Dear Mr. Deacon:
Enclosed are your approved plans and specifications for recently issued permit numbers
WQ0020021 and WQ0020023 that should have been included with your permits. These approved
plans and specifications should be provided to your wastewater system installer. For any major
changes to system components or spray field location, a permit modification application must be
submitted and approved by the Division before construction begins. Once construction is complete,
you should receive a copy of the record drawings. These drawings must be retained for the life of the
facility and should be transferred to any subsequent property owners.
On an additional subject, our records indicate you are the Permittee for two other single family
wastewater treatment and disposal systems. Please note Condition V(3) which indicates the permit
is not transferable. If these properties have been sold, a change of ownership request and
accompanying documentation must be submitted to the Division, similar to what was submitted for
your Cub Creek Estate lots. If you are still the present owner, note that WQ0013853 expires June
30, 2002. Condition V(10) requires a renewal application be submitted at least six months prior to
expiration of this permit. Our records do not reflect that a renewal request has been submitted.
