Dear Mr. Tillage:
In accordance with your permit modification application package received on May 1, 2002, we
are forwarding herewith a modified Attachment B, certified November 12, 2002, to the Town of Holly
Springs for the subject residuals land application program.
This certification is being issued to modify Attachment B to:
♦ Delete land application sites owned by Mr. Ed Coffman and Mr. Eddie Gardner. These sites, totaling
180.5 net acres, are no longer approved to receive residuals as these landowners have chosen not to
continue participating in this residuals land application program.
♦ Approve land application sites owned by various landowners in Harnett County for land application
of residuals under this program. These sites, totaling 161.24 net acres, have been transferred from a
residuals land application program for which the permit has been rescinded by the Division of Water
Quality (i.e., Permit No. WQ0006298 issued to Swift Textiles, Inc.) and from the Town of Apex
Residuals Land Application Program (i.e., Permit No. WQ0001060). Note that Permit No.
WQ0001060 is currently going through an administrative amendment to remove the transferred sites.
♦ Renumber all approved land application sites, per your residuals management company's request,
according to Synagro South, Inc.'s standard site numbering/identification system.
Please replace the existing Attachment B of Permit No. WQ0019098 that was previously
certified on June 1, 2001, with this modified Attachment B. Note that only the vicinity and buffer
maps for any new and modified land application sites are attached to this modified Attachment B. It is
the Permittee's responsibility to ensure that a complete permit is created .by inserting this modified
Attachment B and vicinity and buffer maps into Permit No. WQ0019098, issued on June 1, 2001, and
making sure to retain any vicinity and buffer maps for any existing and unmodified land application sites.

This modified Attachment B shall be effective from the date of certification; shall void
Attachment B of Permit No. WQ0019098 that was previously certified on June 1, 2001; and shall be
subject to the conditions and limitations as specified therein as well as those specified in the above -
referenced permit. As always, remember to take the time to review this modified Attachment B
thoroughly, as some of the conditions contained therein may have been changed since the last
certification. Note that this modified Attachment B is hereby incorporated by reference into Permit No.
WQ0019098, issued on June 1, 2001.
If any parts, requirements, and/or limitations contained in this certification or modified
Attachment B are unacceptable, you have the right to request an adjudicatory hearing upon written
request within 30 days following receipt of the certification. This request shall be in the from of a written
petition, conforming to Chapter 150B of the North Carolina General Statutes and filed with the Office of
Administrative Hearings at 6714 Mail Service Center, Raleigh, North Carolina 27699-6714. Unless such
demands are made, this permit shall be final and binding.
