During a routine quality control/quality assurance check of recently -issued permits, it was
discovered that the Permit No. WQ0021402, dated November 8, 2002, to the Town of Franklinville for
the subject residuals land application program was erroneously distributed without the inclusion of
approved vicinity and site buffer maps.
Enclosed please find copies of a vicinity map of the residuals source -generating facility. This map
should be inserted after Attachment A of the permit. Also, please find enclosed vicinity and buffer maps
of the approved land application sites. These maps should be inserted after Attachment B of this permit.
We apologize for this omission and appreciate your assistance in resolving this issue. if you need
