This certification is being issued to modify Attachment B to: ♦ Approve land application sites owned by various landowners in Harnett County for land application of residuals under this program. These sites, totaling 211.50 net acres, are transferred from the Town of Erwin's residuals Iand application program (i.e., Permit No. WQ0006119)and are spread over eight sites (i.e., Site Nos. NC -HA -307-04, NC -HA -307-05, NC -HA -307-06, NC -HA -307-07, NC -HA -307-08, NC -HA -307-09, NC -HA -307-10, and NC - HA -307-11). Please (replace the existing Attachment B of Permit No. WQ0019098 that was previously certified on December 20, 2004, with this modified Attachment B. Note that only the vicinity and buffer maps for any new and modified residuals source -generating facilities and land application sites are attached to the modified Attachment A and Attachment B. It is the Permittee's responsibility to ensure that a complete permit is created by inserting the modified Attachment A, Attachment B, as well as their respective vicinity and buffer maps, into Permit No. WQ0019098, issued on June 1, 2001, and making sure to retain any vicinity and buffer maps for any existing and unmodified residuals source -generating facilities and land application sites. The modified Attachment B shall be effective from the date of certification; shall void the Attachment B of Permit No. WQ0019098 that was previously certified on December 20, 2004; and shall be subject to the conditions and limitations as specified therein and well as those specified in the above - referenced permit. As always, remember to take the time to review the modified Attachment B thoroughly, as some of the conditions contained therein may have been changed since their last certification. Note that the modified Attachment B is hereby incorporated by reference into Permit No. WQ0019098, issued on June 1, 2001. This request shall be in the form of a written petition, conforming to Chapter 150B of the North Carolina General Statutes and filed with the Office of Administrative Hearings at 6714 Mail Service Center, Raleigh, North Carolina 27699-6714.
