to be back-up ORC(s) of the facilities in accordance with 15A NCAC 8G ,0201. The
ORC shall visit the facilities in accordance with 15A NCAC 8G .0204 or as specified in this
permit and shall comply with all other conditions specified in these rules.
3. A suitable, year-round vegetative cover shall be maintained on the irrigation areas.
4. Irrigation shall not be performed during inclement weather or when the ground is in a condition
that will cause runoff.
5. Adequate measures shall be taken to prevent wastewater runoff from the irrigation fields
G. The facilities shall be effectively maintained and operated as a non -discharge system to prevent
the discharge of any wastewater resulting from the operation of these facilities.
7. a. Until the demolition of the wastewater treatment facilities located at St. James Plantation and
Winding River Plantation, no type of wastewater other than that from St. James Plantation
shall be irrigated onto the irrigation areas located at St. James PIantation, no type of
wastewater other than that from Winding River Plantation shall be irrigated onto the
irrigation areas located at Winding River Plantation, and no type of wastewater other than
that from the West Brunswick Regional Water Reclamation Facility shall be irrigated onto
the West Brunswick Drip Irrigation Facility.

b. After the demolition of the wastewater treatment facilities located at St. James Plantation and
Winding River Plantation, no type of wastewater other than that from the West Brunswick
Regional Water Reclamation Facility shall be irrigated onto any of the irrigation areas.
8. Freeboard in all irrigation and storage ponds shall not be less than two feet at any time.
9. Waste -level gauges, to monitor waste levels in the irrigation/storage ponds, shall be installed
within 60 days of issuance of this permit. These gauges shall have readily visible permanent
markings indicating the maximum liquid level at the top of the temporary liquid storage volume,
minimum liquid level at the bottom of the temporary liquid storage volume, and top of the dam
elevations. Caution must be taken not to damage the integrity of the liners when installing the
gauges.
10. A protective vegetative cover shall be established and maintained on all earthen basin
embankments (outside toe of embankment to maximum pumping elevation), berms, pipe runs,
erosion control areas, and surface water diversions. Trees, shrubs, and other woody vegetation
shall not be allowed to grow on the earthen basin dikes or embankments. Earthen basin
embankment areas shall be kept mowed or otherwise controlled and accessible.
11. a. Until the demolition of the wastewater treatment facilities located at St. James Plantation and
Winding River Plantation, all wastewater from those facilities shall be routed to the five-day
holding pond should the limit for fecal coliform (daily maximum concentration of 25 per 100
ml) or turbidity (instantaneous maximum of 10 NTU) be exceeded, until such time that the
problems associated with the treatment capability of the wastewater treatment plant have
been corrected_ The wastewater in the five-day holding pond shall be pumped back to the
treatment plant to be retreated or treated in the five-day pond prior to discharge to the storage
pond.
b. After the demolition of the wastewater treatment facilities located at St. James Plantation and
Winding River Plantation, should the limit for fecal coliform (daily maximum concentration
of 25 per 100 ml) or turbidity (instantaneous maximum of 10 NTU) be exceeded, all
wastewater shall be handled in one of two ways, until the levels have stabilized below these
maximum levels. The West Brunswick Regional Water Reclamation Facility may further
treat the wastewater to bring it into compliance with reclaimed water standards without
returning the water to the facility's headworks, or may reroute it to the headworks of the
wastewater treatment facility for treatment to reuse standards.

12. For the West Brunswick Reclaimed Water Drip Irrigation Facility, the wastewater applied, to the

III. MONITORING AND REPORTING REQUIREMENTS
Any monitoring (including groundwater, surface water, soil or plant tissue analyses) deemed
necessary by the Division to insure surface and ground water protection will be established and
an acceptable sampling reporting schedule shall be followed.
2. Influent flow shall be continuously monitored and daily flow values shall be reported on Form
NDMR for each treatment facility.

The Permittee shall install an appropriate flow measurement device consistent with approved
engineering and scientific practices to ensure the accuracy and reliability of influent flow
measurement. Flow measurement devices selected shall be capable of measuring flows with a
maximum deviation of less than 10 percent from true flow, accurately calibrated at a minimum of
once per year, and maintained to ensure that the accuracy of the measurements is consistent with
the accepted capability of that type of device. The Permittee shall keep records of flow
measurement device calibration on file fora period of at least three years. At a minimum, data to
be included in this documentation shall be:
a. Date of flow measurement device calibration
b. Name of person performing calibration
c. Percent from true flow
Plans and specifications for the flow measuring device shall be prepared by a licensed
Professional Engineer and submitted to the appropriate regional office 30 days prior to
installation. The flow -measuring device shall be installed within 1 year of the date of this permit.
3_ As an indicator of proper operation and maintenance, the West Brunswick Regional Water

G. The Permittee shall maintain adequate records tracking the amount of wastewater disposed for
each facility. These records shall include, but are not necessarily limited to the following
information:
a. Date and time of irrigation,
b. Volume of wastewater irrigated,
c. Zone irrigated,
d. Length of time zone is irrigated,
e. Continuous weekly, monthly, and year-to-date hydraulic (inches/acre) loadings for each zone,
f. Weather conditions, and
g. Maintenance of cover crops.
7. Freeboard in all irrigation/storage ponds shall be recorded weekly.

8. Three (3) copies of all monitoring data [as specified in Conditions 111(2) and II1(3)] on Form
NDMR-1 and three (3) copies of all operation and disposal records [as specified in Conditions
11I(4) and 111(5)] on Form NDAR-1 shall be submitted monthly on or before the last day of the
following month_ All information shall be submitted to the following address:

9. A record shall be maintained of all residuals removed from these facilities. This record shall
include the name of the hauler, permit authorizing the disposal or a letter from a municipality
agreeing to accept the residuals, date the residuals were hauled, and volume of residuals removed.
10. Maintenance logs shall be maintained at the West Brunswick Regional Water Reclamation
Facility, the St. James Plantation Wastewater Treatment Facility, and the Winding River
Plantation Wastewater Treatment Facility including but not limited to the following items:
a. Daily sampling results including residual chlorine, settleable matter, and dissolved oxygen in the
aeration basin and at the clarifier weir.
b. Visual observations of the plant and plant site.
c. Record of preventative maintenance (changing of filters, adjusting belt tensions, alarm testing,
diffuser inspections and cleanings, etc.).
d. Date of calibration of flow measurement device.
e. Date and results of power interruption testing on alternate power supply.
I L A maintenance log shall be maintained for the irrigation facilities including but not limited to the
following items:
a. Visual observations of the irrigation storage ponds and spray irrigation sites.
b. Record of preventative maintenance (irrigation pump maintenance, irrigation storage pond
maintenance, changing sprinkler nozzles, etc.).
c. Date of calibration of flow measurement device.
12. Noncompliance Notification:
The Permittee shall report by telephone to the Wilmington Regional Office, telephone number
(910) 395-3900, as soon as possible, but in no case more than 24 hours or on the next working
day following the occurrence or first knowledge of the occurrence of any of the following:
a. Any occurrence at the wastewater treatment facility which results in the treatment of
significant amounts of wastes which are abnormal in quantity or characteristic, such as the
dumping of the contents of a sludge digester; the known passage of a slug of hazardous
substance through the facility; or any other unusual circumstances.
b. Any process unit failure, due to known or unknown reasons, that render the facility incapable
of adequate wastewater treatment such as mechanical or electrical failures of pumps, aerators,
compressors, etc.
Any failure of a pumping station, sewer line, or treatment facility resulting in a by-pass
directly to receiving waters without treatment of all or any portion of the influent to such
station or facility.

d. Any time that self -monitoring information indicates that the facility has gone out of
compliance with its permit limitations.
Occurrences outside normal business hours may also be reported to the Division's
Emergency Response personnel at telephone number (800) 662-7956, (800) 858-0369, or
(919) 733-3300. Persons reporting such occurrences by telephone shall also file a written
report in letter form within five (5) days following first knowledge of the occurrence. This
report must outline the actions taken or proposed to be taken to ensure that the problem does
not recur.
IV. GROUNDWATER REQUIREMENTS
For West Brunswick Reclaimed Water Drip Irrigation Facility
1. Well Construction / Abandonment Criteria;
a. Prior to beginning waste disposal operations monitor well(s), MW-1 through MW-6 shall be
installed to monitor groundwater quality. The well(s) shall be constructed such that the water
level in the well is never above or below the screened (open) portion of the well at any time
during the year. The general location and name for each well is marked on the attached sheet.
Each monitoring well shall be located at the Review boundary, constructed in accordance
with this permit, and approved by the Wilmington Regional Office.
b. The well(s) must be constructed by either a North Carolina Certified Well Contractor, the
property owner, or the property lessee according to General Statutes 87-98.4(b)(2). If the
construction is not performed by a certified well contractor, the property owner or lessee,
provided that they are a natural person, must physically perform the actual well construction
activities. All well(s) trust be constructed according to the North Carolina Well Construction
Standards (15A NCAC 2C .0108) and any local county rules.
c. Prior to beginning waste disposal operations at the West Brunswick Drip Irrigation Facility,
all shallow piezometers installed for the purposed of soil/hydrogeologic investigation shall be
permanently abandoned_ Within thirty (30) days of abandonment, a Well Abandonment
Record (GW-30 form) shall be completed for each well abandoned and mailed to the address
listed in the "Reporting / Documentation" section of the Groundwater Requirements. The
well(s) must be abandoned by a North Carolina Certified Well Contractor according to the
North Carolina Well Construction Standards (15A NCAC 2C .0113) and local county rules.
d- The Wilmington Regional Office, telephone number (910) 395--3900 shall be notified at least
forty-eight (48) hours prior to the construction of any monitoring well so that an inspection
can be made of the monitoring well location. Such notification to the regional groundwater
supervisor shall be made during the normal office hours from 8:00 a.m. until 5:00 p.m. on
Monday through Friday, excluding state holidays.
e. Within thirty (30) days of completion of all well construction activities, a certification must
be received from a professional engineer or a licensed geologist certifying that the monitoring
well(s) are located according to this permit.

2. Sampling Criteria:
a. Monitor well(s) MW-1, MW-2, MW-3, MW-4, MW-5, and MW-6 shall be sampled initially
after construction and prior to waste disposal operations, and thereafter every March, July and
November for the parameters listed below. Prior to sampling the parameters, the
measurement of water levels must be taken. The depth to water in each well shall be
measured from the surveyed point on the top of the casing. The measuring points (top of well
casing) of all monitoring wells shall be surveyed relative to a common datum.

b. For Total Organic Carbon (TOC), if concentrations greater than 10 mg/1 are detected in any
down -gradient monitoring well, additional sampling and analysis must be conducted to
identify the individual constituents comprising this TOC concentration. if the TOC
concentration as measured in the background monitor well exceeds 10 mg/1, this
concentration will be taken to represent the naturally occurring TOC concentration. Any
exceedances of this naturally occurring TOC concentration in the down -gradient wells shall
be subject to the additional sampling and analysis as described above.
c. For Volatile Organic Compounds (VOCs) sampled in November, use only one of the
following methods:
1) Standard Method 6230D, Practical Quantitation Limit (PQL) at 0.5 ug/L or less
2) Standard Method 6210, PQL at 0.5 ug/L or less
3) EPA Method 8021, Low Concentration, PQL at 0.5 ug/L or less
4) EPA Method 9260, Low Concentration, PQL at 0.5 ug/L or less
5) Another method with prior approval by the Groundwater Section Chief.
Any of the referenced methods used for VOCs must at a minimum., include all the
constituents listed in Table VIIl of Standard Method 6230D. Any method used must provide
a PQL of 0.5 ug/L or less, which must be supported by laboratory proficiency studies as
required by the DWQ Laboratory Certification Unit. Any constituents detected above the
Method Detection Limit (MDL) but below the PQL of 0.5 ug/L must be qualified (estimated)
and reported.
d. Any laboratory selected to analyze parameters must be Division of Water Quality (DWQ)
certified for those parameters required.

3. Reporting / Documentation:
a. All reports and documentation (GW-1, GW-30, GW-59) shall be mailed to the following
address:

Updated blank forms (GW-1, GW-30, GW-59) may be downloaded from the Groundwater
Section's website at htt :// w.ehnr.state.nc.us/ or requested from the address mentioned
above.
b. For the initial sampling of the well(s), the permittee shall submit a copy of the GW-1 Form
(Well Construction Record) with the Compliance Monitoring Form (GW-59) for each well to
the address listed in the "Reporting / Documentation" section of the Groundwater
Requirements. Initial Compliance Monitoring Forms that do not include copies of the GW-1
form may be returned to the permittee without being processed. Failure to submit these
forms as required by this permit may result in the initiation of enforcement activities pursuant
to NC General Statutes 143-215.6A.
The results of the sampling and analysis must be received on Form GW-59 (Groundwater
Quality Monitoring: Compliance Report Form) by the Groundwater Section (address listed in
the "Reporting / Documentation" section of the Groundwater Requirements), on or before the
last working day of the month following the sampling month. The data of all groundwater
sampling analyses required by the permit conditions must be reported using the most recent
GW-59 form along with attached copies of the laboratory analyses.
d. Within sixty (60) days of completion of all monitoring well(s), the permittee shall submit two
original copies of a scaled site map (scale no greater than 1"=100'); however, special
provisions may be granted upon prior approval for large properties. The map(s) must include
the following information:
1) The location and identity of each monitoring well.
2) The location of major components of the waste disposal system.
3) The location of property boundaries within 500 feet of the disposal area(s).
4) The latitude and longitude of the established horizontal control monument.
5) The elevation of the top of the well casing (which shall be known as the "treasuring
point") relative to a common datum.
6) The depth of water below the measuring point at the time the measuring point is
established.
7) The location of Review and Compliance boundaries.
S) The date the map is prepared and/or revised.
Control monuments shall be installed in such a manner and made of such materials that the
monument will not be destroyed due to activities that may take place on the property. Maps
and any supporting documentation shall be sent to the Groundwater Section as addressed in
the "Reporting / Documentation" above.
The permittee is responsible for the geographic accuracy of any map submitted, however
produced.

For All Irri ation Facilities:
4. Vertical Separation Requirements:
a. Waste shall not be applied or discharged onto or below the land surface when the vertical
separation between the waste and the seasonal high water table is less than one (1) foot.
Verification of the water table elevation can be confirmed by water level readings obtained
from the monitor well(s) near the site or auger boring(s), which must be done within 24
hours, prior to application of wastewater. Any open borings must be properly filled with
native soil, prior to application, to decrease the chance of any waste contaminating the
groundwater.
5. Applicable Boundaries:
a. The COMPLIANCE BOUNDARY for the disposal system is specified by regulations in 15A
NCAC 2L, Groundwater Classifications and Standards. The Compliance Boundary for the
disposal system individually permitted on or after December 30, 1983 is established at either
250 feet from the waste disposal area, or 50 feet within the property boundary, whichever is
closest to the waste disposal area. An exceedance of Groundwater Quality Standards at or
beyond the Compliance Boundary is subject to remediation action according to 15A NCAC
2L .0106(d)(2).
b. The REVIEW BOUNDARY is established around the disposal systems midway between the
Compliance Boundary and the perimeter of the waste disposal area. Any exceedance of
standards at the Review Boundary shall require action in accordance with 15A NCAC 2L
.0106(d)(1).
6. Additional Requirements:
a. Any additional groundwater quality monitoring, as deemed necessary by the Division, shall
be provided.
V. INSPECTIONS
1. Adequate inspection, maintenance, and cleaning shall be provided by the Permittee to insure
proper operation of the subject facilities.
2. The Permittee or his designee shall inspect the wastewater treatment and disposal facilities to
prevent malfunctions and deterioration, operator errors and discharges which may cause or lead to
the release of wastes to the environment, a threat to human health, or a nuisance. The Permittee
shall keep an inspection log or summary including at least the date and time of inspection,
observations made, and any maintenance, repairs, or corrective actions taken by the Permittee.
This log of inspections shall be maintained by the Permittee for a period of three years from the
date of the inspection and shall be made available upon request to the Division or other permitting
authority.
3. Any duly authorized officer, employee, or representative of the Division may, upon presentation
of credentials, enter and inspect any property, premises or place on or related to the disposal site
or facility at any reasonable time for the purpose of determining compliance with this permit;
may inspect or copy any records that must be maintained under the terms and conditions of this
permit, and may obtain samples of groundwater, surface water, or leachate.

VI. GENERAL CONDITIONS
L This permit shall become voidable unless the facilities are constructed in accordance with the
conditions of this permit, the approved plans and specifications, and other supporting data.
2. This permit is effective only with respect to the nature and volume of wastes described in the
application and other supporting data.
3. This permit is not transferable. In the event there is a desire for the facilities to change
ownership, or there is a name change of the Permittee, a formal permit request must be submitted
to the Division accompanied by an application fee, documentation from the parties involved, and
other supporting materials as may be appropriate. The approval of this request will be considered
on its merits and may or may not be approved.
4. Failure to abide by the conditions and limitations contained in this permit may subject the
Permittee to an enforcement action by the Division in accordance with North Carolina General
Statute 143-215.6A to 143-215.6C.
5. The issuance of this permit does not exempt the Permittee from complying with any and all
statutes, rules, regulations, or ordinances which may be imposed by other government agencies
(local, state, and federal) which have jurisdiction, including but not limited to applicable river
buffer rules in 15A NCAC 2B.0200, erosion and sedimentation control requirements in 15A
NCAC Chapter 4 and under the Division's General Permit NCGO10000, and any requirements
pertaining to wetlands under 15A NCAC 2B .0200 and 2H .0500.
6. The Permittee shall retain a set of approved plans and specifications for the subject facility for the
life of the project.
7. The Permittee shall pay the annual administering and compliance fee within thirty days of being
billed by the Division. Failure to pay the fee accordingly may cause the Division to initiate action
to revoke this permit as specified by 15 NCAC 2H .0205 (c)(4).
8. The Permittee, at least six (6) months prior to the expiration of this permit, shall request its
extension. Upon receipt of the request, the Commission will review the adequacy of the facilities
described therein, and if warranted, will extend the permit for such period of time and under such
conditions and limitations as it may deem appropriate.
