Dear Mr. Schmidt:
Dee Freeman
Secretary
Subject: Permit No. WQ0023310
Warsaw Sanitation Trailer Wash Facility
Wastewater Irrigation System
Duplin County
In accordance with your permit application major modification request received June 27, 2011,
and subsequent additional information received July 14, 2011, we are forwarding herewith Permit No.
WQ0023310 dated August 11, 2011, to Murphy -Brown, LLC for the continued operation of the subject
wastewater treatment and wastewater irrigation facilities.
The subject permit has been modified to reflect the proposed replacement of the passive grit
separator with a mechanical vibratory screen separator which will include the addition of a temporary
storage basin, two pumps, and a mixer.
A review of the recent monitoring history of this facility has disclosed a large number of
violations associated with the timely submission of NDMR and NDAR-1 Forms, missing parameters,
missing data, measurement frequency violations, and high levels of nitrate nitrogen and fecal coliform in
the groundwater monitoring samples. The timely and complete submission of monitoring data is a
permit requirement and failure to meet these conditions can result in the Division taking necessary
enforcement action.
On August 5, 2009, Session Law 2009-406, entitled "An Act to Extend Certain Government
Approvals Affecting the Development of Real Property Within the State," was enacted by the General
Assembly and signed into law. The Act, known as the Permit Extension Act of 2009, extends the
expiration date of certain government approvals and permits. On August 2, 2010, this Act was extended
an additional year. The subject permit falls within the scope of this Act and is therefore being
extended to August 31, 2014 and shall be subject to the conditions and limitations as specified
therein.
This permit shall be effective from the date of issuance until August 31, 2014, shall void Permit
No. WQ0023310 issued August 3, 2007, and shall be subject to the conditions and limitations as specified
therein. Please pay particular attention to the monitoring requirements listed in Attachments A, B and C
for they may differ from the previous permit issuance. Failure to establish an adequate system for
collecting and maintaining the required operational information shall result in future compliance
problems,

the effluent, the Permittee shall take immediate corrective actions including Division required actions,
such as the construction of additional or replacement wastewater treatment or disposal facilities.
2. This permit shall not relieve the Permittee of their responsibility for damages to groundwater or
surface water resulting from the operation of this facility.
