The following corrections have been made to the permit: Condition IV.10., regarding maintenance log for storage pond and spray site is being removed. Attachment A — Limitations and Monitoring Requirements Chlorine, Total Residual-50060, is being removed from the monitoring parameter for the PPI 001. 2. The 250,000-gpd monthly average flow Iimit is being removed. Footnote (b) is being added to clarify the timeframe for which the turbidity daily maximum is evaluated. Note that your interpretation for footnote (c) on Attachment A (regarding the sampling requirement for PPI 002) is correct, Attachment -Approved Bulk Fill Reclaimed Water Uses 1. The phrase "not washing" associated with the sewer cleaning use is being removed. 2. The phrase "not street cleaning" associated with street sweeping is being replaced with phrase "not street washing" in accordance with paragraph (b) of Rule 2T .0910, 3. Sewer cleaning and street sweeping uses are being moved into the second category (For use by certified bulk reclaimed water users). Please note that Fire fighting and reclaimed water distribution line pressure testing still remain in the first category ("For use by Permittee..."). The Permittee shall develop and maintain a program of routine review and inspection of reclaimed water users. 11. The residuals generated from these treatment facilities must be disposed / utilized in accordance with 15A NCAC 02T .1100. The Permittee shall maintain a residual management plan pursuant to 15A NCAC 02T .0914. 12. Should the limit for fecal coliform (daily maximum concentration of 25 per 100 ml) or turbidity (instantaneous maximum of 10 NTU) be exceeded, all wastewater shall be handled in one of three ways, until the levels have stabilized below these maximum levels. The City of Raleigh may further treat the wastewater to bring it into compliance with reclaimed water standards without returning the water to the facility's headworks, may reroute it to the headworks of the Little Creek Wastewater Treatment Facility for treatment to reuse standards, or may discharge it in strict accordance with the conditions of the facility's existing NPDES permit. 13. The Permittee shall implement adequate measures to prevent reclaimed water runoff from the application areas and points of use served by the Little Creek Reclaimed Water Utilization System and all associated facilities. IV. MONITORING AND REPORTING REQUIREMENTS Any monitoring (including groundwater, surface water, soil or plant tissue analyses) deemed necessary by the Division to insure surface and ground water protection will be established and an acceptable sampling reporting schedule shall be followed. 2. All laboratory analyses for effluent, ground waters, or surface waters shall be made by laboratory certified by the Division for the required parameters under 15A NCAC 02H .0800. 3. The effluent from the subject facilities shall be monitored by the Permittee at the frequency(ies) and location(s) for the parameter(s) specified in Attachment A. 4. Upon installation of a continuous flow meter, The Pennittee shall maintain an appropriate flow measurement device consistent with approved engineering and scientific practices to ensure the accuracy and reliability of flow measurement. Flow measurement devices selected shall be capable of measuring flows with a maximum deviation of less than 10 percent from true flow, accurately calibrated at a minimum of once per year, and maintained to ensure that the accuracy of the measurements is consistent with the accepted capability of that type of device. The Permittee shall keep records of flow measurement device calibration on file for a period of at least three years. At a minimum, data to be included in this documentation shall be: a. Date of flow measurement device calibration, b. Name of person performing calibration, and c. Percent from true flow. The Permittee shall maintain adequate records for bulk reclaimed water distribution. These records shall include, but are not necessarily limited to, the following information.: a. Date and time of bulk distribution event, b. Name and drivers license number of truck operator, c. Bulk fill certification number or equivalent (proof of certification provided to reclaimed water User), d. Name of end customer, e. Intended use of the bulk reclaimed water, f. Number of gallons provided at each bulk distribution event, g. Turbidity measurement results, and h. Total volume of bulk reclaimed water distributed each month. 7 6. Should the quality of the reclaimed water sampled from the points of use deviate substantially from the maximum daily limit, described in Attachment A, for each reclaimed water parameter sampled, the Permittee shall notify the Division and shall investigate why the deviation is occurring and take any steps necessary to correct the situation. 7. The Permittee shall maintain adequate records of the status of approved users. These records shall include, but shall not be limited to, the following: a. Locations of all users and/or use areas; b. Names and approval status of users and use areas; c. Compliance status of all users; and d. Measured or estimated volumes of reclaimed water by users or use areas. Three (3) copies of all monitoring data [as specified in Conditions IV.3] on Form NDMR-1 shall be submitted monthly on or before the last day of the following month. All records maintained in accordance with Condition IV.8. shall be kept for a minimum of five years and shall be made available to the Division upon request. 10. Noncompliance Notification: The Permittee shall report by telephone to the Raleigh Regional Office, telephone number (919) 791-4200, as soon as possible, but in no case more than 24 hours or on the next working day following the occurrence or first knowledge of the occurrence of any of the following: a. Any occurrence at the wastewater treatment facility which results in the treatment of significant amounts of wastes which are abnormal in quantity or characteristic, such as the dumping of the contents of a sludge digester; the known passage of a slug of hazardous substance through the facility; or any other unusual circumstances. b. Any process unit failure, due to known or unknown reasons, that render the facility incapable of adequate wastewater treatment such as mechanical or electrical failures of pumps, aerators, compressors, etc. c. Any failure of a pumping station, sewer line, or treatment facility resulting in a by-pass directly to receiving waters without treatment of all or any portion of the influent to such station or facility. d. Any time that self -monitoring information indicates that the facility has gone out of compliance with its permit limitations. Occurrences outside normal business hours may also be reported to the Division's Emergency Response personnel at telephone number (800) 662-7956, (800) 858-0368, or (919) 733-3300. Persons reporting such occurrences by telephone shall also file a written report in letter form within five (5) days following first knowledge of the occurrence. This report must outline the actions taken or proposed to be taken to ensure that the problem does not recur.
