In accordance with an email request received on December 27, 2007, we are forwarding herewith corrected
Permit No. WQ0022804, issued February 1, 2008, for the subject facility. This permit shall be effective from
the date of issuance until September 30, 2008, shall void Permit No. WQ0022804 issued April 10, 2007, and
shall be subject to the conditions and limitations as specified therein,
The following corrections have been made to the permit:
1. The description of the permitted facility has been changed, replacing "dual recirculating 20 square
foot Advantex — AX20 Filter" to "a single recirculating 20 square foot Advantex — AX20 Filter".
Please replace page 1 of the existing Permit No. WQ0022804, issued April 10, 2007, with this corrected
page.

In accordance with the provisions of Article 21 of Chapter 143, General Statutes of North Carolina as
amended, and other applicable Laws, Rules, and Regulations
PERMISSION IS HEREBY GRANTED TO

FOR THE
construction and operation of a 480 gallon per day (GPD) drip irrigation wastewater treatment and
disposal system consisting of a 1,500 gallon baffled septic tank with effluent filter, a single recirculating
20 square foot Advantex - AX20 Filter with VeriComm control panel, a 7 GPM ultraviolet disinfection
unit (International Water -Guard model IWG 1 -LV -WW -3) capable of sterilizing flow rates up to 10 GPM
as long as the effluent quality treated by the unit does not exceed 15 mg/1 BOD and 15 mg/l TSS, a 2,500
gallon storage and dosing tank, a 20 GPM pump operating at 110 feet of TDH, audio and visual high
water alarms, rain sensor, and 12,960 square feet of drip irrigation area with approximately 4,320 linear
feet of drip line containing 2,160 emitters, and all associated piping and appurtenances, to serve the Grant
Sohm & Kevin Elder residence at 4 Centennial Court in Apex, North Carolina with no discharge of
wastes to the surface waters, pursuant to the application. received April 4, 2007, and in conformity with
the project plan, specifications, and other supporting data subsequently filed and approved by the
Department of Environment and Natural Resources and considered a part of this permit.
This permit shall be effective from the date of issuance until September 30, 2008, shall void
Permit No. WQ0022804 issued on October 24, 2003, and shall be subject to the following specified
conditions and limitations;
I. PERFORMANCE STANDARDS
Upon completion of construction and prior to operation of this permitted facility, a certification
must be received from a professional engineer certifying that the permitted facility has been
installed in accordance with this permit, the approved plans and specifications, and other
supporting materials, If this project is to be completed in phases and partially certifiedyou
shall retain the responsibili1y to track further construction =roved under the same permit. and
shall provide a final certificate of completion once the entire project has been completed. Mail
the Certification to the Land Application Unit, 1636 Mail Service Center, Raleigh, NC 27699-
1636.
2. The Raleigh Regional Office, telephone number [phone], shall be notified at least forty-
eight (48) hours in advance of operation of the installed facilities so that an in-place inspection
can be made. Such notification to the regional supervisor shall be made during the normal
