Carolina 28398 Dee Freeman Secretary Subject: Permit No. WQ0023310 Warsaw Sanitation Trailer Wash Facility Wastewater Irrigation System Duplin County Dear Mr. Westerbeek: In accordance with your permit minor modification request received March 21, 2012, and site inspection performed by Division staff on April 5, 2012, we are forwarding herewith Permit No. WQ0023310 dated April 27, 2012, to Murphy -Brown, LLC for the continued operation of the subject wastewater treatment and wastewater irrigation facilities. The subject permit has been modified to reflect a change in the Waste Utilization Plan (WUP) cropping sequence. Please note this condition (Condition III. 9.) has been revised such that future minor changes to the WUP will not require pen -nit modification, only submittal of the new WUP to the Division. Additionally, the permit has been modified to require continuous flow monitoring (see Attachment A and Condition IV. 3.). Recent NDMR data indicate the facility is already monitoring continuously via a flow meter. Please pay particular attention to the monitoring requirements listed in Attachments A, B and C. Failure to establish an adequate system for collecting and maintaining the required operational information shall result in future compliance problems. During the site inspection, it was not clear whether the existing monitoring network is located correctly at the review boundary. Division staff directed Murphy -Brown to perform a monitoring well location determination and submit the results to the Wilmington Regional Office. As such the following permit condition is new: ➢ Condition I.I. —requires an updated site plan with actual monitoring well locations shown in relation to compliance and review boundaries within 60 days, installation of new monitoring wells if existing wells are not properly located, and abandonment of any damaged and/or improperly placed wells within 120 days. If any parts, requirements or limitations contained in this permit are unacceptable, the Permittee has the right to request an adjudicatory hearing upon written request within 30 days following receipt of AQUIFER PROTECTION SECTION 1636 Mail Service Center, Raleigh, North Carolina 27699-1636 Location: 512 N.
